In-Network Plan (Culinary Network Dentist)Deductible: NoneMaximum Yearly Benefit: No MaximumCo-pays: Depending on the service

• Dental services must be from a Culinary Network dentist in the Nevada Dental Benefits (NDB) group.

• There are no co-pays for examinations, basic cleanings, x-rays and silver fillings.

• Orthodontics (braces) are covered for you and your eligible dependents (see page 5 and 6 for your benefits).

• Dual Culinary Coverage: If you and your spouse are both participants in the Culinary Health Fund Dental Plan, you and your eligible dependents will not have any co-pays.

• Anesthesia for some extractions are covered.

• Benefit eligible dependents (under 19): For regular 24-month treatments*, the co-pay is $850 ($300 at the beginning of treatment and then $50 for 11 months).

• Benefit for you and your eligible dependents (age 19 and over): The co-pay is $3,100 ($900 at the beginning of treatment and then $50 every month for 36 months, plus a maximum of $400 at the end of active treatment).

• You will only receive your orthodontic benefit during the months that you are eligible and are in treatment.

If you and/or your dependent already have braces and are new to the plan, Culinary will not cover your treatment.

* For eligible dependents (under 19):

Dental treatment depends on the diagnosis. If treatment will be longer than 24 months or requires additional appliances (a device that is used to move or position teeth during treatment), you may have to pay additional costs.

• You may go to any licensed dentist of your choice in the United States.

• Nevada Dental Benefits will pay for charges that are covered up to the total listed in our “Schedule of Allowables”. You will pay the difference between what NDB pays and what the Non-Culinary Network dentist charges.

• Dual Culinary Coverage: If you and your spouse both have Culinary dental benefits, you and your eligible dependents may still have co-pays and extra costs.

Out-of-Network Plan (Non-Culinary Network Dentist)
