Who Can Use Dodge Park Rest Home Services

• Individuals who diagnosed with Alzheimer’s disease• Cognitive Impairment individuals• The frails and the elderly who need help with all or partial ADL• Limited mobility and 2 person assist individuals• Residents who required medication management• Individual who are potentially a wonderer• Residents who need catheter and oxygen• Individuals who ned Diabetic management• Individuals who might need special diet monitoring• People in the community who might need a better socialization and

stimulation during day and evening hours.

Who Can Not Use Dodge Park Rest Home Services

• Young adult with mental disabilities or• Young adult with aggressive behavior• Residents who required 24 Hours ON GOING SKILLED nursing

care*• Residents who required extensive level of nursing supervision

We will screen all potential referral seven days a week to ensure that all potential admission are a perfect fit to our unique model of care.

* Clearly define those prohibited condition for admission.

• Private Room $240 A Day• Semi-Private $205 A Day• Three Beds Room $195 A Day• Four Beds Room $185 A Day

Most long-term care insurance programs cover all or part of the cost of services provided by The Day Club.

Once all personal funds spent down residents qualify for EAEDC**

What is The Day Club?

• The Day Club is a supportive, social day program for elder men and women

• The Day Club provides elders with an alternative to staying home and gives caregivers the opportunity to leave loved ones while they work or run errands.

How is The Day Club

Different?• The Day Club is a private, social program

and provides an alternative to subsidized day care and day health programs.

• Respite care is also available in addition to extended hours on weekends.

• Unlike other day programs for seniors, The Day Club is open seven days a week from 7:00 a.m. to 7:00 p.m.

What Services are Available at

The Day Club?The Day Club offers its members:• Hot meals and snacks.• Activities, events, outing and entertainment.• Nursing supervision on site.• The opportunity to receive Physical Therapy,

Occupational Therapy or Visiting Nurse Appointments on site, subject to insurance approval.

Who Can Participate inThe Day Club?

• The Day Club is a private program,

open to seniors who want an alternative to staying home.• The program’s twenty-four hour nursing supervision allows

seniors who are:

* Mobility Impaired

* Have Some Diminished Cognitive Abilities

* Need Medication Reminders

* And Need Some Supervision with activities of Daily Living to participate in The Day Club.
