This document, the 4th version of the Dissemination Plan (D1.22), covers the period from M19 to M24, and aims to update and improve based on the previous versions (D1.7, D1.20 and D1.21) while taking into account the strategies proposed there.
The communication improvements proposed for the previous period have been implemented and relevant outcomes have been obtained, specially while creating awareness about POLYCARE and collaborating with other integrated care related projects. We analyse them and propose additional improvements are proposed in this document to continue with this trend.
This document, also includes the expected dissemination activities proposed by partners for the next period.
In the next version of the Dissemination Plan (M30), the proposed changes and improvements will be re-evaluated and possibly updated once again. Approaching dissemination via this methodology ensures continuous improvement and a higher impact.

1 INTRODUCTIONThis deliverable of the Dissemination Plan identifies needs and interest of stakeholders and compares it with current dissemination materials. Where necessary improvements are proposed.
The objective is to achieve and continuously improving approach with regards to exploitation and dissemination resulting in an enhanced impact.
The document takes the methodology and the communications actions described in the previous versions (D1.7, D1.20 and D1.21) referencing in some points of this versions.
1.1 Intended audience This document is oriented to:

1.2 Overview of the documentThe document structure comprises the following sections:
Section 3 explains the result of the implementation of the communication improvements planned for the previous period.
Section 4 provides the expected dissemination actions proposed by the consortium for the next reporting period – from January 2018 to June 2018 (M25-M30).
Annex 1 includes an update with the communications actions proposed in the first version of the Dissemination Plan, in terms of improvements. It includes proposes additional means to increase POLYCARE awareness and disseminate project results.

2 COMMUNICATION IMPROVEMENTS
2.1Analysis of previous communication improvements
2.1.1Website optimization and social mediaThe website structure has not been changed during the period from M19 to M24 whereas the content and information has been updated as the project progresses. By means of the “contact” tab, POLYCARE has received over 10 requests for further information on the project or requesting the possibility to take part and be aware of project activities.
It was requested during the project review to include the project duration information in the website and get updated the last calendar events as well and both aspects have been done.
With regards the social media communication, during the past months it has been increased the presence in the social media pushing new contents and materials related to the POLYCARE project and integrated health care in general. The statistics results regarding the management and invigoration for this period continues achieving the objectives initially stated. The intensity and frequency of content publication (mid-low) and the type of published contents (3 rd party contents periodically and self-generated contents punctually) predetermine the engagement related data obtained. Being the fact that the pilots do not have yet started, the impact of the contents published is limited and it is expected to increase as POLYCARE results arise.
On the other hand and relative to the community data obtained, all social network profiles have increased the volume of followers, being remarkable this increment in Twitter. The only community which has not increase the community is YouTube, mainly due to the fact that no videos have been published during the current reporting period.

2.1.2NewslettersThe first POLYCARE newsletter was released during M19 and it was distributed by means of the several dissemination channels and also internally at each of the project participants. It is planned to periodically release a newsletter on POLYCARE or integrated care related topics.
2.1.3Collaborations with other projectsInitial collaboration with other project focused during M13 to M18 on disseminating results and pieces of news of interests leveraging each other’s social media channels. During the current M19-M24 period we have taken this collaboration to a next level and we have proposed to one of the most relevant integrated care events in Europe - ICIC18 (International Conference on Integrated Care) – a workshop on the most relevant aspects that each of the projects (POLYCARE, CONNECARE, ICT4Life, ProACT and Caregiverspro) is bringing to the European scene. POLYCARE is leading this effort with the main target of sharing integrated care knowledge, experiences and increase the awareness of our projects. We are awaiting for the feedback from ICI18 organizers that should come around February 2018.

4 ANNEX 1: COMMUNICATION ACTIONSThis section shows the proposed new communication improvements for the next period of the project and is based on the specific objectives and messages defined in the first version of the Dissemination Plan (D1.7) submitted in June 2016.
4.1 Website optimization Description of the action
For the next reporting period, there are significant website structure modifications planned. Each partner will include updated information about the project and links to the central website to deal with the objectives reported below.
