PA Board of Barber Examiners Indiana County James E. Williams, license no. BL054029, of Indiana, Indiana County, was suspended for a period of one

(1) year, such suspension to be immediately stayed in favor of probation subject to the terms and

conditions of the Board; he was ordered to pay a civil penalty in the amount of $250; and a public

reprimand was placed on his permanent disciplinary record with the Board, because he has been convicted

Lancaster County Darnell A. Thomas, license no. CO292001, of Lancaster, Lancaster County, was suspended pursuant to

the Order of the Court of Common Pleas of Philadelphia County dated March 15, 2018, which the court

issued under section 4355 of the Domestic Relations Code. The suspension is effective immediately.

PA Board of Certified Real Estate Appraisers Luzerne County John D. McDermott, certificate no. RL001758L, of Wyoming, Luzerne County, was ordered to pay a civil

penalty of $2,500; pay the costs of investigation of $500; and shall attend and successfully complete the

remedial education given by McKissock Learning; Scope of Work Appraisals and Inspections and The

Sales Comparison Approach, because he failed to complete an appraisal assignment within the

PA Board of Dentistry Lehigh County Alfred George Khallouf, license no. DS035075, of Allentown, Lehigh County, was indefinitely

suspended retroactive to July 31, 2017, such suspension to be immediately stayed in favor of probation

for no less than three years for violating a previous agreement entered into with the Commonwealth.

Montgomery County Ryan Joo-Yeul Baek, license no. DS037069, of East Norriton, Montgomery County, had his license to

practice as a dentist in the Commonwealth of Pennsylvania suspended for a period of thirty (30) days

because he was found guilty of a misdemeanor involving moral turpitude. (07/20/18)

Northampton County Carl Joseph Milano, license no. DS020803L, of Easton, Northampton County, was levied a civil penalty

of ten thousand dollars and had a public reprimand placed on his permanent disciplinary record with the

board because he administered anesthesia while his permit to do so was expired. (07/20/18)

Philadelphia County Susan A. Fox, license no. DS037400L, of Philadelphia, Philadelphia County, had her license to practice

as a dentist in the Commonwealth of Pennsylvania suspended for a period of eighteen months retroactive

to January 1, 2018, because she violated a Board Order that was entered as part of a previous disciplinary

proceeding. (07/20/18)

PA Board of Professional Engineers, Land Surveyors and Geologists No disciplinary actions taken.

PA Board of Funeral Directors

Delaware County Leychawne D. Johns, funeral trainee, registration no. FT148049, of Chester, Delaware County, was

levied a $40,000 civil penalty and revoked for engaging in fraud or deceit in obtaining or attempting to

obtain a license, because he was convicted of crimes involving moral turpitude, because he was convicted

of a felony, because he practiced as a funeral director without a valid funeral director license, and

because he operated a funeral establishment without Board approval. (04/23/18)

Indiana County John Allen Lefdahl, license no. FD013773L, of Indiana, Indiana County, was ordered to pay a civil

penalty of $1,200, because he failed to comply with the continuing education requirements in that he was

6 hours short of the minimum 6 total hours of continuing education required for the renewal of his license,

and he obtained or attempted to obtain a license by the practice of any fraud or deceit. (08/15/18)

a $2,000 civil penalty and was indefinitely suspended stayed in favor of probation for no less than one

year based upon having engaged in fraud or deceit in obtaining or attempting to obtain a license on two

occasions and being convicted of a crime of moral turpitude. (04/19/18)

Warren County Edward W. Lloyd, license no. FD013826L, of Youngsville, Warren County, permanently and voluntarily

surrendered his license, because he failed to comply with the continuing education requirements to renew

his funeral director license in that he was 3 hours short of the minimum 6 hours required for the renewal

of his license. (08/15/18)

PA Board of Massage Therapy

Centre County Carl Eric Nelson, license no. MSG011095, of Pleasant Gap, Centre County, had his license to practice as

a massage therapist temporarily suspended, pending a hearing, on the grounds that his continued practice

within the Commonwealth may be an immediate and clear danger to the public health and safety.

Chester County Greta Lynn Denny, license no. MSG007607, of Toughkenamon, Chester County, was ordered to pay a

civil penalty of $1,000, take 15 hours of continuing education dealing with ethics and professional

boundaries, and shall be suspended for no less than three (3) years, with the first six (6) months to be

served as an active suspension and the remaining thirty (30) months stayed in favor of no less than thirty

(30) months of probation, because she engaged in sexual intimacies with a client during the professional

Steven Torres, license no. MSG006966, of Phoenixville, Chester County, had his license to practice

massage therapy temporarily suspended, pending a hearing, on the grounds that his continued practice

within the Commonwealth may be an immediate and clear danger to the public health and safety.

a medical physician and surgeon temporarily suspended, pending a hearing, on the grounds that his

Schuylkill County Kelly Ann Starr, license no. YM002066L, last known of Frackville, Schuylkill County, was indefinitely

suspended for no less than three years and assessed a $1,000 civil penalty based on being convicted of a

Out of State James H. Meyers, PA license no. MD430190, of Louisville, Kentucky, was revoked based on

disciplinary action taken by another state. (05/07/18)

Paul Joseph Cotropia, PA license no. MD038208E, last known of Houston, Texas, was revoked based on

Michael David Larson, of Williamstown, New Jersey, was granted a PA license to practice respiratory

therapy and immediately suspended for no less than three years, said suspension immediately stayed in

favor of no less than three years of probation, based upon being addicted to drugs, narcotics, or alcohol.

pay a civil penalty of $250, based on findings that she failed to know through personal knowledge or have

satisfactory evidence that the person appearing before her was the person described in and who was

executing the instrument and failed to be familiar with the duties of a notary public. (08/13/18)

Dauphin County Debra S. Peterson, notary commission no. 1190958, of Harrisburg, Dauphin County, was suspended, with

such suspension immediately stayed in favor of probation, based on findings she failed to record the

character of the act in her notary register, failed to be familiar with the duties of a notary public and failed

to be of good character, integrity, and ability. (04/27/18)

Lawrence County Rhonda Curiale, notary commission no. 1033810, of New Castle, Lawrence County, is suspended for six

months, stayed in favor of probation, and required to attend six hours of approved notary education and

pay a civil penalty of $250, based on findings that she failed to know through personal knowledge or have

satisfactory evidence that the person appearing before her was the person described in and who was

executing the instrument, failed to be familiar with the duties of a notary public and was not of sufficient

ability to hold the office of notary public. (08/06/18)

Luzerne County Mary Ann Bevilaqua, notary commission no. 1011387, of Dallas, Luzerne County, permanently and

voluntarily surrendered her notary commission because she failed to know through personal knowledge or

have satisfactory evidence that the person appearing before her was the person described in and who was

executing the instrument by notarizing Stephanie L. McAvoy aka Stephanie L. Holdt’s signature on the

agreement without her personal appearance; failed to be familiar with the duties and responsibilities of a

notary public by notarizing Stephanie L. McAvoy aka Stephanie L. Holdt’s signature on the agreement

without her personal appearance, failing to include the essential notarial language on page 12 of the

agreement, and by acting as a witness for Stephanie L. McAvoy’s signature while also notarizing her

signature; and failed to be of good character, integrity and ability, by notarizing Stephanie L. McAvoy aka

Stephanie L. Holdt’s signature on the agreement without her personal appearance, failing to include the

essential notarial language on page 12 of the agreement, and by acting as a witness for Stephanie L.

McAvoy’s signature while also notarizing her signature. (08/17/18)

Mercer County Dominic P. Depofi, notary commission no. 1225289, of Greenville, Mercer County, agreed to the

suspension of his notary commission for a period of fifteen months; following a period of three months of

active suspension, the remaining period of suspension shall be stayed in favor of probation; to

successfully complete a six-hour notary practice and procedure course, and to pay a civil penalty in the

amount of $500 and probation assessment fees in the amount of $600 because he failed to know through

personal knowledge or satisfactory evidence that the person appearing before him was the person

described in and executing the instrument by failing to require the personal appearance of Vickey J.

Feezle when notarizing her signature on the Oil and Gas Lease and Memorandum of Lease; failed to

maintain an accurate notary register by failing to record the date of the act, character of the act, and fee

Montgomery County Anne P. Bergin, notary commission no. 1203080, of Conshohocken, Montgomery County, agreed to the

suspension of her notary commission for twelve months, such suspension immediately stayed in favor of

probation, to successfully complete a six-hour notary practice and procedure course, and to pay a civil

penalty in the amount of $500 and probation assessment fees in the amount of $600 because she failed to

require personal appearance during a notarization; failed to maintain an accurate notary register by failing

to record the fee collected and character of the act; failed to be familiar with the duties and responsibilities

of a notary public; and failed to be of good character, integrity, and ability. (08/23/18)

Philadelphia County Natallia Darashenka, notary commission no. 1238612, of Philadelphia, Philadelphia County, agreed to the

suspension of her notary commission for six months, such suspension immediately stayed in favor of

probation, to successfully complete a six-hour notary practice and procedure course, and to pay a civil

penalty in the amount of $500 and probation assessment fees in the amount of $300 because she failed to

maintain custody and control of her notary seal, by leaving the seal in an unsecured area where Maria

Kruglik was able to access and use it; failed to maintain an accurate notary register, by failing to record

the character of the act and failing to indicate each notarization separately; and failed to be familiar with

the duties and responsibilities of a notary public, by failing to maintain custody and control of her notary

stamp and failing to maintain an accurate notary register. (07/31/18)

Melba Terrell, notary commission no. 1277681, of Philadelphia, Philadelphia County, permanently and

voluntarily surrendered her notary commission because she notarized the signature of Mamie L. Bradley

on a deed transfer when Mamie L. Bradley did not appear before Respondent because she was deceased;

failed to maintain an accurate notary register of all official acts performed by failing to record the

notarization of Mamie L. Bradley’s signature in her notary register, failing to record the character of the

act and date of the instrument in her notary register, and failing to indicate each notarization separately as

evidenced by her recorded fees above the fixed fees set by the Secretary; in the alternative to failing to

indicate each notarization separately in her notary register, Respondent charged fees in excess of the fees

fixed by the Secretary of the Commonwealth, as evidenced by her recorded fees in her notary register;

failed to be familiar with the duties and responsibilities of a notary public; and failed to be of good

character, integrity, and ability. (08/03/18)

Gary F. Dydak, notary commission no. 1043917, of Philadelphia, Philadelphia County, agreed to the

suspension of his notary commission for twelve months, such suspension immediately stayed in favor of

probation, to successfully complete a six-hour notary practice and procedure course, and to pay a civil

penalty in the amount of $500 and probation assessment fees in the amount of $600 because he failed to

know through personal knowledge or satisfactory evidence that the person appearing before Respondent

was the person described in and who executed the instrument, by failing to require the personal

appearance of Dawn M. Ortiz during the notarization of the document; failed to maintain custody and

control of his notary register, in by failing to be able to locate his register upon request; failed to be

familiar with the duties and responsibilities of a notary public, by failing to require personal appearance

during the notarization of the document and failing to maintain custody and control of his notary register;

and failed to be of good character, integrity and ability, by failing to require personal appearance during

the notarization of the document and failing to maintain custody and control of his notary register.

based on findings he failed to know through personal knowledge or have satisfactory evidence that the

person appearing before him was the person described in and who was executing the instrument, failed to

keep and maintain custody and control of an accurate chronological register of all official acts done by

virtue of Respondent’s notary commission, failed to be familiar with the duties of a notary public and was

not of sufficient ability to hold the office of notary public. (05/25/18)

PA Board of Nursing

Erie County Marisha Dawn Winiarcyk, of Erie, Erie County, was assessed a civil penalty of $10,000, based on

findings she practiced practical nursing without a valid current license. (04/24/18)

Melisa Lyn Hayes, license no. PN276541, of Erie, Erie County, was indefinitely suspended, with such

suspension to be stayed in favor of no less than six (6) months of probation subject to the completion of a

continuing education course on the unauthorized accessing of a patient’s medical record, as well as

completing eight (8) hours of continuing education in nursing law and ethics, and a public reprimand was

placed on her permanent disciplinary record with the Board because she engaged in unprofessional

Lancaster County Lauren Michelle McMullen, license no. PN295612, last known of Ephrata, Lancaster County, was placed

on probation concurrent with her criminal probation based on receiving Accelerated Rehabilitative

Disposition in lieu of felony charges. (03/20/18)

Treamonesha N. Thompson, license no. PN279250, of Lititz, Lancaster County, was revoked based on

pleading guilty to felonies in Florida and disciplinary taken against her license by another state.

Lehigh County Miosoty E. Ramos-Bauza, license no. RN671890, of Allentown, Lehigh County, was indefinitely

suspended for no less than three years with the first 12 months to be actively served and the remainder

stayed in favor of probation based on her felony convictions. (03/29/18)

Philadelphia County Linda Renee Bryant, license no. PN283607, of Philadelphia, Philadelphia County, was publicly

reprimanded and suspended for six (6) months, with such suspension immediately stayed in favor of no

less than six (6) months probation and ordered to complete 8 hours of continuing education, because she

left a nursing assignment prior to the proper reporting and notification to the appropriate department head

or personnel of such an action. (07/25/18)

Venango County Justin Ryan Taylor, license no. PN295108, of Oil City, Venango County, was indefinitely suspended and

assessed a $766.27 cost of investigation fee based on being unable to practice with reasonable skill and

safety to patients by reason of mental or physical illness or condition or physiological or psychological

dependence upon alcohol, hallucinogenic or narcotic drugs or other drugs which tend to impair judgment

or coordination. (04/24/18)

indefinitely suspended and assessed costs of investigation of $227.50 based upon being addicted to

Jennifer Marie Tucker, PA license no. PN293120, of Bethel, Vermont, was indefinitely suspended and

assessed a $500 civil penalty based on disciplinary action taken by another state and failing to report same

to the Board. (04/25/18) Wanda Kornegay Gray a/k/a/ Wanda F. Gray, PA license PN151110L, of Bowie, Maryland, was reprimanded and issued a civil penalty of $500, based on her failure to report a disciplinary action received from another state. (08/04/18)

Karen M. Kang Lockyer, PA license no. RN545156, of Lihue, Hawaii, had her license to practice as a

registered nurse in the Commonwealth of Pennsylvania placed on indefinite probationary status and had a

public reprimand placed on her permanent disciplinary record with the board, because she had a license

suspended or revoked or has received other disciplinary action by the proper licensing authority in another

Bucks County Jan Neil Widerman, license no. OS004029L, of Holland, Bucks County, was suspended for no less than

nine months retroactive to September 27, 2017, based on misdemeanor convictions under the Drug Act.

Crawford County John B. Zinnamosca, license no. OS002926L, of Conneaut Lake, Crawford County, agreed to the

permanent voluntary surrender of his license to practice as an osteopathic physician and surgeon in the

Commonwealth of Pennsylvania, because he is guilty of unprofessional conduct by treating patients

beyond that which is permitted by an active-retired physician; and he violated the law by not maintaining

Erie County Bryan D. Moles, license no. OS014608, of Edinboro, Erie County, had his license to practice as an

osteopathic physician and surgeon temporarily suspended, pending a hearing, on the grounds that his

continued practice within the Commonwealth may be an immediate and clear danger to the public health

and safety. (08/22/18)

Montgomery County Lawrence I. Miller, license no. OS009548L, of Lansdale, Montgomery County, was indefinitely

suspended until he applies for and receives approval from the Board for reinstatement, under the terms

and conditions set forth by the Board including completion of an Individualized Physician Clinical

Competency Assessment Program approved by the Board, because he engaged in unprofessional conduct,

including any departure from, or the failure to conform to, the standards of acceptable and prevailing

PA Board of Pharmacy

Beaver County Med-Fast Pharmacy Institutional, permit no. PP415600L, of Aliquippa, Beaver County, was ordered to

pay a civil penalty of seven thousand five hundred dollars, because after the suspension or revocation of

the license of a pharmacist employed by Med-Fast Pharmacy Institutional, it was shown that the illegal

actions of said pharmacist were or should have been within the knowledge of the Med-Fast Pharmacy

Bucks County Ryan Andrew Stout, license no. RP443133, of Perkasie, Bucks County, was indefinitely suspended for no

less than three (3) years, such suspension to be immediately stayed in favor of no less than three (3) years

of probation, unless that period of probation is extended or modified for cause by mutual agreement of the

Respondent and the Professional Health Monitoring Program (“PHMP”), Disciplinary Monitoring Unit

with the continuing education requirements to renew his Real Estate Salesperson license in that he was

fourteen hours short of the minimum fourteen total hours of continuing education required for the renewal

of his license; and failed to provide accurate and truthful information in connection with an application for

examination, licensure, registration or approval. (08/21/18)

Lehigh County Stephen N. Walden, license no. AB046343A, of Slatington, Lehigh County, was ordered to pay a civil

penalty in the amount of $3,500 and an assessment fee of $726.68 towards costs of investigation; and he

shall attend and successfully complete at least thirty (30) hours of remedial education on the topic of Real

Estate Law at the Broker level subject to the terms and conditions of the Commission, because he

distributed the escrow money while there was a dispute; he violated the law in that his conduct in filling

in the escrow disbursement section of the Termination of Sale Agreement demonstrates bad faith,

dishonesty, and untrustworthiness; and he failed to deal honestly and in good faith. (08/21/18)

vehicle salesperson license he obtained in 2005 under the name "Alberto Maldonado", and willfully made

a false statement as to a material matter in any oath or affidavit required by the Act by certifying that the

information provided in his application for a vehicle salesperson license was true and correct when

Respondent intentionally and knowingly failed to disclose the previous vehicle salesperson license he

obtained in 2005 under the name “Alberto Maldonado” on said application. (08/07/18)

Monroe County Larry L. Clark, Jr., license no. MV246114, of Blakeslee, Monroe County, permanently and voluntarily

surrendered his license to practice as a vehicle salesperson in the Commonwealth of Pennsylvania,

because he on two separate occasions was convicted, or entered a plea of guilty, or entered a plea of nolo

contendere in a court of competent jurisdiction in this or any other state or Federal jurisdiction of forgery,

embezzlement, obtaining money under false pretenses, extortion, conspiracy to defraud, bribery, odometer

tampering or any other crime involving moral turpitude while his current license is in force or within five

years prior to the application for or issuance of a license; and made material misstatements on his

Salespersons Initial License Application and on his Salesperson Change of Employer/Reactivating

voluntarily surrendered his license to practice as a vehicle salesperson in the Commonwealth of
