How to Succeed in this Course Complete the course orientation presentation and syllabus quiz in Brightspace before beginning. Obtain suitable employment and submit the Site Approval Request Form. Work in sales and or

marketing is preferred however, due to some remaining COVID19 limitations work experiences with a service component will qualify. Keep in mind, seeking an internship in sales is still the best option for professional development.

Review the Internship Search Guide at the end of this syllabus if you need assistance finding a position.

Register for CSR 30700 within the semester in which the work and academic hours are being completed.

Read and sign a notice and Assumption of Risk form; in Blackboard. Must be completed to receive a grade.

Work a minimum of 140 hours AFTER the Site Approval Request Form has been verified and approved by Keith Molter. No work hours may be counted towards the required minimum of 140 hours until the Site Approval Request Form is submitted and approved! Assignments turned in prior to this approval will not be accepted or graded.

Complete all course assignments in a timely (by the due dates) and professional manner in Brightspace.

Represent Purdue University and the College of Health & Human Sciences in a professional manner on the job at all times.

Complete satisfactory on-the-job performance. If your job performance is not satisfactory or you are terminated for just cause, we reserve the right to assign an “F” for your course grade.

Provide and maintain verification of total hours worked through pay stubs which reflect cumulative work hours and/or weekly log signed by your work supervisor and submitted on a weekly basis. Hours worked must be verified and stated through time sheets or pay stubs. A misrepresentation of hours-worked will result in a failing grade.

REMIND your Work Supervisor to send the Midway Supervisor Evaluation and the Final Supervisor Evaluation DIRECTLY to the instructor. It is your responsibility to remind your work supervisor at least a week before so he/she is certain to complete the form and mail/email it as a PDF document on time. NO HAND DELIVERED SUPERVISOR REPORTS FROM STUDENTS WILL BE ACCEPTED. The grades for these two assignments come directly from your supervisor based on their evaluation of your performance.

ATTENTION International Students: It is VERY IMPORTANT that you consult with the International Students & Scholars office in Schleman Hall and complete CPT paperwork if you are completing any work experience or volunteer experience in the United States.

Assignments All assignments are due at 11:59 PM, ET on the specified date. I encourage you to “mentally enroll” in the academic portion of this course. Day 1 of the work week

begins on Monday mornings and ends of the following Sunday evening. I will generally post reminders or send class emails on Sunday afternoons. All course grades and scores will be posted in Brightspace. Grades will not be curved. The more

points that each individual collects during the term, the higher the grade they will receive. Standard rounding rules will be used, in the case of partial points. Final grades will be given based on the following:

Final Paper Written Term Paper

Sun., Dec 5 (by 11:59 p.m., ET). Turned is as one document in Brightspace. If you fail to turn in your final paper or verification of hours, you are not eligible for an Incomplete grade. Keep a copy of your entire report. This internship is a form of independent study and all feedback comes at the end of the course when you receive your grade and pick up your papers. Do not expect any feedback during the semester other than for the Site Approval Notification and deadline reminders. Final Reports will not be accepted after the due date!

Midway Supervisor Evaluation

15 points

October 17. This document should not be turned in at the end with the final eval! The Evaluation shall be in PDF form and emailed to the lecturer directly from your supervisor’s email address. The supervisor should sign and date the Mid Semester Evaluation. Under no circumstances should the Mid Semester Evaluation come from you or through your hands. If it does, it will count as zero points. Non-receipt of the Midway or Final Supervisor Evaluation will result in a "0."

Final Supervisor Evaluation

Attendance Policy during Covid-19 Find the link in the Brightspace page on Attendance and Grief Absence policies under the University Policies menu. Because this course is delivered virtually, we will focus on participation and contribution through online check-ins, assignment receipt, email correspondence, and office hour discussions. In the current context of COVID-19, in-person attendance will not be a factor in the final grades, but you (the student) still need to inform the instructor(s) of any conflict that can be anticipated and will affect the submission of an assignment or the ability to contribute to a group project. When conflicts can be anticipated, such as for many University-sponsored activities and religious observations, the student should inform the instructor of the situation as far in advance as possible. For unanticipated or emergency conflict, when advance notification to an instructor is not possible, the student should contact the instructor as soon as possible by email or by phone. When the student is unable to make direct contact with the instructor and is unable to leave word with the instructor’s department because of circumstances beyond the student’s control, and in cases of bereavement, quarantine, or isolation, the student or the student’s representative should contact the Office of the Dean of Students via email or phone at 765-494-1747.

Academic Guidance in the Event a Student is Quarantined/Isolated:

If you must quarantine or isolate at any point in time during the semester, please reach out to your TA, or [email protected] via email at your earliest opportunity so that we can communicate about how you can continue to learn remotely. Work with the Protect Purdue Health Center (PPHC) to get documentation and support, including access to an Academic Case Manager who can provide you with general guidelines/resources around communicating with your instructors, be available for academic support, and offer suggestions for how to be successful when learning remotely. Your Academic Case Manager can be reached at [email protected]. Importantly, if you find yourself too sick to progress in the course, notify your academic case manager and notify Keith Molter via email or Brightspace. We will make arrangements based on your particular situation. Please note that, according to Details for Students on Normal Operations for Fall 2021 announced on the Protect Purdue website, “individuals who test positive for COVID-19 are not guaranteed remote access to all course activities, materials, and assignments.”

How long must I miss class and quarantine if I have a high-risk exposure to someone who has COVID-19? A high-risk exposure is defined by the CDC as coming within 6 feet of an infected individual for 15 or more minutes where both individuals are not wearing masks. Full document The Protect Purdue Health Center’s medical protocol, based on CDC guidance and scientific reports, requires that:

Fall 2021 An unvaccinated individual who has had a high-risk exposure may not attend class and

will be required to quarantine for 14 days after any and every high-risk exposure, regardless of symptoms.

A vaccinated individual who has had a high-risk exposure may continue with normal day-to-day activities, including attending class, and will not be required to quarantine as long as they remain asymptomatic.

All students, including those in isolation or quarantine, may access and will receive medical care and support from PPHC and Purdue University Student Health Center (PUSH) as normal.

Classroom Guidance regarding Protect Purdue The Protect Purdue Plan, which includes the Protect Purdue Pledge, is campus policy and as such all members of the Purdue community must comply with the required health and safety guidelines. Required behaviors in this class include: staying home and contacting the Protect Purdue Health Center (496-INFO) if you feel ill or know you have been exposed to the virus; properly wearing a mask in classrooms and campus building, at all times (e.g., mask covers nose and mouth, no eating/drinking in the classroom), disinfecting desk/workspace before and after use, maintaining appropriate social distancing with peers and instructors (including when entering/exiting classrooms), refraining from moving furniture, avoiding shared use of personal items, maintaining robust hygiene (e.g., handwashing, disposal of tissues) prior to, during and after class, and following all safety directions from the instructor. Students who are not engaging in these behaviors (e.g., wearing a mask) will be offered the opportunity to comply. If non-compliance continues, possible results include instructors asking the student to leave class and instructors dismissing the whole class. Students who do not comply with the required health behaviors are violating the University Code of Conduct and will be reported to the Dean of Students Office with sanctions ranging from educational requirements to dismissal from the university. Any student who has substantial reason to believe that another person in a campus room (e.g., classroom) is threatening the safety of others by not complying (e.g., not properly wearing a mask) may leave the room without consequence. The student is encouraged to report the behavior to and discuss the next steps with their instructor. Students also have the option of reporting the behavior to the Office of the Student Rights and Responsibilities. See also Purdue University Bill of Student Rights.

Academic Integrity Students suspected of cheating or being unethical on an assignment, quiz, or exam may not receive a grade for that assignment or exam or may fail the course. This course adheres to the Purdue policy on academic dishonesty as such:

Purdue prohibits "dishonesty in connection with any University activity. Cheating, plagiarism, or knowingly furnishing false information to the University are examples of dishonesty."
