DEED OF UNDERTAKING

KNOW ALL MEN BY THESE PRESENTS:

I, ARMINE REINA T. DE VERA, of legal age, married, and a resident of Aringay, La Union, Philippines, after being sworn to in accordance with law, depose and state:

1. That I am seeking clearance from DON MARIANO MARCOS MEMORIAL STATE UNIVERSITY (DMMMSU), my employer, for my release as an employee with a plantilla position of Accountant II.

2. That being an Accountant/Head of Finance of the Open University System/Mid-La Union Campus of said institution, my position entails much financial responsibility being an Accountable Officer.

3. That currently DMMMSU has received several Audit Observation Memorandum, Notice of Suspension and Notice of Disallowances to which I am one of the persons liable for the Mid-La Union Campus.

4. That the deficiencies are already complied with as far as my office is concerned and as far as those audit reports received by my office prior to my seeking clearance.

5. That I promise to lend assistance to the compliance of any and all other noted deficiencies as far as my office is concern affecting the period by which I was designated as Accountant/Head of Finance even when I am not already connected with DMMMSU.

6. That I also undertake to become liable in my personal capacity even if not already connected with DMMMSU in case the appeal of DMMMSU in the above noted deficiencies would turn out to be futile.

7. That even if a clearance is issued by DMMMSU with a statement of clearing me from all obligations, I shall not use it against DMMMSU especially for those transactions which might be questioned in the future for which I am responsible at during my tenure with DMMMSU as Accountant/Head of Finance for Mid-LA Union Campus and the Open University System.

8. That I am executing this undertaking to guarantee that all my obligations with DMMMSU will be complied with in exchange with the grant of employment clearance from said institution.

BEFORE ME, a Notary Public for and in the City of San Fernando, Province of La Union, personally appeared ARMINE REINA T.
