PA Board of Accountancy

Allegheny County Anna M. Baird, license no. CA016272L of Bridgeville, Allegheny County, was ordered to pay a civil

penalty of $5,000, her license to practice as a certified public accountant in this Commonwealth

suspended for five years, and a public reprimand placed on her record because she was suspended of the

right to practice before a federal governmental agency. (12/20/11)

Bucks County William J. Tobin, Jr., license no. CA028775L of Churchville, Bucks County, was ordered to pay a civil

penalty in the amount of $2,500 because he practiced on a lapsed license. (1/17/12)

Chester County Jeffreys & Associates, PC, license no. AF001624L of Exton, Chester County, was ordered to pay a civil

penalty in the amount of $1,500 because it practiced as an accountancy firm when it was not currently

Liberty Property Trust, license no. PX177073, of Malvern, Chester County, was ordered to pay a civil

penalty of $3,000, costs of investigation in the amount of $296.24, and suspended its approval to practice

as an accountancy program sponsor for a period of three years, such suspension immediately stayed in

favor of three years probation, because it failed to renew its approval to offer continuing education

Dauphin County Dena Beth Markowitz, license no. CA031530L of Harrisburg, Dauphin County, was ordered to pay a civil

penalty of $150 because she failed to complete at least 20 hours of continuing education during calendar

year 2009 in the biennial period from January 1, 2008 to December 31, 2009. (1/17/12)

Delaware County George E. Annett, license no. CA005688L of Havertown, Delaware County, had a public reprimand

placed on his permanent board record and the board agreed to the immediate permanent voluntary

surrender of his license and certificate to practice as a certified public accountant because he practiced as

a certified public accountant when he was not currently licensed and registered under the Act. (11/15/11)

Jefferson County McKinley & Co., PC, license no.AF054005 of Brookville, Jefferson County, was ordered to pay a civil

penalty of $7,000 and the cost of investigation of $477.30 because it practiced as a certified public

accountancy firm when it was not currently licensed and registered from January 2003 through September

Lackawanna County Michael Geoffrey Mellner, license no. CA014285L of Scranton, Lackawanna County, was ordered to pay

a civil penalty of $3,000; successfully complete an eight-hour continuing professional education course on

practice and records management within sixty (60) days of the board’s adoption of the consent agreement;

and his license suspended for a period of one (1) year, such suspension to be immediately stayed in favor

of probation, subject to certain terms and conditions because he failed to exercise due professional care in

Monroe County William Edward Livesey, Jr, license no. CA034263R of Stroudsburg, Monroe County, was ordered to pay

a civil penalty in the amount of $600 and, within six months of the effective date of this order, complete

40 hours overall of continuing professional education, because he did not complete the requisite 20 credit

hours of continuing professional education during calendar year 2008 in the biennial period and did not

complete the requisite 80 credit hours of continuing professional education during the applicable biennial

penalty in the amount of $300 as well as undergo an unsupervised period of probation, the duration of

which will be eight months beginning on the first day of the month following the board’s issuance of the

order, because he practiced as an architect during three specific biennial license renewal periods when he

because he failed to complete an appraisal assignment within the requirements of the USPAP; failed or

failed to meet the minimum qualifications for renewal of his certificate and knowingly made a false

statement, submitted false information or refused to provide complete information in response to a

revocation of his licenses because he displayed misconduct while carrying on the practice of chiropractic,

committed immoral or unprofessional conduct, willfully engaged in sexual activity with a patient and

committed an act involving moral turpitude, dishonesty or corruption. (01/26/12)

PA Board of Cosmetology

Allegheny County Kathy L. Crissman t/d/b/a Kathy Miller Beauty Salon, license nos. CB080973L and CO149099L, of Harwick,

Allegheny County, was ordered to pay a civil penalty of $1,000 because she, while trading and doing business

as Kathy Miller Beauty Salon, practiced cosmetology in a grossly incompetent manner through the

Hanover, York County, was ordered to pay a civil penalty of $2,800 because she practiced cosmetology in

a grossly incompetent manner through the use of razor tools and razor blades in the practice of

cosmetology; failed to maintain supplies, tools, instruments, and appliances in a sanitary condition; and

employed unlicensed individuals in the practice of cosmetology. (11/07/11)

PA Board of Dentistry

Fayette County Francis A. Jacobyansky, license no. DS023122L of Normalville, Fayette County, was ordered to pay a

civil penalty of $6,000 and a public reprimand was placed on his permanent disciplinary record with the

board because he employed a non-licensed dental hygienist and knowingly or negligently employed a

dental hygienist whose license or certificate was not current. (12/16/11)

York County Kevin John Kanya, license no. DS024024L of Red Lion, York County, was ordered to pay the costs of

investigation of $1,000 and was actively suspended for no less than five years and, after a period of no

less than three months and successful completion of requirements as set forth in the agreement, he may

petition the board for a stayed suspension in favor of no less than five years of probation subject to terms

and conditions as set forth in the agreement because he is unable to practice dentistry with reasonable skill

and safety to patients by reason of illness, drunkenness, excessive use of controlled substances, chemicals

or any other type of material, or as the result of any mental or physical condition. (12/16/11)

Out of State Clifford Madison Linkous, license no. DS018318L of Morgantown, WV, permanently and voluntarily

surrendered his license because he was disciplined by the proper licensing authority of another state and

he failed to notify the board of the disciplinary action on the biennial renewal application or within ninety

PA Board of Professional Engineers, Land Surveyors and Geologists

Dauphin County Terry L. Fought, license no. PE023343E of Harrisburg, Dauphin County, was ordered to pay a civil

penalty in the amount of $1,000 and costs of investigation in the amount of $399.40 because he practiced

engineering without having at the time of doing so a valid unexpired license properly registered and

offered to practice professional engineering without having at the time of doing so a valid unexpired

Beaver County George T. Wise, license nos. FD013534L and FS015274 of Beaver Falls, Beaver County, was ordered to

pay restitution in amount of $6,296 as well as have his licenses suspended for no less than 18 months

because he failed to deposit monies received for a preneed funeral contract immediately into an escrow

account in, or transfer the same in trust to, a banking institution in this Commonwealth, as well as failed

to fulfill the obligations of his profession by failing to report to the board within 90 days of the receipt of

preneed/prepaid funds the information required in that subsection for the contract regarding the subject

funeral services. After no less than two months of suspension, he may petition for this suspension to be

stayed in favor of probation for the remainder of the 18-month period. (01/04/12)

PA Board of Medicine

Beaver County Richard Albert Schollaert, license no. MD015711E of Ambridge, Beaver County, was indefinitely

suspended for five years, such suspension immediately stayed in favor of five years probation and subject

to certain terms and conditions, because he failed to practice the profession with reasonable skill and

safety to patients by reason of addiction to alcohol. (01/24/11)

Dauphin County Aimal Khan, license no. MD434927, of Hummelstown, Dauphin County, was indefinitely suspended for

no less than two years, such suspension to be immediately stayed in favor of not less than two years

probation, subject to certain terms and conditions, because he is unfit to practice as a physician due to

mental illness. (01/24/12)

Delaware County Satyendra Kumar Diwan, license no. MD035387L, of Newtown Square, Delaware County, permanently

and voluntarily surrendered his license because he engaged in sexually exploitation and/or sexual

behavior with patients. (12/27/11)

Lackawanna County Sharon K. Skiscim, license no. YM001889L of Dunmore, Lackawanna County, agreed to the permanent

voluntary surrender of her license because she engaged in unprofessional conduct by failing to conform to

penalty of $1,200 and costs of investigation of $169.43 because they failed to possess a certificate of

occupancy; failed to have a separate written consignment agreement for each vehicle; failed to have terms

of sale listed on the consignment sales agreement to include the minimum selling price and the amount of

or the formula for determining the dealer’s commission; and failed to produce business records when an

2, 2012, after which she may petition the board for a stay of no less than two years of probation, because
