Ketone Bodies in DKA
• Unless -hydroxybutyrate (-OH B) is specifically
ordered, the ketone bodies are estimated by the
nitroprusside reaction in the lab, which
measures only acetone and acetoacetate (AcAc)
• In DKA, the dominant ketoacid is -hydroxybutyric acid (-OH B), especially in cases of poor tissue perfusion.
• During recovery, the balance shifts to acetoacetic acid (AcAc)
Ketone Measurements in DKA
• During recovery, results from the nitroprusside
test might wrongly indicate that the ketone
concentration is not improving or is even
getting worse.
• The best biochemical indicator of resolution of
keto-acid excess is simply the anion gap.
• There is no rationale for follow-up ketone
measurements after the initial measurement.
Anion Gap Metabolic Acidosis
• The normal anion gap in mEq/L is calculated as: [Na] - [Cl + HCO3]
• The normal gap is <12 mEq/L
Management of DKA
• Replacement of fluids losses
• Correction of hyperglycemia/metabolic acidosis
• Replacement of electrolytes losses
• Detection and treatment of precipitating causes
• Conversion to a maintenance diabetes regimen (prevention of recurrence)

Potassium Replacement
• K+ > 5.2 mEq/L: no supplemental is required
• K+ = 3.3 – 5.2 mEq/L: 20 -30 mEq K+ of each liter of IV fluid to keep serum K+ between
4-5 mEq/L
• K+ < 3.3 mEq/L: hold insulin and give 20 -30 mEq/h until K+ > 3.3 mEq/L

Bicarbonate Administration
• pH ≥ 6.9: no bicarbonate
• pH < 6.9: 100 mmol in 400 ml H20 + 20 mEq KCL, infuse for 2 h

Management of DKA
• Check electrolytes, BUN, Cr, venous pH and
glucose every 2-4 hrs until stable.
• After resolution of DKA and when patient is
able to eat, initiate SC multidose insulin
regimen.

Management of DKA
• To transfer from IV to SC, continue IV insulin
infusion for 1-2 hr after SC insulin begun to
ensure adequate plasma insulin levels.
• In insulin naive patients, start at 0.5 U/kg to
0.8 U/kg per day and adjust insulin as needed.

When to Transition FromIV Insulin Infusion to SC Insulin
• BG <200 mg/dL and 2 of the following
HCO3 ≥ 15 mEq/L
Venous pH > 7.3
Anion gap ≤ 12 mEq/L
• Allow an overlap of 1-2 h between SC insulin and discontinuation of IV insulin

Conventional Insulin Guidelines
• When starting insulin, initially infuse 0.1 to 0.14
units/kg/h
• If plasma glucose does not decrease by 50-75 mg
in the first hour, increase the infusion rate of
insulin
• Continue insulin infusion until anion gap closes
• Initiate SC insulin at least 2 h before interruption
of insulin infusion

DKA Management Pitfalls
• บริหารสารน้้าและเกลือโซเดียมทดแทนไม่เพียงพอ ซึ่งจะเป็นผลให้
ระดับน้้าตาลในเลือดลดลงช้าหรือไม่ลดลงเลย• บริหารอินสุลินอย่างไม่ต่อเนื่อง เนื่องจากรอผลการตรวจระดับน้้าตาล
ในเลือดซึ่งรายงานล่าช้าหรือละเลยการติดตามผล ซึ่งจะเป็นผลให้
ระดับน้้าตาลในเลือดลดลงช้าและภาวะกรดเมตะบอลิคยังคงมีอยู่
• หยุดการบริหารอินสุลนิต่อเนื่องเมื่อใหก้ารรกัษาจนผู้ป่วยมีระดับ
น้้าตาลในเลือดลดลง ซึ่งจะเป็นผลให้เกิด DKA ขึ้นซ้้าไดอ้ีก

• ละเลยการบริหารสารละลายเด็กซโ์ตรสเมื่อให้การรกัษาจนผู้ป่วยมีภาวะ
น้้าตาลในเลือดลดลงอยู่ที่ 200 มก./ดล. โดยที่ยงัคงได้รับอินสุลิน อย่างต่อเนื่องอยู่ ซึ่งจะเปน็ผลให้ผู้ป่วยมีภาวะน้้าตาลต่้าในเลือด
• ละเลยการปรบัปริมาณของอินสุลนิที่บริหารเมื่อให้การรักษาจนผู้ปว่ยมี
ระดับน้้าตาลในเลือดต้่ากว่า 200 มก./ดล. และภาวะกรดหายแล้ว ซึ่ง
จะเป็นผลให้ผู้ป่วยมีภาวะน้้าตาลต่้าในเลือด
DKA Management Pitfalls
• ละเลยการบรหิารโปแตสเซียมทดแทนหรอืบริหารไมเ่พียงพอ ซึ่งจะเป็น
ผลให้เกิดภาวะโปแตสเซียมต่้าและหัวใจเต้นผิดจังหวะได้
• บริหารโซเดียมไบคาร์บอเนตโดยไม่จ้าเป็นหรือมากเกนิไปจนท้าให้เกิด
ภาวะแทรกซ้อนซึ่งท้าให้ผู้ป่วยมีอาการแย่ลงได้
DKA Management Pitfalls
• เปลี่ยนชนิดของอินสุลินที่ใช้จาก RI เป็น NPH เร็วเกนิไป หรือเปลี่ยน จากการรกัษาด้วยอินสุลินเป็นยาลดน้า้ตาลชนิดกินอย่างไม่เหมาะสม
ซึ่งเป็นผลให้ผู้ป่วยมี DKA เกิดขึ้นซ้้า• ละเลยการสืบค้นหาและและรักษาปัจจัยชักน้า ซึ่งเปน็ผลให้ผู้ป่วยไม่
ตอบสนองต่อการรักษาเท่าที่ควรหรือท้าให้ผู้ป่วยมีอาการเลวลง
Possible Precipitating Causes or Factors in DKA
• Nonadherence to medication regimen
• Poor “sick-day” management
• Dehydration
• Renal insufficiency
• Infection (intra-abdominal, pyelonephritis, flu)
• Myocardial infarction, stroke
• Steroid therapy, other drugs or substances
Causes of Morbidity and Mortality in DKA
• Shock
• Hypokalemia during treatment
• Hypoglycemia during treatment
• Cerebral edema during treatment
• Hypophosphatemia
Causes of Morbidity and Mortality in DKA
• Acute renal failure
• Adult respiratory distress syndrome
• Vascular thrombosis
• Precipitating illness, including MI, stroke,
sepsis, pancreatitis, pneumonia
