Form 3I am keen to obtain a position within this facility in order to further develop my skills, knowledge and

experience as a registered nurse within the occupational environment. I have future goals to engage

in post graduate study with an aim to specialising in areas such as wound management, stoma

care, remote area, aged care/dementia, mental health nursing, as well as specialised clinical areas

Employment History

10 /11/2014- to current time- Registered Nurse/ Practice nurse at Quacks on Bamford Lane (Private Practice Medical center), Mater hospital (on call) RN,Townsville General Hospital (on-call)RN, Medimobile Mobile Vacination Clinic (Immunisation Nurse).

I Currently work a part time position at the Quacks on Bamford Lane- a private practice medical centrer. This practice has five current MD’s as well as periodical Medical students.

1. Duties include providing direct nursing care (triage) to clients who phone or walk in.

2. Immunisations for adults and children including baby and toddler health checks. This includes travel vacinations as well as influenza and Q fever.

3. Baby and child growth and development recording- weight height etc. As well as the government 4 year old health checks.

4. Set up and assistance with minor operations, mirena and implanon insertions.5. Sterilsation of instruments, stock control and ordering.6. ECG, spirometry, Hearing and sight checks. Use of the IMP machine.7. Wound and infection checks, suture removals, minor wound dressings. Care of

chronic wounds including leg ulcers. 8. Iv medication administration- short term triage.9. Working knowledge of Medical director and Prac soft ware.10. IM,SC and IV managments.11.Pap smear and swab registration, with pathology Labs.12.Halter monitoring for ECG 24 hours. 13.Billing and Medicare- reception duties when required. (Use of prac soft software

systems.)14.Booking for clients with GP’s, cash and epos management.15.Phone consultations for call back case managment. 16.Client education on health management.17.Emmergency triage of accidents in conjuntion with GP’s.18.Medical assessments for employmers.

Mater hospital (on call) 11/03/2014-

I have worked on call at the Mater Hospital duties involed all areas of patient care within the assigned areas of work- cardiac, surgical and medical.

I adhere to and abide by the Occupational safety and policy framework for both the Mater Hospital and the Townsville hospital employees.

Townsville General Hospital (on call) 03/3/15- I work on call at the Townsville hospital across multiple departments- General, Medical,

Palliative, Aged care, Dementia, Mental Health, Paeds, Emergency, Rehabilitation. I endeavour to deliver the safe provision of care and have a commitment towards

advocating for patients and clients. I do so by keeping abreast of the ethical and legal dimensions of nursing as covered under the Queensland Health Department

Medimobile Mobile Vaccination Service

I work for a private vaccination clinic that services both the public and private sector for mainly Flu vaccinations.

I travel to rural and remote locations to service isolated communities. I ensure safe medication administration and preserve the cold chain for all vaccines.

10/03/14-30/11/14- Registered Nurse –contract- Palliative Care Unit- Townsville Hospital:

1. Duties include providing direct nursing care to not only palliative patients but also any outlie patients that may be temporarily put in our department. I have worked under the direction of Shift co-ordinators/CNC and NUM to co-ordinate patient care planning including liaising with RMO, physios, speech, OT, Social workers, Discharge planners etc in direct patient care provision.

2. I actively respond to the physical, emotional, spiritual and cultural needs of clients included within the Activities of Daily Living.

3. I work with the client and their families to control symptoms of chronic diseases as well as having an educative role in all areas of their disease progression.

4. I am proficient in the use of sub cut intimas and Nikki pump syringe drivers. I am proficient in the use of IV medication administration/ IM/SC mediations administration.

Gaps in Work / Practice History

From 1995- 2007 I undertook family responsibilities and worked from home as a home day-care provider, childcare assistant and teacher’s aide while taking time to raise my children and provide direct nursing care to my father who was suffering from Parkinson’s disease.

I proceeded in 2007 to undertake a period of study with the competency assessment board to regain my nursing qualification to practice (2007).

I then went on to work in Canberra in 2008, returning to Townsville and commencing work with the Townsville General Hospital and starting the Bachelor of nursing science degree.

In 2012 I undertook additional study to gain my medication endorsement with the attainment of Diploma of nursing (division 2) through the Southbank Institute of TAFE.
