Training and Experience I have no gaps in my training. I have trained through the traditional route of PRHO/SHO/SpR prior to changes with MMC. As an SHO I completed a Basic Surgical Rotation in a variety of specialities relevant to plastic surgery including, more unusually, anatomy demonstrating and ending with 18 months of plastic surgery post MRCS. This

gave me an excellent grounding for the subsequent 6 years as a plastic surgery trainee in the Southwest UK, during which I took the FRCS Plast, and on completion was awarded CCT in Plastic Surgery in 2011. The core of training, in which I have been exposed to all subspecialties, has been spent in Plymouth and Bristol. Additional fellowships in internationally renowned centres in my subspecialist interests have further strengthened my experience. In October 2011 I was appointed as a fixed term consultant at NBT in particular to support the lower limb, breast and sarcoma services. In addition to these specific duties I have contributed to the on­call rota, and supported the plastics trauma and skin cancer services. I have worked hard, safely, efficiently, effectively and maintained a good rapport with my consultant colleagues, management and other core team members as well as embracing my duties as a trainer and mentor. I have a track record of satisfied patients and keep my own database for purposes of audit and research. Subspecialist Interests and Fellowships 1. Lower Limb Surgery I have been inspired in this emerging subspecialty by the influence of my mentor, Mr. Umraz Khan. I have worked on a specific lower limb firm for six months, as well as being actively involved in lower limb cases throughout my career and during two microsurgical fellowships. As a trainee I performed over 50 free flaps in the lower limb, as well as developing competencies in hindquarter, above and below knee amputation, and local flaps including gastrocnemius, gracilis, dorsalis pedis and medial plantar/instep flaps. I work well with orthopaedic colleagues, have attended the relevant AO courses, and have experience in frame fixation and limb shortening. In 2010, I took the initiative to work with an international leader in the subspecialty, Professor Zoran Arnez in Cattinara Hospital, Trieste, Italy. This fellowship post was given prospective training approval, and funded through a BAPRAS travelling award as well as an Ethicon Bursary. Both awards were given in open competition, and the maximum award approved. During this 4 month post I was able to participate in the management of a large number of complex cases, simply not possible in the majority of UK centres within the same timeframe. On return, my report and reflections on training in the UK compared to Italy were published in the College Bulletin. Since appointed consultant I have performed over 200 flaps at NBT for complex lower limb trauma, as well as assisting in over another 100. I have also performed revisional elective surgery in another 100 lower limb patients. I have developed a good rapport and working relationship with orthopaedic colleagues at NBT and the frame surgeons at the BRI. I have set up a joint lower limb clinic in addition to the pre­existing clinic led by Umraz Khan which was overburdened by the demands on the service and waiting times have decreased to satisfactory levels in consequence. I have also worked with Umraz Khan to develop an outreach service to Gloucester Royal Hospital on a spoke­and­hub basis. A number of cases have been performed at GRH which has enabled the service to be delivered closer to home for patients and saved transfer costs and bed occupancy at NBT. I appreciate the costs involved in delivering the service and have met with coders to discuss ways to improve correct remuneration I have a number of publications and presentations in this field, and was invited faculty on the BAPRAS advanced educational course on reconstructive limb surgery in Manchester in September 2014 and the AO orthoplastics conference in Coventry April

2012. Feedback from both courses was very positive and I look forward to expanding this role. I am involved in the process of setting up an international orthoplastics course with a predominantly Bristol led faculty, as well as forming an orthoplastics subspecialist interest group within BAPRAS and the BOA. I firmly believe we have the opportunity to establish NBT as an international leader in orthoplastic reconstruction. 2. Sarcoma I have had a long interest in sarcoma and its reconstructive implications. I have worked with sarcoma surgeons in Plymouth, Bristol and Nottingham and have regularly attended the supra­regional sarcoma meetings and audit. I have related publications and presentations. I subscribe to Sarcoma UK, have attended their workshops and completed the BAPRAS advanced course. As a trainee I became competent in forequarter and hindquarter amputations as well as lower level amputations and chest wall reconstruction techniques. In addition, my experience in microsurgery and limb reconstruction lends itself towards this subspecialty which often involves the limb and often requires flap, sometimes free flap reconstruction. In May 2010, I took a month unpaid leave to work with Professor Meirion Thomas at the Marsden Hospital London, with particular exposure to the management of intra­abdominal and retroperitoneal sarcoma which is generally not possible as a plastic surgery trainee. There I also learned tru­cut biopsy techniques and the use of Ultrasound imaging for guided biopsy. I worked with Graeme perks at Nottingham as an oncoplastic reconstruction fellow. This included a weekly sarcoma MDT and clinic and all day sarcoma list. I operated independently on a number of major sarcoma cases and set up a sarcoma database for the purposes of audit and research. Since February 2012 I have worked alongside Paul Wilson at NBT as a core team member of Sarcoma MDT. I attend the weekly sarcoma MDT and clinic and have developed a good working relationship with members of the team. I have performed over 300 service­related operations including major sarcoma and reconstruction. I attended the internal peer review and am working with Paul Wilson on ways to streamline and still improve the service, including the setting up of outreach clinics Postgraduate Clinical Courses Completed

Audit I have been in involved in audit activity throughout my career and have presented at departmental, interdepartmental and regional meetings. My aim has been to complete two projects a year. A number of projects have led to demonstrable improvements in outcomes and have been presented nationally or regionally. In response to the Darzi white paper, and even more recent commissioning changes, I have focused lately on auditing quality of patient­derived outcome measures rather than simple medical audit. Moreover, as consultants we can no longer expect our complications to be collated and presented at junior level as has historically been the case. I keep a database of all my procedures and complications for audit and research purposes and have presented them myself at departmental audit. Clinical Governance & Management

I subscribe to Hospital Doctor and receive bulletins from the institute for innovation and improvement and health service journal online. I have attended the regular consultant and departmental business meetings and, I believe, made valuable contributions to them

I am a member of the Pressure Sore Steering Group. We have developed pathways of referral within NBT for new pressure sores as well as the ‘skin bundle’ to reduce inpatient pressure sore frequency. We perform RCA for all grade 3 or 4 pressure sores arising in the trust. This has important implications for CNST as well as CQUIN targets.

I have worked with Umraz Khan to formalise pathways of referral and treatment within the major trauma network.. I have worked with and presented to coders to improve their understanding of the speciality and attain the correct coding categories and remuneration.

I have produced a variety of patient information leaflets. Recently, I have worked with Caroline Oats, breast care nurse, to produce better patient information on fat transfer and set up a database of the large volume transfers performed in NBT. I am working with Chris Millman (Sarcoma Nurse) and the Macmillan team to improve patient information for sarcoma.

I managed the department rota at Frenchay for 12 months. I maintained a healthy rapport between 27 juniors, maximised training opportunities while maintaining the heavy service needs. During my leadership, juniors were new deal and EWTD

compliant. I instituted changes to an open, accessible and fair system for booking of leave which had been a previous point of conflict. I also created a new working arrangement for registrars, giving each a stable job plan. This led to better communication and continuity for staff and patients. Feedback from juniors was also extremely positive. I have assisted in the production of an induction programme for CT1/2s in the department and an induction/information booklet.

I am responsible for recruitments of junior doctors in our department, writing adverts and job descriptions, selecting candidates on NHS Jobs and Interviewing.

Teaching I enjoy teaching and take every opportunity formally and informally to educate students, medical and nursing staff. Specific duties include:­

I have a long association with Bristol University. I am a member of the Senior Common Room and Ex­loco tutor at Manor Hall. I was elected as a Convocation representative to Bristol University Court, effectively the University governing body. I have experience setting, observing and marking undergraduate examinations as an anatomy demonstrator, and am a member of the interview panel for medical and dental admissions. I contributed to designing a new undergraduate medical curriculum at Bristol University. I have supervise a number of medical students taking special study modules in our department.

I am an educational supervisor and attend the ARCP meetings for Southwest Plastic surgery trainees. I have experience as faculty on Basic Surgical Skills, ESPRAS, BAPRAS Educational and AO Courses. I am very enthusiastic about my speciality. I presented for the speciality at a Careers Day for medical students and foundation years at the Peninsula Medical School. I have set up an exchange of trainees between the UK and Cattinara Hospital in Italy.

I was responsible for Creation of a teaching rotafor registrars in plastic surgery in

Plymouth covering the BAPS syllabus over a period of three years, creation of a teaching rota for SHOs in surgery at Derriford Hospital to include core parts of the MRCS syllabus, initiation of a plastic surgery trauma teaching timetable specifically for the accident and emergency SHOs at Derriford Hospital, and delivery of a variety of talks while an SHO and registrar in plastic and reconstructive surgery using Microsoft Powerpoint™. I initiated grand rounds and practice Vivas for the registrars in plastic and reconstructive surgery while in Nottingham.
