Reviews the accomplished form together with all the requirements. Retrieves case records and verifies if the decision has become final and executory (FINEX). (Below 5 yrs.) 10 minutes (5 yrs.& Above) 20 minutes (Archived) 30 minutes Admin Staff/Clerk of the Adjudicator Encodes and issues a Certificate of Finality. 10 minutes Admin Staff/Clerk of the Adjudicator 2 Receives the Certification. Requirements:

1. Accomplished Request Form (DARAB Request Form) or Letter-request;

2. For No. 3 in the above enumeration, a letter-authority and any government issued I.D.

from the principal and the representative;

3. Certified True Copy/ Ecopy of the TCT/EP/CLOA

Duration: 20 minutes- LCMS based

Maximum of three (3) working days if non-LCMS based/archived Applica

Schedule of Availability of Service: Monday to Friday, 8:00 am to 5:00 pm (NO NOON BREAK)

Who may avail of the Service:

1. Any party to the case;

2. Any counsel of record; or

3. Any person authorized by a party/counsel to the case.

Requirements:

1. Accomplished Request Form (DARAB Request Form) or Letter-request;

2. For No. 3 in the above enumeration, a letter-authority and ID from the principal and the representative.

Duration: Below 50 pages - 1 hr. & 5 minutes (active/on-going cases); 1 hr. & 30 minutes (Above 5 years );

1 hr. & 45 minutes (archived)

50 pages & Above- 1 hr. & 45 minutes (active cases);

2 hrs. (Above 5 years);

2 hrs. & 15 minutes (archived)
