Introduction
Medicaid is a program designed to help states meet the cost of necessary health care for low-income and medically needy populations. Under Section 1915c of the Social Security Act, states may request waivers of certain Federal requirements in order to develop community-based services funded by Medicaid. Individuals in the Department of Health, Developmental Disabilities Division system who are Medicaid eligible can apply for these Medicaid funded services. Needs and supports are identified through a person-centered planning process with case managers who coordinate and assist individuals in accessing these services from qualified and approved providers. This booklet contains information about the providers of Medicaid waiver services in Hawaii. It was designed to be a resource to assist individuals, their family members, and/or guardians in accessing and selecting an agency that best meets their needs. Possible questions you may want to ask agencies have also been included to assist you in choosing the right provider of services. Please take the time to contact and/or meet with them to discuss the services they have to offer you. This booklet presents a listing of agencies that provide Medicaid waiver services in Hawaii. The listing in this book-let was complied for informational purposes only and is not intended to be an endorsement by the Department of Health.

After you have identified what you want and need, here are some questions you may want to
ask the agencies.
“What activities/services do you offer that will meet my needs?” Give examples of what you want.
“Do I get to select/choose the type of activity that I would like to do? What are some of your fun activities?”
“Do you provide activities such as art, music, computers, dancing, exercise, gardening, swimming, camping?” Write down other activities they offer that are not listed.
“Is each person’s activities individualized to meet what is important to the individual?”
“What are your hours of operation? Do you have services on the weekends and holidays? What time does the program begin and end each day?”
“What happens if my worker can’t come to support me. Will you call me ahead of time? Will you send another person to take his/her place? What do you do so that the substitute person is familiar with me?”
“If I have an emergency, will you be able to help me?”
“Do you provide training for your staff? What kind?”
“Do you do drug screening of your staff?”
“I’d like to select, or participate in the selection of the person who will support me in the activities I like to do. Can this be done?”
“I’d like to visit. Whom do I call to set up an appointment?” When visiting, talk to the program coordinator or staff and participants about the program activities.
“Where are you located? Do you have a program near me?” (Tell them your address.)

To help you discuss services with the agencies, here is a description of waiver services.
Adult Day Health aka ‘ADH’ Adult Day Health (ADH) services are provided in a program-based group setting to help participants become more independent. Services may include activities such as skill development, communication and interpersonal skills. Transportation to and from the ADH is included as part of ADH. ADH services and Community Learning Services (CLS) Group are a package of services that provide both program-based and community-based activities.
Chore Services needed to maintain the home in a clean, sanitary and safe environment. Chore is available to participants living in their own home and are without natural (non-paid) supports or who are living with family but the members of the household are physically unable to perform the tasks.
Community Learning Services aka ‘CLS’ Services in the community to support the participant in improving access to the community through increasing skills, improving communication, developing and maintaining friend-ships, and gaining experience with the opportunities available in the community.
Employment Services Services include Discovery & Career Planning and Individual Employment Supports. The intent of these services is to assist participants to prepare, obtain, and sustain paid employment in integrated jobs in the community at or above minimum wage.
Non-Medical Transportation The service may be used by a participant who lives in a rural or other area where public transportation is limited or non-existent or if the participant requires door-to-door transportation because he/she is unable to reasonably access the bus stop or other public pick-up location. Transportation for medical service is not included.

Personal Assistance/Habilitation aka ’PAB’ A range of assistance or training to enable participants to acquire, retain, or improve in skills related to living in the family home or their own home. PAB services may include assistance and training with activities of daily living such as bathing or dressing, independent living skills such as laundry or cooking, communication and interpersonal skills. A different service, Community Learning Services (CLS), is used in the community. Personal Emergency Response System aka ‘PERS’ PERS is an electronic device that enables participants to main-tain safety in the community and secure help in an emergency.
Residential Habilitation aka ‘ResHab’ Residential Habilitation (ResHab) services are provided to participants living in licensed and certified homes. ResHab provides assistance and training with activities of daily living and independent living skills; participating in typical household activities, family functions and community events; and social and leisure skill development.
Respite Respite services are provided to participants living in family homes and are furnished on a short-term basis to provide relief to those persons who normally provide uncompensated care for the participant for at least a portion of the day. Respite may be furnished in various locations.
Skilled Nursing Services within the scope of the Nurse Practice Act that require the education, assessment, judgment and intervention of a registered professional nurse (RN), or licensed practical nurse (LPN) under the supervision of an RN. Skilled Nursing services are provided on an intermittent, part-time and time-limited basis.

Training & Consultation aka ‘T&C’ Training & Consultation services assist unpaid caregivers, paid service supervisors and/or paid support staff in implementing the goals and outcomes developed from the person-centered planning process and included in the Individualized Service Plan (ISP). The service may include evaluation and assessment; the development of recommendations for person-centered goals and outcomes; initial and/or ongoing training and/or technical assistance to implement the goals and outcomes; development of nurse delegation plans; supportive counseling to strengthen families; and monitoring the implementation of the goals and outcomes.
Vehicular Modifications Adaptations to an automobile or van owned by the participant or family to accommodate the special needs of the participant. Vehicular modifications enable the participant to integrate more fully into the community and to ensure the participant’s health, welfare and safety. Waiver Emergency Service — Out-of-Home Stabili-
zation (formerly called Crisis Shelter) Out-of-Home Stabilization (OHS) provides emergency out-of-home placement of adult participants in need of intensive intervention to avoid institutionalization or more restrictive placement and to return to the current or a new living situation once stable. The Crisis Shelter/OHS is located at a site operat-ed by the provider. This is a short-term, temporary service.
Waiver Emergency Service — Crisis Mobile Out-reach (aka ‘CMO’)
This service includes the initial call requesting outreach and the immediate, face-to-face, on-site crisis support to a participant experiencing an active crisis which is impacting the participant's ability to function within their family, living situation, and/or community environments.
