Criteria: When a student submits 2 FMX series each having one error less than the number resulting in a failing grade (9 for DH 173),

she/he will be removed from Radiology Rotation and required to expose and develop a full mouth series on a DXTR. When a student submits 2 BWX series each having one error less than the number resulting in a failing grade (3 for DH 173),

she/he will be removed from Radiology Rotation and required to expose and develop a bitewing series on a DXTR. When a student fails 3 FMX's or 2 BWX's, she/he will be removed from Radiology Rotation and will not be permitted to

Perform one (1) periodontal charting this semester utilizing the Symbols for Perio Charting (Section 6 Clinic Procedures in the Clinic Manual) PRIOR to removal of calculus; evaluate and utilize findings to plan dental hygiene care.

The acceptable patient charted will present as a Class 5 or 6 with a current and complete FMX (not a pano and 4 BWX). The quadrant selected must have a minimum of 6 teeth, 2 furcations, and 4 pockets that measure 5 or 5+ mm. Correctly probe, record, and chart periodontal findings for one quadrant. The selected quadrant MUST be anesthetized prior to initiating this competency. Submit perio charting form with DCEF. It is expected that the student will recall this patient in the fall, clinical semester II.

The objective is for students to learn how to probe AROUND calculus. Complete periodontal chartings are to be performed on all Class 5 and 6 patients prior to any scaling.

6. Successfully demonstrate correct use of Gracey & universal curets on an appropriate clinic patient.

7. Successfully demonstrate correct use of the ultrasonic scaler on appropriate clinic patients.

8. Successfully demonstrate correct placement of 3 sealants on an appropriate patient(s) according to established criteria.

9. Radiographic techniques according to established protocol and to acceptable standards (see page 46 in this syllabus) for this semester by successfully completing:a. Four (4) full mouth series. b. Two (2) bitewing series.

For each set of radiographs, the student will 1) Evaluate his\her technique using the green radiographic technique form2) Determine the needed retakes3) Consult with the block instructor for initial read4) Describe to the instructor which films need retakes and why, and5) Review the films for pathology with the instructor using the Radiographic Interpretation form

NOTE 1: When a student submits 2 FMX series with 9 or more errors, she/he will be required to expose and develop a full mouth series on a DXTR. When a student submits 2 BWX series with 3 or more errors, she/he will be required to expose and develop a bitewing series on a DXTR.

NOTE 2: When a student fails 3 FMX's or 2 BWX's, she/he will be removed from x-ray rotation and will not be permitted to expose radiographs on patients until satisfactory remediation is completed.

10. Utilize appropriate infection control procedures for the dental setting including: prevention of cross contamination, disinfection of the operatory, and sterilization of instruments.

11. Assessment, DH Diagnosis and Treatment Planning at all initial assessment appointments

a. Record the gingival condition and 6-point probe readings, mobility, recession, and furcations on each patient’s Oral Evaluation form. Evaluate and utilize the findings to plan dental hygiene care (develop a treatment plan).

b. Recognize patients in need of referral to a dentist and/or physician and follow through with such a referral.

c. Counsel patients with the aid of a plaque index to deliver individualized oral hygiene instruction based on patient dexterity, motivation and oral health status.

d. Identify patients in need of pain control procedures , and arrange for or administer such procedures according to acceptable standard for this semester.

12. Update Patient Assessment Data on all patients at every appointment after the initial assessment appointment according to established protocol (review the patient’s medical history, oral evaluation, and home care procedures, provide preventive counseling to patients after disclosing the amount of biofilm, and evaluate all areas previously scaled for completion of debridement and for healing).

13. Demonstrate correct technique using universal curets, Gracey curets and sickles to a 2nd semester level. Maintain sharp and properly contoured instruments.

Attendance is inextricably linked to academic success and students are expected to attend all scheduled class, labs and clinical sessions. It is an essential workplace skill. Tardiness is considered unprofessional. If it is necessary to miss a class period, it is the student’s responsibility to obtain notes and assignments from another student. In the event of an absence, notify the course coordinator (831-479-6480) or call the Dental Hygiene Program number 479-6471 or 479-6431. It is a good idea to have these phone numbers in your cell phone.No make-up didactic tests will be given in the event of an absence. In the event that the student has 5 absences (more than 2 weeks of class) the student may be dropped from the course. A tardy is defined as being late to class one (1) minute or more. When the student is late to clinic/lab or lecture five (5) times the final course grade will be lowered one grade level. Unusual circumstances will be discussed on an individual basis between the student and course coordinator.The clinic and lecture/lab sessions are class hours. Attendance is required whether a patient cancels or fails an appointment. Students are expected to attend each class session and to engage in patient care/support duties for the entire session.Children of students are not allowed in class, clinic (unless as a patient), or in room 610 for safety reasons. Participation includes class contributions, discussion roles, promptness, and regular completion of assignments, and preparedness. Participation also includes serving as a cooperative patient for the assigned lab partner(s).

ASSIGNMENTS:

Assignments must be completed on time and according to specified criteria. LATE ASSIGNMENTS are accepted at the discretion of the clinic coordinator

Prior to class or laboratory students are responsible for reviewing all assigned reading and videos as well as for

providing the appropriate armamentarium for all labs and clinics.4. TYPODONT PRACTICE : It is expected that students will spend time outside of class (approximately one hour per

week) in practicing scaling and instrumentation principles. Typodont mounts for home practice may be checked out for the semester ($200 deposit required).

NOTE: It is illegal for unlicensed students to practice on another person (including other DH students) outside of the Cabrillo dental hygiene clinic. Graded skills can be performed only during scheduled DH173 clinic hours.

CLINICAL EVALUATION PROTOCOLS FOR DH173, 181, and 183 1.15.08, 5.5.09

To enhance learning/teaching students are to be evaluated by their assigned block instructor in clinic and lab. Only the assigned block instructor has the authority to refer you to another instructor. Seeking another instructor without permission of the assigned block instructor is not acceptable. Failure to follow this protocol is considered a violation of professionalism and may result in a grade code of 05/U. Only RDH clinical instructors shall evaluate root planing periodontal charting and mock boards unless otherwise directed by the Clinic Coordinator.

STUDENT PARTNER PROGRAM 9.94, 5.11.96, 1.19.09, 3.6.09

In order to master the volume of information required in this course, the students are encouraged to form small study groups. The learning process seems to be more permanent if verbal discussion is added to the learning tools of reading the text, listening to the lecture and reviewing notes.Students are encouraged to form working groups after the first week of class, to meet outside of class to review assignments, and to confer by phone.For some specified assignments students may complete their exercises with a partner and share the grade. During the exercises, partners are free to discuss the questions and their answers quietly until they reach agreement. In this case the team submits one completed exercise paper with both names; the grade for the exercise is recorded for both members of the team.Several students can work on a student team, but only two to three students are allowed on an exercise-taking team. Some students may prefer to work alone; solo exercise taking is allowed. 1. Laboratory Skill Demonstration: The student must demonstrate skill competence in the lab on typodonts and partners in order to perform the service in clinic on patients.

2. Required Clinical & Mock Board Competency Exams must be completed during the semester. These Competency Exams are graded using the Competency form and the DCEF. The following scale is used: Satisfactory/correct = 01; Needs Improvement = 02; Unsatisfactory = 03. A clinical skill demonstration or Competency Exam not attempted will result in the grade of 03 for that test. As time allows before the last day of clinic, the student must repeat a failed skill and must achieve a grade of 01 or 02 competency level in order to pass the course.

3. Non-graded performance: After successful completion of a required performance test, it is expected that the student will consistently demonstrate competency in that skill (code 04, Satisfactory). The repeated achievement of a code 05 or code 06 (unsatisfactory) level for a skill will result in remediation and may adversely affect the student’s grade.

4. Professional behavior. Students must demonstrate all professional behaviors & attendance requirements at a satisfactory level.

5. Quizzes will be given covering material presented in class, laboratory, and reading assignments. 6. Comprehensive written midterm and final examinations will be given at scheduled times during the semester.

Skill Evaluations (non-requirement) occur daily and are recorded on the DCEF. Skill Evaluations are non-graded and provide the student with the opportunity to monitor and improve instrumentation skills (non-graded coding: “04” and “05”). Failure to demonstrate continued skill building in labs and clinics, as evidenced by 05’s, will result in an academic consultation with the Course Coordinator to determine further progression in the course.

Competency Exams (requirements) are recorded on the DCEF & the appropriate Competency form and are either at scheduled times or are completed during any clinic session with the approval of the block instructor. This approval must be granted before the procedure is undertaken. Time restrictions may apply. COMPETENCIES may not be performed on dental hygiene students without the written approval of the Clinic Coordinator. Criteria for COMPETENCIES are in the clinic manual and/or in the course syllabus.

Skill Maintenance - Once the student has successfully demonstrated competence for a specified procedure at a Satisfactory (01) or Needs Improvement (02) performance level, he/she should continue to perform that procedure to a competent level. The number of errors allowed shall follow the schedule below. A partial quadrant completed is graded with a proportional decrease in maximum points. When more than one quadrant is completed in a clinic appointment on a class 4, 5, or 6 patient, a “03/05” in any one quadrant will result in a “03/05” being recorded on the D-CEF for that appointment. The number of quadrants to be completed at each appointment shall be determined by the difficulty of the patient (via discussion with the block instructor) and by the clinical semester. There will be one Mock Board Exam that will be tested as a Clinical Competency Exam (CCE) demonstrating the student’s ability in providing total patient care and the second Mock Board Exam will focus on end product. Further information will be given in class. The graded competency exams can be attempted multiple times and must be passed by week 15. See the following guidelines.

EVALUATION For Clinical Competency Exam (CCE)

The CCE will be performed at a scaling appointment of a patient who is a Class 3 or Class 4. The skills to be evaluated are preparation of the unit, Health History, Oral Assessment, Periodontal Assessment, DH Diagnosis, Treatment Planning, Disclosing and Preventive Counseling, Universal Curets and Sickle Scalers, Polishing and Fluoride Trays, record keeping and standard precautions. The end product (supra, sub, tissue laceration) is also evaluated.

The student will scale two quadrants to completion on the class 3 patient or one quad on a class 4 patient. The area to be scaled will be pre-approved in clinic and reviewed by the CCE examiner The criteria are on the exam form.

What happens if a student does not pass this exam? Tutoring will be recommended. Retesting is scheduled by the student during clinic hours. The CCE must be passed by week 15.

Frequently Asked Questions regarding the Clinical Competency ExamsWhen does the exam begin and end? The exam will begin at 8 a.m. and 1 p.m. The exam ends at 11:30 and 4:30 – this means patients are to be out of the clinic and all paper work completed by the student. Examiners will arrive for a pre-exam faculty huddle at 7:45 and 12:45

The examiner is responsible for documenting student in and out time on the Clinical Competency Exam form and observing the entire process from setting up the operatory to disinfecting it and from assessment to instrumentation to completion of clinic paperwork.

What is the student responsible for during this exam? The student is responsible for having all appropriate forms at the operatory.

o Must have 5 teeth with explorer detectable subgingival calculusHow are patients identified? Some students may identify a patient they have assessed. In other cases second year students may refer a patient who was assessed during 2nd year clinics.

NOTE: Faculty pre-approval is required. During the assessment the examiner will explore and review the teeth to be scaled for this exam.

What is the student to do about confirming their patients before the exam? The students are instructed to confirm their patient’s appointment and to have an alternate plan if the patient fails, presents with a condition that prevents treatment, or cancels at the last minute. As part of the alternate plan, they are encouraged to have a list of patients who are able to come in at the last minutes or to have a back up patient.When confirming the patient’s appointment prior to the exam, the students are reminded to ask the patient appropriate questions, i.e. has the patient had any changes in their medical history since their last visit or taken new meds; remind the patient to eat.

What is included in the assessment portion of the exam? It is expected that the patients have been previously assessed this semester. Patient assessment update is part of the exam. For the CCE this includes a review of all information on the OE form including confirming the accuracy of missing teeth, recession, occlusion, and existing restorations. In addition a 6 point probe will be performed during the exam in the quadrant determined by the examiner. Students will identify furcations, mucogingival involvements, and mobility as required for the second clinical competency exam.

What is to be scaled during this exam? Two quadrants of a class 3 patient or 1 quad of a class 4 patient are to be scaled. PRE-APPROVAL of the patient is REQUIRED for the CCE and the Mock Board Competency.

PROCEDURE FOR CLINICAL COMPETENCY EXAM 3.3.08, 11.9.08, 12.11.08

All students will be present in the facility during the hours of the scheduled CCE.

One student will take the exam as the operator in the assigned operatory; and one student will participate as the recorder.

The operator will be told the name of the recorder during the exam.

The process: Starting at 8 AM (or 1 PM) the student enters the clinic and prepares the operatory for treatment. The patient is then seated and health history is reviewed with appropriate paperwork completed. After obtaining examiner sign off of the patient’s medical history and Dentrix route slip, the operator performs an assessment or assessment update.

In the event the patient was previously assessed, the operatory will confirm the EO/IO Exam data as recorded on the Oral Evaluation form and probe one quadrant as indicated by the examiner.

The student then reviews the treatment plan and obtains patient consent.

The examiner will review the assessment data and concur with the treatment plan. The student then provides OHI based on assessment data & PI, instruments, polishes and administers fluoride. The examiner will observe technique and will perform a check out. Upon completion of treatment the operator will breakdown the operatory.

CRITERIA FOR MOCK BOARD COMPETENCIES (2 required, one of which is a CCE)

The student will provide one Class 3 patient and one Class 4 patient for these clinical exams. The patients must present with explorer detectable clicks per the criteria of calculus case classification.

Two clinical instructors must approve the patient prior to the exam and the instructors must initial OE form.

EVALUATION FOR MOCK BOARD COMPETENCIES

The student will probe one quadrant and scale all four quadrants to completion on one Class 3 patient and on one Class 4 patient.

Probe readings for one quadrant will be recorded and evaluated on the Mock Board Competency Exam grade sheet prior to initiation of scaling.

The student is evaluated on sub, supra, stain, tissue trauma, probing, professionalism, and infection control. Specific criteria are listed on the Mock Board Grade Sheet – see the following page.

At times a student may encounter a patient who meets the following acceptable equivalencies:

1 class 3 = 1 quad of a light class 51 class 3 = 2 quads of a class 41 class 4 = 2 quads of a light class 5

A grade of "01" or “02” in each category is considered acceptable on Mock Board Competency exams. If performance falls below this level for any category, a grade of "03" will be assigned and the student will repeat the examination. Reexamination will involve scheduling another patient for the mock board competency exam. The mock board exam must be successfully completed by the 15th week of class.

Time Allotment and Scheduling

The student should schedule the mock board competency appointment during a 4 hour clinic session. The patient must be completed in the two and one-half hours of scaling time (this includes scaling and polishing, but does not include the time needed for probing, anesthesia, check in or check out).

The mock board exam can be completed in one appointment/clinic session or over two appointments.

If the test is completed over two appointments, at the first appointment the student must scale the selected teeth to completion and must be checked out on the area scaled prior to dismissal of the patient. The patient must be completed in a total of two and one half hours of scaling time

Time management is a part of the Mock Board Competency experience.

MOCK BOARD PROCEDURE: A multi-step process

A. Mock Board Approval: Prior to scheduling the Mock Board Competency Exam, two dental hygiene clinical instructors must concur with the student clinician that the patient is acceptable and must document their concurrence on the Oral Evaluation Form.

B. The Mock Board patient will be scheduled for a 4-hour clinic sessionC. The Mock Board appointment: the student

1. Completes the top part of the Mock Board Grade sheet2. Seats the patient and reviews patient's health history3. Takes patient's blood pressure, pulse, and respiration if required by medication or if

anesthesia will be administered4. Requests instructor to sign off med history and to administer anesthesia, if needed. Note: a

D. The instructor evaluates the calculus and grades the probe readings in the selected quadrant 1. The instructor records the "Time In" for scaling on the Mock Board Grade Sheet

E. The student initiates scaling . The patient is to be scaled and polished in the allotted time of 2.5 hours. The clock starts ticking when the student begins scaling.

F. Check out after completion of scaling and optional polishing : 1. The student will notify the Block Instructor when she/he has completed scaling/polishing OR

when 2.5 hours have passed, whichever comes first.2. One instructor will record “ time out” and evaluate the quadrant(s) scaled. The student

and instructor will conference to discuss errors3. All identified errors are recorded on the Mock Board Grade Sheet and the DCEF.

a. If any errors are found, the student is expected to remove those deposits prior to the patient's dismissal OR have the patient return for completion of the quad(s).
