Starting Monday, June 5, 2006 the Human Resources Department launches the 3rd annual

“iCOD Internet Offence Declaration Process to all TCDSB employees, under Ontario Regulation 521/01, for the “2006/07” school year

The Board’s “iCOD” (Internet Criminal Offence Declaration) website will be available for all employees to complete and submit their required offence declaration for the 2006/07 school year. As a reminder to all employees, the Ontario Regulation 521/01 requires all school boards to collect offence declarations from all employees by each September 1. All employees are required to make an annual offence declaration. Failure to do so may result in suspension or termination of employment. The iCOD website is a highly secure, private and confidential. Employees may access this website in the privacy of their homes, at their place of work or at a public library through any web browser that supports a minimum 128-bit encryption. The iCOD site will be available starting Monday June 5, 2006 through to August 31, 2006. However, we are asking all school-based employees to complete their offence declaration by June 30, 2006. Employees may access iCOD through the following links: • the iCOD website at : https://icod.tcdsb.org • through the Board’s Internet website home page at: http://www.tcdsb.org under the “Staff” page at:

http://www.tcdsb.org/staff • through the Board’s Intranet site at: http://intranet.tcdsb.org Should you need assistance once you are on the iCOD website, you may contact Human Resources at: 416-222-8282, ext. 2999 or e-mail your concerns or questions to [email protected]. Tips to note before logging on to iCOD: For those employees that do remember their iCOD password from last June’s offence declaration process, all you have to do is sign onto iCOD and complete this year’s 2006/07 offence declaration. For those employees that do not remember their “iCOD password” from last June’s offence declaration process, as well as any new employees that did not participate in the 2005/06 process, please refer to the upper right hand corner of your payslip for “iCOD registration information.” You will need to refer to this information when accessing iCOD to register and complete your declaration: • Connect to: https://icod.tcdsb.org. You must register as a “new user”. • Once you are in the “Registration for New User” screen, you will be asked to enter a password in a field called “User

Password”. This password does not have to be a TCDSB password that you may currently be using for other TCDSB applications. iCOD is a totally separate system, independent from any other TCDSB application. Create your own password for iCOD and privately document this number in your personal belongings.

• An iCOD userid will be provided to you by the iCOD application once you have registered. Remember this userid; store it with your iCOD password that you created.

• Once you have registered in iCOD, you must continue in the application with the “declaration process”. Frequently Asked Questions: For a list of Frequently Asked Questions (FAQs) about the TCDSB iCOD process please click on the FAQ link on the http://icod.tcdsb.org home page or go directly to http://www.tcdsb.org/icoddocs/FAQ.htm to access TCDSB iCOD FAQs directly. Should anyone be uncertain if they have committed a “criminal” offence, please contact your personal legal counsel for further clarification. TCDSB POLICY B.R.05 TCDSB Principals are required to submit permit applications for all pre-scheduled school events or meetings (i.e. parent-teacher interviews, school council meetings, school dances, school anniversaries, team practices, etc.) for the 2006-2007 school year by JULY 7, 2006. A permit is required for any event taking place after 6:00pm Monday through Friday and for any part of the day on Saturday, Sunday and holidays. This will ensure that these school functions are part of the Permits Calendar before any permits are approved for non-school organizations, as outlined in Policy B.R. 05. The Catherine Temple Mathematics Award is presented annually to an eligible female student graduating from a TCDSB secondary school to encourage and support her in her pursuit of studies in mathematics or mathematics related field at a Canadian post-secondary institution. A scholarship shall be awarded to a graduating female student pursuing studies in mathematics: i. Academic standing shall be the main criteria used in the selection of the recipient ii. The student will also have a record of significant contribution to the school community especially in areas

New playground at St. Matthias North York Mirror May. 26, 2006 Staff and students at St. Matthias Catholic School marked the official opening of their new playground structure last Friday. On hand for the celebration was Trustee John Del Grande, Superintendent of Education Michael McMorrow and Janet Bertani, Chair of the Catholic School Advisory Council. The kindergarten playground is meant to be a safe play space for the youngest students at the school. The structure is the first step in the playground renovation, which will include a paved pathway and the addition of some green space. Initiated by the school's parent coucil, the structure was funded through a donation from Trustee Del Grande, according to the board. St. Matthias is located 101 Van Horne Ave. in North York.

Local school receives literacy award LISA RAINFORD Etobicoke Guardian 05/25/06 Nativity of Our Lord Catholic School is the recipient of the George Ermellini Literacy Award for 2005/06. The George Ermellini Literacy Fund was established in honour of George Ermellini, a dedicated and much-loved Toronto Catholic District School Board educator, who passed away 10 days ago. Ermellini was well known for his commitment to the promotion of literacy within the Catholic board. "It's a great honour," said principal John Shain. "George was a former superintendant, who I had the honour of working with when I was a Grade 8 teacher so it's an honour on a personal level as well."

The kindergarten to Grade 8 school was recognized for its program that opens its doors to the wider community by welcoming seniors from the Catholic Women's League (CWL) and inviting them to help students with their reading. The CWL Literacy Intervention Program was the brainchild of CWL member Audrey Jarvis and coordinated by teacher Sharon Nagle, a longtime teacher at Nativity of Our Lord. "She's very creative," Shain said. "She was the one who made all the connections and put the program together. It not only helps the seniors, but the children as well."

The seniors offer Grade 1, 2 and 3 students one-on-one assistance with reading. The program has been in existence for the past two years. During weekly reading sessions, students have the opportunity to build both reading skills and self-confidence as they work with their senior reading buddies.

"This program is a valuable community building tool and has given added meaning to many of our seniors' lives... we see the interaction between the seniors and the students during our monthly community Mass as students and seniors wave at each other or even sit together," the school stated in its proposal to the Metropolitan Toronto Catholic Education Foundation. The funds awarded will be used to purchase more books for this increasingly popular program.

St. Henry’s Grade 6 Entrepreneurial Venture wins a BMO National Student Innovation Award. The Entrepreneurial Adventure program is sponsored by the Learning Partnership and BMO Financial group where students select a business venture that is suitable and innovative. Mrs Patroni’s Grade 6 students decided on a dinner/fashion show called Stylin’n’ Dinin’. Students worked in groups planning and costing the menu for the three course dinner meal, securing donations and getting the word around through various methods of advertising. The class was assisted by Mr Bonair, the business partner for the venture, a realtor from Remax, who provided numerous ideas and help for the business aspect, drawn from his wealth of knowledge of business.

A luncheon/ fashion show was held for the students of the school, complete with rules for fine dining etiquette. A dinner show was put on for the parents and community. The auditorium was transformed into a fine dining restaurant. The ambience of fine ding was created with subdued lighting, cloth table covers, fresh flower centre pieces and crockery and cutlery. The walls and stage area were decorated with art by the Grade 7’s and 8’s. Guests were treated to a scrumptious meal served by and array of preteen proficient waiters and waitress. Live entertainment provided by the grade 6 ‘in class’ talent accompanied the meal. The fashion show featured fashion lines worn by confident and stunning eleven year old boys and girls. The glamour displayed on the runway by the young models gave a new name to stylin’! The evening was brought to a close by a version of Be Our Guest performed by our budding entrepreneurs.

The net profit of $1152.39 will be presented to Covenant House, a shelter for the homeless youth, a charity researched and chosen by the students on June 13th. Award applications from 69 venture teams from across the country were submitted to the Learning Partnership. Each application was reviewed by a team of volunteer judges from BMO Financial Group. St. Henry‘s Grade 6 entrepreneurial venture Stylin’ n’ Dinin’ was confirmed the recipient for the “Teamwork Award”. Ventures for this award are recognized for their ability to build partnerships that were nurtured and maintained through the project. Congratulations to the students, teachers and business partner for working together to pull off a full scale formal dinner fashion show event for the school community- a first for the school.

A copy of your 2006 Certificate of Qualification or a copy of your 2006 Interim Certificate of Qualification (without ABQ conditions) from the Ontario College of Teachers MUST accompany this registration form. A Certificate of Registration/An Interim Certificate of Qualification with ABQ CONDITIONS or a copy of your Ontario College of Teachers membership card is not adequate for registration. One important change in the Religion AQ program for the fall is that in order to be recommended for this Additional Qualification Course a candidate must attend and participate in all classes. Should something arise which makes it impossible for you to attend a class, you need to contact the instructional leader and the course director. A make-up assignment equal to the number of hours missed will be assigned. The Ontario College of Teachers has mandated full 100% attendance for all Additional Qualification Courses. • To submit your completed registration package, it is NOT recommended that you use the Board courier as this does not operate in the summer months.
