Dogs are living longer with advanced veterinar

medicine and excellent nutrition, but as they age,

things that were once easy now become an effort.

Chasing a ball becomes a stiff walk or jumping up

to your bed no longer happens and getting up from

the floor brings on a groan.

With this book we will help you to understand

everything you need to know about arthritis

starting from the basics and continuing with

explanations on why and how.

What is Arthritis?

Arthritis is very common disease both in humans and dogs. As lifespan grows - arthritis becomes more and more common. Arthritis is not a single disease, it is just a way to refer to joint pain or joint diseases. There are many types of arthritis. It can cause permanent joint changes either in one or more joints depending on the cause.

Osteoarthritis (OA) is the most common type of arthritis. It is a degenerative disease in dogs and is a major cause of disability. OA is mainly characterized by an activation of inflammatory processes at a molecular level, ultimately leading to a gradual deterioration of the cartilage. When cartilage - the natural cushioning surface on the both ends of bones - wears away, bones start to rub against each other. The resulting friction causes pain and loss of mobility. Inflammation in joint can also lead to development of bony growths called spurs.

What Causes Arthritis?

For a long time, it was believed that arthritis is caused by “wear and tear” of joints over time. However, now it is a disease of the joint. Development of OA can be contributed by many factors.

Weight can be large contributor to development of OA since being overweight puts additional pressure on hips and joints. Many years of excess weight inflicting extra pressure on cartilage will cause it to break down faster. Excess fat tissue also produces more inflammatory chemicals that contribute to damaging the joints.

Injury or overuse also lead to having an increased risk of OA. Repetitive movements or occurred injuries such as fractures, surgery or ligament damage can speed cartilage breakdown process. Most cases develop because of abnormal rubbing within the joint caused by joint instability after ligament damage, damage to or abnormal cartilage development, or damage caused by trauma such as fractures.

It is estimated that one in five dogs suffers from this debilitating disease, which significantly reduces their mobility and causes severe pain. Typically, arthritis is a problem seen in older dogs (problem for every second dog), but the condition can develop from an early age following problems with bone and joint development.

Know the Signs of Arthritis in Dogs!

Other factors can contribute to this condition as well. Various genetic traits can increase risk of development, different bone and joint disorders, metabolic disorders, or abnormal growing speed which interferes with cartilage development. Increased lifespan is a growing factor that leads to OA since cartilage use is extended.

Any joint in your dog’s body can be affected by OA, but usually OA develops in hips, elbows, knees, wrists, shoulders, ankles and lower back.

Since osteoarthritis is a progressive condition, the initial symptoms can be tricky to spot. Over time, you may find that your dog is less keen to be active. It may also have lameness in his limbs. These symptoms may get worse when the dog is inactive for long periods of time or in the colder, damper weather.

The clinical signs of dog arthritis usually appear gradually and slowly worsen over time. The first joint pain dog symptoms are often so mild that the owners often miss them. So how do you recognize Arthritis in a dog?

How to Diagnose?

It is best to seek help of your veterinarian to diagnose arthritis. Since arthritis may not be the only cause of the symptoms - only veterinarian can make diagnosis of osteoarthritis.

Your veterinarian might:

• Look at thorough medical history including when the current symptoms started and if they have increased in intensity.

• Perform a Physical examination paying attention to its limbs, back and joints. Veterinarian will be looking for swelling, heat and signs of discomfort.

11WHAT YOU SHOULD KNOW ABOUT DOG ARTHRITIS

• Perform radiograph analysis (X-ray). This is very effective tool for identifying and assessing arthritis. It can show changes in the joint capsules, soft tissue thickening, narrowing of joint spaces, joint fluid build-up, cartilage changes, bone changes, miner- alization of soft tissues, intra-articular calcified bodies (osteophytes) and other physical changes that are known to be associated with arthritis.

• Perform analysis of the joint fluid to understand the severity of disease.

• In some cases, magnetic resonance imaging (MRI) and computed tomography (CT) are performed as well to get additional information about arthritis.

• Infectious arthritis caused by bacteria (such as those trans- mitted through tick bites), viruses, fungi or other organisms.

• Immune-mediated arthritis (cells of the body's immune system damage joint cartilage).

• Cancer, typically involving the bone or cartilage.

• Conditions that cause muscle weakness or lethargy (such as thyroid disease in dogs).

• Injuries to muscles, tendons or ligaments.

12WHAT YOU SHOULD KNOW ABOUT DOG ARTHRITIS

Could It Be Something Other Than Arthritis?

Different medical conditions can cause the same or similar medical conditions as arthritis. Therefore, it is important to find the right diagnosis before starting the treatment.

There are some signs such as inactivity that can be associated with many different diseases. But even more specific signs, such as periodic limping or a decrease in jumping, can be associated with other medical conditions including:

To make sure that you know the right diagnosis and thus can decide the right treatment for your dog - consult a veterinarian who can further help you with providing different treatment options.

Unfortunately, currently there is no definite cure for

osteoarthritis and most treatment regimens focus

on symptom management and pain reduction

through prescription drugs and the use of

supplements. Since osteoarthritis is a progressive

condition, you should act to prevent the speed of its

development.

However, some treatments can help to regenerate

some of the already existing damage.

Drugs for Arthritis

Usually your veterinarian will prescribe nonsteroidal anti-inflammatory drugs or NSAID’s to help reduce swelling, stiffness and joint pain. NSAID’s usually are safe to use but there are some concerns such as kidney, liver or digestive problems. These drugs work by blocking inflammatory mediators, so they will help with pain, but they will also slow down healing process. If your dog experiences behavior changes, eating less, skin redness, diarrhea or vomiting - you should immediately stop giving NSAID’s to your dog and call veterinarian.

In case your dog doesn’t respond to the usual medication, your vet might prescribe other painkillers that are usually stronger with larger risk of side effects. Keep in mind that NSAID’s can cause issues in other parts of the body even if they help with the inflammation.

Supplements for Arthritis

There are alternative treatments such as taking supplements like glucosamine, chondroitin, astaxanthin. Research shows that they can help reduce swelling and protect the remaining cartilage.

Glucosamine will help to create new molecules that form the cartilage of your dog’s joints. It can help with boosting or repairing lost joint fluid.

Chondroitin can repair connective tissue, keep cartilage tissue hydrated and assist in cushioning impact stress.

Astaxanthin relieves pain and inflammation and cleans the cells of the body. It can block and handle several different chemicals that create pain.

Surgical Alternatives for Arthritis

In cases, where no other method has helped, surgical intervention can be considered. Since there are different joints that can suffer from arthritis there are also different surgeries involved. Potential surgeries might be arthroscopic surgery, joint replacement, joint repair and joint fusion, and more treatments are under development all the time. You must keep in mind that if your dog is overweight or old – then he is most likely a bad candidate for surgical treatment since the risk of surgery is high in such patient.

Arthroscopy is a minimally invasive joint surgery that allows veterinarian to inspect the affected joint more thoroughly. Veterinarian will make a few small incisions through which an arthroscope will be inserted into the joint. Arthroscope is a small instrument that contains fiber-optic cables and specialized lighting that illuminates joint structure. It also contains lenses that magnify the illuminated joint structures for a better assessment. The acquired pictures are sent to the TV monitor through which surgeon can examine the joint in detail and decide on the next steps needed for the best recovery.

Joint replacement has been available for dogs for quite some time already. During the surgery the diseased joint is cut out and replaced by prosthetic joint. Usually this is the last approach to help with joint problems because of the risks involved. Some risks are loosening of the implant, failure of the supporting ligamentous structures and infection.

17MANAGING ARTHRITIS

Recovery time depends on how severe the condition of the joint is prior to surgery. To prevent damage to the new joint implant usually the first 7 to 10 days after surgery dogs should engage in rehabilitation program. After that patents are placed on a recovery program that consists of careful exercise. The precautions are usually necessary up until 12 weeks after the surgery.

However, if your dog hasn’t shown any positive response to other treatments and is still suffering from pain – joint replacement surgery might be the right call. In most cases joint replacement leads to pain-free function.

Joint Fusion or Arthrodesis is permanent joining of the bones that form the affected joint. This is done to restore acceptable limp function in cases where no other solution is possible. Cartilage of the joint is removed, and the joint is permanently fused together. Metal inserts are placed to secure the healing position of the joint. Once the bones are healed together, your dog will feel significantly less pain. The recovery time after joint fusion usually is up to two months. This is also when restriction plan must be followed. The activities that are not allowed during recovery time are any free activities (playing, jumping, running or long walks), no stairs, no outdoor dog runs and no slippery floors. Your dog should be kept in confinement for this period of time.

Coconut Oil

It has been found that coconut oil helps to relieve joint pain among canine arthritis sufferers, which is welcome news for many pet owners. Coconut oil is also used widely to improve canine metabolic function and help with weight loss.

The coconut oil is made up of a mixture of saturated, monounsaturated and polyunsaturated fats. Together these 3 fats contain an impressive list of such fatty acids as lauric acid (48%), caprylic acid (8%), capric acid (7%), linoleic acid (2%) and oleic acid (6%). These fatty acids provide anti-inflammatory, anti-fungal, anti-bacterial properties and they fight off viruses. Additionally, coconut oil components aid in digestion, improve brain energy and mental function in older dogs, reduces coughing and it helps to eliminate hairballs. It can also lead to aesthetic improvements such as glossier coat and removing of dog odor. But the most important thing - coconut oil can help arthritis and bone health.

Studies have shown that CBD oil can have powerful pain-relieving and anti-inflammatory effects, thus helping your dog with arthritis symptom treatment.

CBD is one of the two most known cannabidiols that a cannabis plant contains. Unlike THC, that is psychoactive, CBD doesn’t have any psychoactive properties. Research suggests that CBD is safe and far better tolerated than standard medications.

CBD can reduce inflammation throughout whole body - including joints and it can reduce swelling as well. It also provides pain relief.

However, each location has different regulation about CBD oil. Mostly it is not regulated, but you must keep in mind that some regulations have restrictions on how CBD was obtained or how much THC residue is present in CBD oil.

• Improve the Quality of Bedding

Give a well-padded bed to your dog, away from cold and damp drafts, and if possible, add ramps so that your dog can easily get into and out of his bed. Also, try to opt for slip resistant flooring, wherever possible.

• Massage and Warm Compress

Give your dog a good body massages occasionally, to help stimu-late blood flow. You can also apply a warm compress over sore joints — the heat will help relieve aching joints. Use a hot water bottle or blanket for best results, but make sure the compress you are giving is not too hot — you don’t want them to get burned!

Other Natural Ways of Helping Your Dog

By taking a proactive approach to preserving the integrity and function of your dog’s ligaments, tendons, and joints throughout life, you may be able to avoid the need for invasive procedures. Here are some therapies and remedies that might help.

• Exercise Your Dog

Allow your dog to exercise moderately every day to keep muscles active; however, be careful not to stress them too much. Too much exercise can lead to pain, whereas too little will make the arthritic condition of your dog worse. You can judge from the way they interact and move after exercise. Try to avoid repetition in exercising.

Too much exercise can lead to pain, whereas too little will make the arthritic condition of your dog worse. You can judge from the way they interact and move after exercise. Try to avoid repetition in exercising.

For arthritic dogs, you can always begin with a nice slow warm-up, like a low-impact short walk. Gradually, allow them to move a bit faster once they’ve warmed up for a minute or two.

Other popular exercise choices for arthritic dogs include swimming, leash walking, short hikes (go for trails with low inclines), and little indoor play sessions.

After all, you want your pet to be happy and in shape. (The idea is to help them with mobility, not tire them out).

In addition, you can also start by giving them natural food products and supplements, physiotherapy, acupuncture, etc. for arthritis treatment.

Maintaining a healthy weight will help to slow down arthritis development as well since the amounts of stress applied to joints will be smaller.

• Give the Right Diet to Your Dog

Keep in mind that nutrition plays a very important role when it comes to maintaining the quality of life for your dog, especially when they’re struggling with arthritis.

You should know that there are six essential nutrients that your dog needs to get a healthy diet. You should pay attention if your dog’s diet includes enough water (approximately 50-60 ml of water per kilogram / 0,5 to 1 ounce per 1 pound), protein, fat, carbohydrates, vitamins and minerals. All these nutrients are essential for basic functions of the body. Some specialists recommend feeding your dog a diet that has the proportions of 5-parts meat with bone, 1-part organ meat and 1-part plant matter.

From meat with bone you can give your dog chicken, lamb, pork, duck, rabbit, kangaroo, beef, turkey and fish. Combine 5 parts meat bone with 1-part organ meat like liver, kidney, spleen, pancreas, brains, tripe or heart and 1-part plant matter like carrots, peas or green beans for best results.

You should avoid certain foods like grains, potatoes, peppers, and tomatoes. They are known to increase arthritis-related inflammation.

On the other hand, foods like celery, alfalfa sprouts, apple cider vinegar (raw organic with the mother of vinegar), mangos, blueberries and papayas are all very good for dogs with arthritis. Feed your dog fresh meat with bones, because cartilage and bone are packed with calcium, glucosamine, chondroitin and other vital minerals that can help with joint health.

Regenerative medicine applies to any medicine that

can help you with repairing, replacing or

regenerate damaged tissue.

It is the combination of physical therapies, right

nutrition, supplementation, and exercise that can

go a long way toward keeping your dog active, agile

and happy. And, while arthritis can’t ever be

completely cured, these remedies for dog arthritis

can help maintain your dog’s quality of life and,

with the help of regenerative stem cell therapies

there is even a possibility to stop the progression of

this degenerative disease.

Therefore, there is an urgent need for alternative

therapy with effective disease-modifying effects.

Stem Cell Therapy

Animal stem cell therapy, in the form of mesenchymal stem (or stromal) cells (MSCs), has recently provided a new paradigm for treating chronic arthritic dogs from symptom management to stimulating regeneration of bones and cartilage, resulting in considerable improvements in quality of life.

Animal stem cell therapy uses stem cells from either your pet (known as autologous stem cell therapy) or another animal of the same species (known as allogeneic stem cell therapy). A veterinarian injects the stem cells into the diseased area, such as a knee joint damaged by osteoarthritis.

28REGENERATIVE MEDICINE

Page 29: Dog Arthritis Guide - Medrego · 2019. 4. 9. · Step-by-step support guide for dog owners who want to avoid or treat arthritis. 2 ... it was believed that arthritis is caused by

How Stem Cell Therapy Works?

The injected stem cells move to the inflamed or damaged tissue, suppress the inflammation, relieve pain, and cause new tissue to grow. Stem cells can differentiate into specialized cells and are able to regenerate tissues such as cartilage, bone, tendon and muscle. This new tissue is more like the original tissue than the scar tissue that would typically grow in an untreated inflamed area.

The industry has seen good results in cases for treatment of arthritis, osteoarthritis, a degenerative joint disease in the hip or elbow, hip dysplasia, calcifications, common degeneration, and inflammation with dog arthritis natural remedy. Research shows that approximately 80% of injections lead to significant improvements with no major complications.

If your dog does have a case of arthritis or some other degenerative joint disease or soft tissue injury, Stem Cell Therapy for dogs can not only help to deal with your four-legged friend’s pain, but it might also stop the progression of the disease or condition, improving the quality of life of your fluffy friend and, thereby, improving yours as well.

• successful animal recovery after soft tissue injuries, like tendon and ligament ruptures in dogs and horses;

• for joint problems, like arthritis, osteoarthritis, hip dysplasia, joint inflammation in dogs and horses;

• for an energy boost in any animal (for immunomodulation pur- poses).

There are two types of stem cell treatments – allogeneic (donor-derived) and autologous (own-derived) stem cell therapies.

Both therapies have shown very good results and efficiency in animal treatments, mostly in dogs (arthritis cases) and horses (tendon or ligament cases). One could debate and argue about which one of them is better. But there is no doubt that both these treatments are safe and cause no side-effects if applied properly.

In veterinary medicine, both allogeneic and autologous stem cell treatments are being used for:

Autologous vs Allogeneic Stem Cell Therapy

Page 32: Dog Arthritis Guide - Medrego · 2019. 4. 9. · Step-by-step support guide for dog owners who want to avoid or treat arthritis. 2 ... it was believed that arthritis is caused by

However, lately, allogeneic stem cell treatments have been gaining more and more popularity across the globe. And there are a few reasons behind that.

First, they are ready to go. Allogeneic stem cell availability is one of the reasons why it is gaining such popularity. Autologous stem cell therapy takes about 2–3 weeks since stem cells need to be extracted from the injured animal. Allogeneic stem cell therapy, on the other hand, can be ordered and delivered to the patient in 2 to 4 days, since the treatment containing the donor’s stem cells is already made and ready for use.

Secondly, it might have a better efficiency rate. Allogeneic stem cells have been derived from young, healthy and powerful donor animal to ensure the best rates for success. Patient’s own stem cells used for allogeneic treatment might not be as active and viable thus decreasing potential benefits.

Third, allogeneic stem cell treatment is a non-invasive procedure for a patient animal. Although autologous stem cell therapy preparation requires only minimally invasive procedure to the patient – it still is an extra procedure that puts animal into stress.

And the last, but not the least – affordability. Simply because allogeneic stem cell treatments use donor-derived cells, it is way easier to produce more stem cell injections (treatment doses) from these cells. Thus, a lower price is possible for the same treatment.

PRP or Platelet – Rich Plasma Injections has been used since 1987 to promote cell regeneration. In PRP therapy specialists use patients own plasma. Plasma is the liquid component in your blood and contains growth factors that can promote healing. Plasma is mostly water, but it also contains proteins, nutrients, glucose, antibodies and other components.

PRP contains separated platelets from blood plasma. Platelets are a component of blood that secrete growth factors and proteins that regulate cell division, stimulate tissue regeneration, and promote healing.

PRP is made from a small amount of blood. With the use of special machine, platelets are separated.

The result is a concentrated injection with growth factors, that can help to repair damaged cartilage, tendons, ligaments, muscles and in some cases – bones. The platelets produce lubricants that suppress inflammatory factors and decrease pain.

Stem Cell therapy and platelet-rich plasma (PRP) are two major treatments in managing OA. Both are under regenerative medicine branch with the ability to repair and rejuvenate body’s damaged tissue.

PRP provides stimulation for improving quality and speed in providing a local healing response to an injury. Stem Cell Therapy provides the ability of tissue regeneration. PRP is usually great for treating injuries that are slow to heal, while Stem Cell Therapy provides a better and faster healing while excreting proteins that regulate local environment.

The Stem Cell Treatment process is more involved than PRP. However, the benefits are usually much more powerful.

It is possible to combine both treatments to achieve the best results in treating OA. It is believed that PRP can maximize the effects of stem cell healing properties.

PRP can assist stem cells in locating the right tissue where to adhere. Platelets from PRP provide scaffold for stem cells to build on. Since all the healing elements in your body travel through blood, it makes sense to use blood derived PRP Plasma as a transport mechanism for Stem Cells.

• Irritability

• Aggression when joints are touched

• Appetite loss

• Abnormal stance when walking (pelvis tucked under; using hind legs with exaggerated care)

The most common joint disorder in dogs is Osteoarthritis, but your dog also could suffer from developmental problems such as hip or elbow dysplasia, cruciate ligament problems, metabolic disease or other disease that cause similar symptoms as arthritis.

However, the most certain way to know if your dog is suffering from joint disorders is to go to your veterinarian and get additional information about your specific case.

If your dog’s symptoms and pain doesn’t go away in two weeks, you should go to your veterinarian. The sooner you will take care of this, the better chances you will have in managing your dog’s pain. The course of treatment will vary depending on the cause of pain and severity of condition.

Usually you can manage pain without needing a surgical treatment. Non-surgical treatment can include weight management, strengthening of muscles, supplement intake or different medications that will help to relieve joint pain and inflammation.

In some cases, surgical treatment must be carried out. This might include injections of different treatments, tissue replacement, arthroscopic cleaning of the joint or in severe cases – replacement of joint.

Finding the best joint supplements for your dog is not simple. It will require trial and error approach. Efficacy of any supplement is affected by dosage, frequency, treatment length and other factors. However just as with us, humans, every dog is different so the effect of using supplements that your dog will have will vary from dog to dog.

There are three main types of joint supplements for dogs.

The largest risk associated with stem cell therapy is just like with any other injection – there is a risk of infection if the treatment protocol is not followed. Our most popular product is Allogenic (from donor) stem cell therapy. Since this product is prepared from a donor animal – it doesn’t have any risk associated with collecting fat. The product is ready when you need it. It contains young and healthy stem cells, tested for all possible diseases and preparation is followed by our ISO certification protocol.

For Autologous (animal’s own cells) stem cell therapy the biggest risk is using a general anesthetic to remove the fat tissue from which we isolate the stem cells. Our partner clinics always carry out a full pre-anesthetic blood test and use the safest available anesthetics. Typically, the fat is collected in about 30 minutes.

In the thousands of animals treated across the world, Vets have not noted side-effects from stem cell therapy. There can be some swelling 1 or 2 days after injecting therapy, but it is completely normal and safe. It is a natural body response to an increased cell activity in the joint.

For more information on safety issues, please, visit our Science & Publications section and see many publications confirming safety.

Stem cell treatment will give your pet pain relief as soon as a few days to a few weeks after the injection with further improvement as healing progresses. The largest results are seen after approximately 60 days post treatment, but this may vary for each dog. To better understand how the results occur see the image below.1

Your doctor should also see the first improvements in US (ultrasound) after ~2 weeks.
