Longitudinal Effects of Multiple Pollutants on Child Growth, Blood Pressure and Cognition. EPA/RD83479801 Site PI ($100,000) The purpose of this project is to study the effects of urban air pollution on somatic growth, blood pressure and cognition, and on related intermediate inflammatory and endocrine outcomes in an ongoing pre-birth cohort from the Boston Metropolitan area. Using longitudinal repeated measures of our outcomes in Project Viva, we will examine the effects of individual pollutants, mixtures and sources. We will also consider factors influencing vulnerability and susceptibility to pollution.

2011-2015 Breastfeeding Promotion RCT: Adolescent Neurocognitive and Respiratory

Function NICHD/ R01HD050758 Multiple PI ($1,812,446) The goal of this study will be to test, in an intention to treat analysis, the extent to which exclusive and prolonged breastfeeding causally influences later health and disease. The proposal is based on the 15-year follow up of 17,046 healthy infants who originally participated in the Promotion of Breastfeeding Intervention Trial (PROBIT), a cluster RCT in the Republic of Belarus and will measure whether the experimental breastfeeding promotion intervention has effects at 15 years on neurocognitive and respiratory function.

The HMORN UCSF Center for Diabetes Translational Research will actively foster and support translational research in diabetes within health care delivery systems affiliated with the HMO Research Network (HMORN) while developing new collaborations with publicly-funded healthcare systems in the State of California.

2012-2017 BPA, Phthalates & Stress: Mechanisms and Interactions for Childhood Obesity NIH/NIEHS R01 ES21357 Site PI ($363,747) Using data from an ongoing pre-birth cohort study in Mexico City, we will examine associations of prenatal exposure to bisphenol A (BPA) and phthalates with disrupted maternal-fetal hypothalamic pituitary adrenal (HPA) axis function and with child adiposity, insulin resistance, and other cardiometabolic outcomes at ages 4 and 7 years.

2012 – 2017 Prenatal Exposure to polyfluoroalkyl chemicals (PFCs) and child growth and development NIH/NIEHS R01 ES21447 Site PI ($605,338) The main objective of this study is to investigate the association between prenatal polyfluoroalkyl chemicals (PFCs) exposure and growth and neurodevelopment in children using data from the Project Viva cohort.

2012 - 2015 Novel Epigenomic Biomarkers, Prenatal Risk Factors, and Childhood Obesity NIH 1R01 NR013945-01 Co-Investigator In This project, we seek to identify novel methylomic biomarkers on cord blood DNA that could be used to reconstruct at birth obesogenic fetal-life experiences and to profile risks of childhood obesity and related cardiometabolic phenotypes. We will use conduct methylomic screenings of ~486K methylation sites on cord blood DNA from Project Viva, and validate our findings in an independent replication set of mother-child pairs from cohort (Generation R) with similar design and individual characteristics.

2013 - 2018 The Fetal and Childhood Environment, Oxidative Balance, Inflammation and Asthma NIH R01AI102960 Multiple PI ($4,015,218) Fetal life exposures to dietary antioxidants and modifiable sources of oxidative stress (overweight, cigarette smoke, traffic pollution or acetaminophen) may have long-term effects on risk of these diseases, and risk may be further reduced or amplified by later childhood exposures. By defining the longitudinal progression of epigenetic, inflammatory, and heterogeneous clinical respiratory responses to modifiable exposures influencing oxidative balance, this study will make a unique contribution to development of strategies for prevention and targeted treatment of asthma and allergic disease.

Narrative Report My interest in health throughout life first led me to pursue a combined clinical training in internal medicine and pediatrics, and subsequently to embark on a research path emphasizing a life course approach to health and disease. My research focuses on nutrition and related behaviors during pregnancy, the postpartum period, and early childhood, and the influence of these experiences on the health of both mother and child. One major focus of my research has been the balance of toxicant risk and nutritional benefit from fish consumption during pregnancy, and the implications of this controversy for women’s behavior and health policy. I have led several studies in this area of investigation using data from Project Viva, an NIH funded pre-birth cohort study of women and their children on which I am Co-Principal Investigator, as well as other data sources. I have explored associations of maternal n-3 and n-6 fatty acid consumption with outcomes of pregnancy such as gestational hypertension, preeclampsia, preterm birth, and fetal growth, as well as longer-term outcomes such as child cognition, obesity, and asthma. I have presented results from these studies at national and international conferences, and my work was highlighted in the 2007 IOM report “Seafood Choices: Balancing Benefits and Risks.” I have been supported in this work by a K23 career development grant from NICHD and an R01 from NIEHS. A second area of research focus is modifiable maternal behaviors during the peripartum period and their associations with maternal and child overweight and related cardiovascular disease

risk. I have published studies on associations of maternal diet and physical activity with pregnancy outcomes such as gestational diabetes, gestational hypertension, preeclampsia, and postpartum weight retention. I have studied associations of maternal smoking with child overweight in a research paper and performed a meta-analysis on the topic. I presented findings from studies of gestational weight gain and offspring overweight in childhood before Institute of Medicine Committees on Maternal Weight and Weight Gain. I am collaborating on studies to follow up mothers and children enrolled in randomized controlled trials in the peripartum period, such as interventions to prevent excess gestational weight gain and promote breastfeeding. Since 2000 I have practiced as a primary care physician at the Gretchen and Edward Fish Center for Women’s Health at Brigham and Women's Hospital. My panel includes male and female patients from adolescents to the elderly. I have a particular interest in medical care for women during the peripartum period. I have also taught in several HMS courses including Patient Doctor I, Patient Doctor II, and Clinical Epidemiology, and continue to lecture in courses at HMS, HSPH, and elsewhere.
