Out of State Ralph V. Estep, of Wilmington, Delaware, was assessed a civil penalty of $1,000 for practicing while not

being licensed to do so in this Commonwealth. (10/29/12)

Nancy J. Wolf, Pennsylvania license no. CA034886L, of Wilmington, Delaware, was issued a public

reprimand based upon disciplinary action taken by the licensing board of another state. (10/31/12)

PA Architects Licensure Board

Out of State Roger S. Lichtman, RA014286B, of Princeton, New Jersey, had his Pennsylvania license suspended

based on his failure to pay a previously imposed civil penalty. (07/19/12)

PA Board of Auctioneer Examiners

Mercer County Paul G. Kiss, license no. AU005419, of Greenville, Mercer County, was ordered to pay a civil penalty of

$500 because he violated the law regarding records of sales and engaged in conduct in connection with a

Butler County Craig A. Gehm, certificate no. RL003195L of Harmony, Butler County, was ordered to pay a civil

remedial education, because he failed to complete an appraisal assignment within the requirements of the

USPAP, failed or refused to exercise reasonable diligence in developing or communicating an appraisal or

preparing an appraisal report, and negligently or incompetently developed an appraisal, prepared an

PA Board of Chiropractic

Adams County John D. Soliday, license no. DC006640L, of Littlestown, Adams County, was assessed an civil penalty of

$577.50 and was indefinitely suspended retroactive to May 15, 2012, based on findings he is unable to

practice chiropractic with reasonable skill and safety due to his excessive drug use. (08/09/12)

Luzerne County Michael Joseph Spiccioli, license no. DC007645L of Kingston, Luzerne County, was suspended for no

less than 24 months immediately stayed in favor of probation subject to terms and conditions as set forth

in the agreement, was ordered to pay a civil penalty of $3,000 and had a public reprimand placed on his

permanent board record, because he was convicted of a felony, a misdemeanor in the practice of

chiropractic, or receiving probation without verdict, disposition in lieu of trial or an Accelerated

Rehabilitative Disposition in the disposition of felony charges, in the courts of this Commonwealth, a

Federal court, or a court of any other state, territory, possession or country; and was convicted of a

the profession because she used or maintained a place for the practice of cosmetology; used or maintained

a place for the practice of natural hair braiding without having first obtained a license or limited license as

provided by the Act; aided and abetted the unlicensed practice of a profession, occupation or business by

using or maintaining a place for the unlicensed practice of cosmetology or natural hair braiding; and aided

Beaver County Janet Furman Kowal, license no. CO188603L, of Beaver Falls, Beaver County, had a public reprimand

placed on her permanent disciplinary record with the board and was ordered to pay a civil penalty of

$1,000 because she provided cosmetology services while her license was lapsed. (11/02/12)

Berks County Jill C. Stuebner, license no. CO255747 of Mohnton, Berks County, was suspended for nonpayment of a

Sarah Marshall, license no. CO259587, of Robesonia, Berks County, was assessed a civil penalty of $500

until at least March 30, 2013 because at the time she sat for the exam on March 31, 2011, she was not of

Delaware County Christine M. Steif, license no. CO234213L, of Drexel Hill, Delaware County, had her license suspended

for nonpayment of a previously imposed civil penalty. (07/09/12)

Erie County Erin D. Johnson, license no. CO238411, of Erie, Erie County, had her license indefinitely suspended,

immediately stayed in favor of probation until she pays all costs, fees and fines imposed by the Court of

Franklin County Nail Glamour, license no. CY104499L, of Chambersburg, Franklin County, was ordered to pay a civil

penalty in the amount of $600 and costs of investigation in the amount of $63.48 because it failed to

comply with federal requirements concerning public safety and failed to maintain all equipment that

comes into contact with clients in a sanitary condition at all times (11/05/12)

Thien C. Luong, license no. CY194895, of Chambersburg, Franklin County, was ordered to pay a civil

penalty of $200 and costs of investigation of $37.30 because the salon was maintained in a dirty condition

at the time of inspection and the salon failed to maintain tools that come into contact with a client in a

Lackawanna County Melissa A. Gonzales, license no. CO275122 of Dunmore, Lackawanna County, was suspended for

suspended for a period of no less than three years and after 30 days of active suspension shall be stayed in

favor of probation, subject to terms and conditions as set forth in the agreement, because he engaged in

Alfred George Khallouf, license no. DS035075 of Allentown, Lehigh County, was issued a public

reprimand and placed on probation for a period of one year, based on findings he pled guilty to a

Northampton County Karen L. Burk, license nos. DH009836L and DHA001375, of Bethlehem, Northampton County, was

suspended for failing to pay a previously imposed citation in the amount of $350. (07/18/12)

Philadelphia County William Andrew Dodd, license no. DS025982L of Philadelphia, Philadelphia County, was ordered to pay

a civil penalty of $1,600 and a public reprimand was placed on his permanent disciplinary record with the

Board because he failed to renew his dental license on or after March 31, 2009 and practiced dentistry

with an expired dental license from April 1, 2009 through July 9, 2010. (10/19/12)

Out of State Fairfield Armistead Ward, license no. DS02669L, of Hampton, Virginia, was issued a formal reprimand

for having pled guilty to a misdemeanor involving moral turpitude. (08/10/12)

Michael Dean Webb, license no. DS024978L of Chesterfield, Virginia, was ordered to pay a civil penalty

of $1,000, because he had his license to practice dentistry or dental hygiene or certificate for expanded

PA Board of Professional Engineers, Land Surveyors and Geologists

Out of State Arthur Coston, Pennsylvania license no. PE062675, of Burlington, NJ, was suspended for failure to pay a

previously imposed civil penalty. (06/05/12)

Steven Robert Close, license no. PE042908R, of Evergreen, CO, permanently and voluntarily surrendered

his license because his license to practice as a professional engineer was disciplined by the proper

licensing authority of another state and he submitted a false or deceptive biennial registration to the board

on two separate occasions. (11/14/12)

David Bernarr Granger, license no. PE042930R, of North Canton, OH, was ordered to pay a civil penalty

of $500 and a public reprimand was placed on his permanent disciplinary record with the Board because

his license to practice as a professional engineer was disciplined by the proper licensing authority of

license to practice massage therapy and immediately placed on probation for not less than two years based

Allegheny County Jacqueline Martha Pringle, license no. MD030749E of Pittsburgh, Allegheny County, was actively

suspended for a minimum period of three years and until such time as she applies for and receives

approval from the board for the reinstatement to active status of her authorization(s) to practice the

profession as provided in the agreement, because she is unable to practice the profession with reasonable

separation, nor was the display properly lit when the business was open during evening hours, the

dealership, additionally, did not properly post its regular business hours in a conspicuous place for the

visiting public, failed to produce business records when an authorized agent of the board reasonably
