If any person suffers loss or damage which is proved to have been caused by any negligent act or omission of Det Norske Veritas, then Det Norske Veritas shall pay compensation to such person for his proved direct loss or damage. However, the compensation shall not exceed an amount equal to ten times the fee charged for the service in question, provided that the maximum compensation shall never exceed USD 2 million. Scope of Work

The following activities were performed during the inspections carried out at Air

Logistics Corporation (ALC), located in Azusa, California (unless stated otherwise for offsite testing). The preparation of the samples was carried out between April 17th, 2012 and June 30th, 2012.

1. Verify calibration of testing equipment.

2. Verify dimensions and identification of the pipe samples.

3. Witness tests of the pipes specimens as per the proposed configuration and procedure.

4. Data review during start-up and completion of the tests.

5. Stamp and sign Air Logistics Corporation reports.

Survey Preparations: 1. Reviewed certificates of calibration for all applicable measurement instruments used during the

preparation of the samples and for monitoring the tests.

2. Verified that all samples were identified and meet the proposed dimensional requirements.

3. Verified the assembly configuration for each proposed test:

a) Enery Release Rate (Gamma Testing)

b) Short Term Spool Test

c) 1000 Hour Lap-Shear (300 degrees farenheit)

d) Impact Test

e) Heat Distortion Temperature (HDT)

Survey Results:

a) Energy Release Rate (Gamma Testing)

• Verified setup configuration of samples as described in the PCC-2, Appendix IV. • Verified Mill Inspection Certificates for test pipes. Test pipe specimens minimum

diameter 100 mm ( 4 inches) and a minimum thickness of 3 mm (0.12 inch). • Verified test stand pressure gauge calibration certificates and monitored steady

pressurization rate.

b) Short Term Spool Test

• Verified the setup configuration of Short Term Spool test samples as described in PCC-2 Standard, Appendix III.

• Verified sample pressure maintained for the period of testing specified in PCC-2 Standard, Appendix III.

• Verified calibration on test stand pressure gauges at Authorized Testing Facility in Riverside , California on June 7, 2012.

• Pipe samples used during the Short Term Spool Testing:

- Sample ID: G03 – PASS (5,550 psig held for 100 seconds) - Sample ID: G05 – PASS (5,650 psig held for 120 seconds)

c) 1000 Hour Lap-Shear (300 degrees farenheit)

• Verified the setup configuration of the samples as described in the PCC-2 Standard, Appendix II-3.

• Reviewed temperature log during 1000 Hour period.

• Tensile testing conducted at Air Logistics Corporation facility in Monrovia,

California on July 10, 2012.

• 5 Samples tested from each lot (2 baseline tests and 2 processed tests)

d) Impact Test

• Verified the setup configuration of impact test on 4 inch sample (Sample #2 from Gamma Test - 20 mm hole) and hole preparation as described in PCC-2 Standard, Article 4.1 (Appendix VI).

• Verified test fixture adequacy to conduct a low velocity impact test with a 0.5 kg (1.1 lb) weight with a 12 mm (0.5 in.) hemispherical indentor.

• Verified Impact Test pipe coupon placement at base of low velocity impact test fixture. The test specimen was supported so that the hole is in the 12 o’clock orientation and the weight strike to the repair at the position of the hole in the test specimen.

• Verified preparations for the low velocity shuttle dropped from a height of dropped from a height of 1 m (39 inches).

• Verified that post impact pressure testing was conducted as described in PCC-2 Standard, Article 4.1 (Appendix VI)

e) Heat Distortion Temperature (HDT)

• Verified the setup configuration of the samples as per described in the PCC-2 Standard.

• HDT coupons with G-03 fabric (Aquawrap) and W-11 fabric (standard resin).

• Verified HDT test coupon placement in testing fixture which produces a precalculated induced flexural stress on the coupon.

• Verified sample temperature at the moment of the test and the temperature log file.

4. Verified collected data during the start-up as per required start-up conditions.

5. Reviewed, signed and stamped Air Logistics Corporation data record sheets.
