– New Medical Form for Other Household Members– Revised Medical Form for Caregivers– TB Test (Blood or Skin Test)– Medical Disqualification: MO endorses that person’s health condition poses a direct threat to the
health and safety of others. Direct threat can’t be eliminated by modifications in policy, practice or procedures.
– Ongoing Medical: Medical, cognitive or other condition that will affect child caring or just an on-going condition—requires annual evaluation.
