STATEMENT OF ACCOMPLISHMENTS AND OBJECTIVES ON RESEARCH, TEACHING, AND

SERVICE / OUTREACH

Personal Statement – I have 30 years of faculty experience in applied health policy, outcomes, education, and

workforce research that qualify me to mentor trainees and students, and to conduct, analyze, and disseminate policy

relevant health outcomes and workforce research data to inform policy makers at the institutional, local, state,

federal, and international levels.

After graduating from the University of Arizona (UA) College of Medicine (COM) in 1984, I completed my family

medicine residency at the University of New Mexico (UNM), then worked for 25 years as a faculty member in the

Department of Family and Community Medicine. In 2012, I returned to the UA Health Sciences (UAHS) as a

tenured professor in the Mel and Enid Zuckerman College of Public Health (MEZCOPH) Community, Environment

& Policy (CEP) Department, with a joint academic appointment in the COM Department of Family & Community

Medicine. I currently serve as the UAHS Associate Vice President for Health Equity, Outreach and Interprofessional

Activities. Since 2013 I have served as the Walter H. Pearce Endowed Chair and Director of the Arizona Center for

Rural Health (AzCRH). I served as Professor & Chair (2015-18) of the CEP Department, as its Division Director

(2014-15), and as its Public Health Policy & Management Section Chair (2012-14).

I have been principal investigator (PI) of state, federal, private foundation, contract, and grant awards of $60 million

over my academic career. Since returning to UAHS in 2012, I have been PI of grant awards of $9.4 million with

$4.4 million awarded for the next three years. My work aims to improve health equity, health insurance coverage

and access to quality health care; to expand interprofessional, community-based service-learning models; and to

assure the sustainability of community-based interprofessional activities, leveraging Public Health Service Act,

Social Security Act, Children’s Health Insurance Program (CHIP), and Patient Protection & Affordable Care Act

(ACA) provisions to support rural interprofessional innovative education and training models.

My first Health Resources Services Administration (HRSA) training grant in 1990 explored ways to train and retain

family physicians in rural and underserved areas, culminating in piloting, implementing, and accrediting three new

“1+2” programs with the second and third year of training outside the large metropolitan teaching hospital to learn

in community settings. By augmenting urban, tertiary care training with community-based, rural experiences, the

retention of resident graduates practicing in the state increased from 25% to 50-75%, with half working in federally

designated health professional shortage areas, medically underserved areas, or serving medically underserved

populations (HPSA/MUA/P). Retention success was duplicated with NM’s first dental residency training program. I

was PI of a seven-year, $4.7 million Kellogg Community Voices health professions training and practice innovation.

In 2007-08 I received immersion training in applied health policy as a Robert Wood Johnson Health Policy Fellow

working for U.S. Senator Jeff Bingaman in Washington, D.C. I researched and drafted health workforce provisions

later included in Title V of the ACA. After returning from DC, I applied health policy skills as president of the New

Mexico Medical Society, drafting medical homes legislation for NM’s Medicaid program, and health workforce data

collection legislation requiring licensed health professionals to complete standard data sets at the time of licensing

and renewal. These bills were signed into law.

During my time working for Governor Martinez as the Director of the New Mexico Office of Health Care Reform in

2011-12, I established New Mexico’s state health insurance exchange / marketplace by writing, submitting, and

receiving funding from the Centers for Medicare and Medicaid Services (CMS) for $34.3 million - engaging

advocates, institutions, insurers, providers, consumers, and stakeholders in the process. The NM Health Insurance

Exchange enrollment exceeded 50,000 in 2018.

At UAHS since 2012, I direct the Arizona Center for Rural Health (AzCRH) to advance its mission to improve the

health and wellness of Arizona’s rural and vulnerable populations. I am PI of Center programs - the State Office of

Rural Health (AzSORH), funded by the state of Arizona and the Health Resources and Services Administration

(HRSA); the Rural Hospital Flexibility Program (AzFlex, HRSA); the AzCRH Navigator Consortium (Department

of Health and Human Services, Centers for Medicare and Medicaid Services, Center for Consumer Information &

Insurance Oversight – DHHS/CMS/CCIIO); the Centers for Disease Control and Prevention (CDC) Arizona

Department of Health Services ADHS–AzCRH Intergovernmental Services Agreement (ISA): Prescription Drug

Misuse/Abuse Reduction Initiative: Online Continuing Medical Education Modules; for 2018-21 AzCRH received

an ADHS ISA of $2.2 million over four years to assist with training first responders to administer naloxone to

reverse opioid overdoses in the field, and develop and implement a training program on screening, brief intervention

and referral for treatment to help address Arizona’s opioid epidemic, overdoses, and deaths. This initiative is related

to ADHS’s $3.1 million, four-year award from the Substance Abuse and Mental Health Services Administration

(SAMHSA) First Responder Comprehensive Addiction & Recovery Act (FR CARA) award.

At UAHS, I use data to inform policy at the institutional, local, state, federal, and international levels. Since

returning to UA, I have given 170 invited formal presentations, chaired 12 regional conferences, and given over

300 print, radio, television, and social media interviews. In 2015, I provided written and oral expert testimony on

Rural Health Care Disparities Created by Medicare Regulations and Payment to the U.S. House of Representatives,

Committee on Ways and Means, Subcommittee on Health in D.C. I provide expertise and technical assistance to

states on health reform through the National Governors Association’s (NGA’s) Expert Roundtables and Policy

Academies on issues including health workforce, graduate medical education (GME), and rural health.

I was appointed to the Ad Hoc Committee on Health Care Workforce and invited to provide expert testimony to

inform policy on three issues under consideration during Arizona’s 2018 legislative session: 1) HB 2197 creating a

health workforce database, 2) HB 2324 voluntary certification of Community Health Workers, and SB 1377 dental

therapist bill. The bills were passed and signed into law in May of 2018.

Health Care Reform – Governor Susana Martinez appointed me Director, New Mexico Office of Health Care

Reform in August of 2011. The priorities of the Office were:

a. New Mexico Health Insurance Exchange (NMHIX) to provide consumer choices for high quality care and

to streamline regulatory and administrative processes. I wrote then submitted the level one establishment

proposal on 09/29/2011, and received CMS notice of award for $34,279,483 on 11/29/2011. In 2013,

legislation was passed signed into law creating NMHIX. In 2018, NMHIX enrollment exceeded 50,000.

b. Medicaid Modernization to tailor services to meet the needs of NM’s vulnerable populations; increase

personal responsibility and incentivize healthy behaviors; institute pay-for-performance to reward health

care outcomes (e.g., health homes); increase administrative efficiency (create a single, manageable waiver).

In 2018, NM’s Medicaid enrollment was over 740,000 or 35% of its population.

c. Health Workforce to collaborate with stakeholders, education and delivery systems to improve access to

quality health care, where it is needed.

d. Control Cost Growth to implement payment and delivery reform pilots and programs that measurably

promote coordination and integration of care.

Health Workforce – Interprofessional, Community-Based Education - Service Learning Models – The U.S.

health professions training pipeline begins and often ends in more prosperous and adequately staffed urban areas,

leaving underserved inner city and rural areas, the elderly and other vulnerable populations without access to quality

care. By creating interprofessional training activities in areas of need, in federally qualified health centers (FQHCs),

hospital sponsored rural health clinics (RHCs), and urban, rural and critical access hospitals (CAHs) - access is

improved, and more health professionals are trained and settle into practices where they are most needed.

Working as health staff for Senator Jeff Bingaman as an RWJ Health Policy Fellow, I researched and drafted

provisions for a comprehensive health workforce bill introduced in the 111th Congress - S.790 “The Health Access

and Health Professions Supply Act of 2009.” S.790 provisions included in H.R. 3590 the “Patient Protection and

Affordable Health Care Act” enacted in March 2010 were:

a) Teaching Health Centers – THC’s (Title V, Sec. 5508), $230 million appropriated for 2011-15. THC’s were

reauthorized for 2015-17, and again in 2018.

b) Sec. 5102 State Health Care Workforce Development Grants and Sec. 5103 Health Care Workforce

Assessment, Status: implemented in 2010.

c) Sec. 5207 Increased Funding for the National Health Service Corps (NHSC).

d) Sec. 5315 United States Public Health Sciences Track.

Since my first grant as an assistant professor in 1990, and continuously through 2018, I have served as principal

investigator (PI) of state, federal, contract, and private foundation funding of $60 million. My first two grants were

Health Resources Services Administration (HRSA) Public Health Service Act (PHSA) Title VII, Section 747

primary care training grants from 1990-96 to balance urban, tertiary care hospital family medicine training with

medically underserved, community-based sites. Three new Accreditation Council for Graduate Medical Education

(ACGME) accredited sites were created in Santa Fe, Las Cruces, and Roswell, NM. At the time, just 25% of the

University of New Mexico (UNM) School of Medicine resident graduates remained in NM to practice. The new

family medicine programs retained 50% to 75% of graduates in NM, most ended up practicing in rural and

underserved areas. These were the models for the ACA Title V, Sec. 5508 Teaching Health Centers provision that I

researched and drafted as a health policy staff member working for Senator Bingaman (NM).

New Mexico was 49th of 50 states in per capita dentists. I helped recruit dental faculty in partnership with the UNM

Department of Surgery. With seed funding from the Kellogg Foundation (Health Commons) grant I was PI on, we

created a dental residency program in 2004. Almost two-thirds of the dental residency graduates remained in NM to

practice, and half were underrepresented minorities. In 2008-09, I served on an Institute of Medicine (now the

National Academies of Science) planning committee for an oral health workforce meeting. In the 2018 Arizona

legislative session I provided expert testimony on the state’s oral health workforce.

Over my academic career, I used data to inform policies at the state and federal levels to retain health professionals

in rural and underserved urban areas. I testified at the state legislature about NM’s health workforce shortages, and

recommended programs to address them, resulting in legislation and a permanent appropriation to create the UNM

Locum Tenens academic primary care practice relief program which I directed starting in 1995, and then founded

UNM’s Specialty Extension Services (SES) Program in 1998. I served as director of both programs through 2007. In

2007 the LT/SES Programs provided > 700 days of practice relief coverage per month with more than 250 resident

graduates placed into NM practices where they had provided LT/SES coverage. Of the 500 residents participating in

the LT/SES Programs, half continued to practice in NM after graduation.

The academic practice relief model and resident training in rural community-based venues, received state, national,

and international recognition for improving the geographic distribution of health services to areas of need. I worked

with New Mexico’s Health Policy Commission, Department of Health, and the Medical, Dental and Nursing Boards

to create data tools to inform public policy and improve access to care. A physician workforce survey was

developed, administered, analyzed, and the results disseminated in 2001 and again in 2006. The survey was

incorporated into the physician electronic licensing and re-licensing in 2010.

To assure data could be generated for nursing, dental, physicians and other licensed clinical providers through state

licensing boards, I worked with state legislators to draft SB 14, the “Health Care Workforce Data Collection,

Analysis and Policy Act of 2011” (Sen. Feldman). Licensed health workforce data is used for federal designations

and scoring of Health Professions Shortage Areas (HPSA’s). Thirty-eight federal programs, including the National

Health Service Corps scholarship and loan repayment programs, use HPSA scoring to allocate funds. Accurate,

timely data measures whether underserved urban areas have an adequate health work force. In 2018, a health

professions workforce data bill was enacted in Arizona (HB 2197).

Health Commons – Community-Based, Patient-Centered Medical Homes - New Mexico and Arizona are vast,

rural states sharing a border with Mexico, with most counties federally designated as HPSA/MUA/P. Intractable

health problems - disparities between economic and ethnic groups, poor access to care, chronic diseases, and the

uninsured - have social determinants. The Health Commons was developed by the UNM Department of Family &

Community Medicine with safety net collaborators. It linked medical, dental, care management, behavioral & social

services in interprofessional, community-based, patient-centered primary care homes.

I was principal investigator (PI) for the W.K. Kellogg Foundation’s Community Voices Healthcare for the

Underserved initiative, an award totaling $4.7 million from 1998-2007. The interventions reconfigured the health

system to respond to individual, provider, and community needs including the UNM Care Plan that created a

primary care home for 15,000 of Bernalillo County’s uninsured. The UNM Care findings were published in

Academic Medicine in 2000. Descriptions of the Health Commons model were published in the American Journal of

Public Health in 2002, and the Annals of Family Medicine in 2006. These models framed the insurance and delivery

system reforms necessary to create public-private partnerships to provide health insurance to 45,000 of the state’s

working poor through the State Coverage Insurance. SCI was a Medicaid expansion allowed by a Centers for

Medicare & Medicaid Services (CMS) Health Insurance Flexibility & Accountability Initiative (HIFA) waiver.

These experiences proved invaluable in creating NM’s state based health insurance exchange / marketplace

(NMHIX), catalyzed by the $34.3 million CMS establishment grant that I wrote and submitted in 2012. NM’s

legislature passed a bill creating NMHIX, signed into law by the governor in 2013. In 2017-18, over 50,000 New

Mexicans selected and paid premiums on NMHIX plans.

New health commons model clinic sites were funded, constructed, and opened in three NM counties (Bernalillo,

Hidalgo, Sandoval). The model worked in frontier areas like Lordsburg, without a physician for over a decade, and

never having a dentist. Hidalgo Medical Services soon became the largest employer in Hidalgo County. The model

catalyzes rural economic development, improves health outcomes, quality, and access to care, and provides sites to

train health professionals to learn team-based, integrated, and coordinated care.

As NM Medical Society president, I researched and drafted House Bill 710, a Medicaid Medical Homes bill (Rep.

Picraux), signed into law in 2009. I then worked on helping practices create medical homes programs for the state’s

550,000 enrollees in Medicaid, Children’s Health Insurance Program (CHIP), and State Coverage Insurance (SCI, a

public-private partnership covering 45,000 of the state’s working poor) programs. I served on the Lovelace Clinic

Foundation (LCF) Research Board. LCF was the state’s Health Information Exchange (HIE), that helped hospitals,

physicians and providers to adopt and “meaningfully use” Electronic Health Records.

My current work relates to informing legislative, regulatory and program policy to improve health equity, increase

health insurance coverage, reduce the uninsured, narrow health disparities, and assure a well-trained and distributed

health workforce to meet the health needs of all Arizonans. These principles are reflected in my grant portfolio and

the Arizona Center for Rural Health’s mission to improve the health and wellness of Arizona’s rural and vulnerable
