mCRPC depends on AR signalling- Androgen deprivation therapy is the cornestone of
treatment for advanced or mestastatic prostate cancer
- 90% of patients respond to first line ADT but, ultimately, most prostate tumours will progress to CRPC despite ADT1,2
- Reactivation of the AR signalling pathway is a key driver in progression to CRPC3,4

Reducción del 44% en el tiempo a la progresión del dolor

CONCLUSIONES• Enzalutamida demostró una eficacia significativa en el paciente
con mCPRC tras quimioterapia:• Supervivencia Global: 37% reducción del riesgo de
muerte. (HR= 0,63).– Mediana de la Supervivencia Global: 18.4 meses versus
13.6 meses en placebo, p<0.001• Supervivencia Libre de Progresión Radiológica: : 60% de
reducción del riesgo de progresión radiográfica o muerte (HR= 0,40).
– Mediana 8.3 meses versus 2.9 meses en placebo, p<0.001
– Mejora de la Calidad de Vida y reducción del dolor– Generalmente bien tolerado en la mayoría de los pacientes
CONCLUSIONES• Es necesario conocer los mecanismos de resistencia
para estratificar pacientes y seleccionar y secuenciar los fármacos
• Enzalutamida parece mantener cierta eficacia tras docetaxel y abiraterona
• Teniendo en cuenta el desarrollo continuo de cambios moleculares y resistencia adquirida a lo largo de la evolución de la enfermedad, se ha de considerar la práctica de biopsia, sólida y líquida, en los momentos de progresión y para monitorizar la respuesta.
