Diabetes Centres in Nova Scotia Who should be referred? Anyone new to insulin (will be seen within 48 to 72 hours.) As well as anyone with: - a new diagnosis of diabetes or established diabetes needing a review - new diabetes medication added - poorly controlled diabetes - recurrent hypoglycemia or at risk of hypoglycemia How do you to refer? 1) Fill out the DC referral form available at http://diabetescare.nshealth.ca. 2) Fax the completed form to the patient’s local Diabetes Centre and send a copy to his or her family doctor. Diabetes Centres (DC) are located across Nova Scotia. They vary in hours of operation from full-time to part-time. A number offer satellite services to smaller communities (as indicated on the map by the dotted lines). DC staff provide self-management support through group and individual education/counselling, assessment/development of treatment plans, problem solving, and skill development (insulin adjustment, meal planning, etc.). Referrals are triaged; the most urgent are often seen within days. Who can refer? • Any physician or allied healthcare provider can refer patients to a Diabetes Centre. Be sure to put the family doctor’s name on the referral. Fax the referral form to the Diabetes Centre closest to the patient’s residence and the patient’s family doctor.
