Tryouts: Monday, August 19th at 6pm at Pines Ice Arena Cost $25 – please bring cash or check made out to “Cypress Bay Lightning” the night of tryouts. What you will need to bring to tryouts:

1. Copy of school ID with picture for the 2019-20 school year 2. Copy of school schedule for 2019-20 school year 3. Copy of the last report card. In order to play, GPA must be above a 2.0 4. All hockey players must wear gear and a light colored jersey for offense and a dark colored jersey for

Please print and fill out the forms listed below. We will need:

• Consent to Treat • FHSAA EL2 – This must be done at the doctor’s office – doctor must sign and stamp this • FHSAA EL3 • Code of Conduct • 2019-20 USA Hockey Registration number

CBH Practice:

• Monday, September 9, September 16 and September 23 at Pines Ice Arena, Time – TBA

CBH Games:

• Monday nights, some Sundays and some Friday nights. Schedule will be placed into Team Snap and the school website as soon as we receive it.
