was for peach brandy. One of his congregants would make him a bottle each Christmas. One year, when the minister went to visit his

Spine surgery may be safer at hospitals than outpatient facilities(Reuters Health) - Patients who get spinal surgery at outpatient centers may be more likely to have serious complications or require repeat operations than their counterparts who get these procedures in a hospital, a U.S. study suggests.Researchers focused on an o p e r a t i o n k n o w n a s a n t e r i o r c e r v i c a l discectomy and fusion (ACDF), which involves removing a damaged disc in the neck to reduce pressure on the spinal cord or nerve root that can cause p a i n , n u m b n e s s a n d weakness. Most of these surgeries are done in hospitals with a one or two night stay, but a growing number of people are going instead to outpat ient centers that may have lower costs in part because they don’t keep patients overnight.T h e s t u d y l o o ke d a t o u t c o m e s f o r 1 , 2 1 5 p a t i e n t s w h o h a d outpat ient ACDF and 10,964 people who had these operations in a hospital between 2011 and 2016.Overall, there were few complications, researchers r e p o r t i n T h e S p i n e Journal.One year after surgery, 5.5 percent of the people who had outpatient surgery needed repeat operations, as did 4.1 percent in the inpatient group.Af t e r a c c o u n t i n g f o r i n d i v i d u a l p a t i e n t characteristics like age,

gender and other health problems, the researchers found that people who had outpatient ACDF were 79 percent more likely to require repeat operations within one year than p a t i e n t s w h o h a d operations in a hospital.Outpatients were also 25 percent more likely to experience postoperative kidney failure.“We were surprised that the outpatient cohort had g r e a t e r r a t e s o f postoperative renal failure since these patients are typically younger and hea l th ie r t o undergo surgery in the outpatient setting,” said senior study author Dr. Don Young Park of the David Geffen School o f M e d i c i n e a t t h e University of California, Los Angeles.“Our study is the first to show that outpatient ACDF is associated with some increased risk, even in ideal surgical candidates,” Park said by email.All of the patients in the s t u d y h a d i n s u r a n c e through Humana.In both the inpatient and outpatient groups, half of the patients were at least 65 to 69 years old.T h e s t u d y w a s n ’ t a controlled experiment designed to prove whether or how the location of surgery might influence the outcomes.Another limitation of the study is that it relied on insurance claims data and lacked detailed medical information on individual

patients, the authors note. Researchers also didn’t h a v e d a t a o n e a r l y complications such as emergency room visits or hospitalizations.Individual patient factors missing from the insurance claims data might explain the slight differences in o u t c o m e s b e t w e e n inpatient and outpatient o p e r a t i o n s , s a i d D r. M a t t h e w M c G i r t o f Carolina Neurosurgery & S p i n e A s s o c i a t e s i n Charlotte, North Carolina, who wasn’t involved in the study.“There was no way to show the patient groups were similar with regards to risk factors for failed fusion,” McGirt said by email.One factor that might have made a d i f f e rence i s smoking status, noted Dr. John Ratliff, a researcher at S t a n f o r d U n i v e r s i t y M e d i c a l C e n t e r i n California who wasn’t involved in the study.“Smoking cessation is important in preventing p o s t - o p e r a t i v e complications,” Ratliff said by email. “Unfortunately, this study does not assess smoking status.”Still, the findings add to the evidence that these operations can be done safely, but should be considered only after other t r e a t m e n t s s u c h a s physical therapy, pain medication or steroid injections fail, doctors say.“ S u r g e r y s h o u l d b e reserved for patients with severe pain despite an

appropriate course of non-operative treatment or for those with neurologic deficits,” said Dr. Frank Phillips, a researcher at Rush University Medical Center in Chicago who wasn’t involved in the study.“In appropriately selected patients, the success rate for ACDF procedure in t e r m s o f i m p r o v i n g symptoms is generally above 90 percent,” Phillips said by email.Part of that selection process should involve c o n s i d e r i n g w h i c h patients have the lowest complication risk and may be the most appropriate candidates for outpatient operations, said Dr. Daniel Refai, a researcher at E m o r y U n i v e r s i t y i n A t l a n t a w h o w a s n ’ t involved in the study.“Insurance companies and p h y s i c i a n - o w n e d ambulatory centers are driving patients to more outpatient surgeries,” Refai said by email. “But without proper selection, patients may be at increased risk with poorer outcomes.”

USDA's Agricultural Research Service Pays It Forward with Research Achievements

With the year drawing to a close, the U.S. Department of a g r i c u l t u r e ' s ( U S D A ) A g r i c u l t u r a l R e s e a r c h Service (ARS) is preparing for 2018 with renewed vigor, building on its scientific and t e c h n o l o g i c a l a c c o m p l i s h m e n t s t o date—from a food nutrient database that consumers can access with the click of a mouse to a natural tire rubber made from guayule, a small flowering shrub native to the southwestern United States.As the USDA's chief in-house scientific research agency, ARS employs 2,000 scientists at 90-plus research locations across the country and overseas. Through research, technology transfer and information sharing, the agency's efforts address agricultural issues of high national importance. These i n c l u d e f o o d s a f e t y , c o n s u m e r n u t r i t i o n , environmental stewardship, rural development, natural resource conservation, and competiveness in global markets."The three technologies we're highlighting today represent the many achievements and advances our scientists have made towards realizing these national priorities," said ARS Administrator Dr. Chavonda J a c o b s -Yo u n g . technologies exemplify the spirit of scientific excellence and innovation that will continue into the New Year and beyond," she added.E d i b l e f i l m s a n d coatings—Oxygen exposure is a major cause of food spoilage. In 2017, ARS researchers found that wraps made from milk proteins like casein were up to 500 times better at keeping oxygen out of food packaging than traditional petroleum-based wraps. Scientists at ARS's Dairy Functional Foods Research Unit in Wyndmoor, Pennsylvania, developed the casein films as a way to create new, value-added markets for U.S. dairy products. These films offer a renewable and biodegradable alternative for food wraps derived from petroleum, a fossil fuel with many competing end-uses and finite availability. The r e s e a r c h e r s ' l a t e s t refinements include making the films more water- and fat-resistant so they can be applied directly to cheese sticks, baked goods and candies.U S D A B r a n d e d Fo o d Products Database—ARS researchers in 2017 added i n f o r m a t i o n o n n e a r l y 1 4 0 , 0 0 0 f o o d i t e m s t o theBranded Food Products Database, which is managed

by scientists at the ARS Nutrient Data Laboratory in Beltsville, Maryland. This database makes it easier to find nutrients contained in manufactured or brand-name foods sold in supermarkets throughout the United States. The database now contains information on 215,000 food items that can be searched by brand name instead of a g e n e r i c d e s c r i p t i o n , providing users with a more complete picture of the variability in products and the nutrients they contain. The result of a public-private partnership, the database has generated more than 22 million page views by 1.6 million users since coming online October 1, 2016. The d a t a b a s e a l s o h e l p s r e s e a r c h e r s i n t h e i r investigations, including connecting changes in the food supply to public-health outcomes.

"Home-grown" rubber—Tires made of natural guayule rubber earned high marks in a study that concluded in 2017, ra is ing prospects for a domestic source of rubber. An ARS-led team of scientists developed an improved v a r i e t y o f g u a y u l e , a southwestern shrub that has been studied for nearly 150 years as a promising source of rubber and as a biofuel

f e e d s t o c k . U n t i l n o w, however, th is drought -tolerant plant lacked the perquisite traits necessary for large-sca le cul t ivat ion, including disease resistance as well as increased rubber and biomass production. The U.S. tire industry relies on 100 percent imported natural rubber, which makes up 80 p e r c e n t o f b o t h h i g h -performance and passenger

(mileage) tires.T h e s e a c h i e v e m e n t s illustrate just some of the many ways in which ARS research impacts Americans a c r o s s t h e b o a r d - f r o m producers and processors to consumers. Agriculture (USDA) Farm S e r v i c e A g e n c y ( F S A ) provides vital farm safety-net assistance to agricultural producers across America.“We’ve seen recent challenges i n f a r m i n c o m e a n d commodity prices,” said Dr. Robert Johansson, Acting Deputy Under Secretary for the Farm Production and Conservation mission area. “The ‘safety net’ provided in the 2014 Farm Bill has helped p r o d u c e r s w i t h s t a n d economic losses as well as losses resulting from natural disasters. Loans for operating expenses, farm purchases and other purposes help current producers stay in business and allow a new generation of farmers and ranchers get their start.”Agriculture demands working c a p i t a l . A c c o r d i n g t o Johansson, FSA provided credit, either directly or g u a r a n t e e d t h r o u g h commerc ia l l enders , t o 120,000 family farmers across the country.In fiscal year 2017, USDA Farm Loan Programs pumped $6 billion in support to a diverse group of producers across America. That was the second highest total in FSA history. Over $2.5 billion of that total was direct and

guaranteed operating loans, and another $3.5 billion was allocated for direct and guaranteed farm ownership l o a n s . T h i s a d d i t i o n a l financing enabled farmers and ranchers across the country to access capital to start their operations, or to expand their existing operations. The new lending continued the recent growth in FSA’s farm loan portfolio.FSA highlights from the year include:Agriculture Risk Coverage and Price Loss Coverage (ARC/PLC) and Conservation Reserve Program (CRP)U S D A i s i s s u i n g approximately $8 billion in payments under the ARC and PLC programs to agricultural producers who suffered market downturns in 2016.In 2017, FSA distributed $1.6 billion in CRP payments to over 375,000 Americans for

doing their part in improving water quality, reducing soil e r o s i o n a n d i n c r e a s i n g wildlife habitat.Disaster AssistanceIn response to Hurricanes Harvey, Irma and Maria, USDA announced special procedures to assist producers in states and territories who lost crops or livestock or had other damage to their farms or ranches. Also, because of the s e v e r e a n d w i d e s p r e a d d a m a g e c a u s e d b y t h e hurricanes, USDA provided

flexibility to assist farm loan borrowers. FSA dispatched additional staff to the affected areas and, in response to a request for assistance, rolled o u t a s p e c i a l p r o g r a m providing vouchers to dairy herd owners in Puerto Rico who used the assistance to purchase feed.USDA also provided extensive assistance for a variety of other disasters throughout the country, including drought in the northern high plains, wildfires in the west and central plains, floods,

tornados, freezes and other storms. For example, in July, USDA authorized the use of additional CRP lands for emergency grazing and haying in and around portions of Montana, North Dakota and South Dakota affected by severe drought. USDA also added the ability for farmers and ranchers in those areas to hay and graze CRP wetland and buffer practices. This followed a previous action in April and June to assist the area and provided livestock producers with an additional feed source. In October, FSA teamed with other USDA agencies to provide assistance to wildfire-damaged areas of northern California, including loans and other disaster assistance programs.New FarmersI n Au g u s t , A g r i c u l t u r e Secretary Sonny Perdue signed a Memorandum of

WHO IS YOUR VICTORY?I caught myself thinking about my son’s football season the other day. I can be forgiven. He’s a senior after all, and we a r e w h i z z i n g p a s t milestones and memories a t an a larming c l ip . R e m e m b e r i n g t h i s s e a s o n ; l o o k i n g a t amazing action shots c a p t u r e d b y photographers with skill that far exceeds my cell phone efforts; and smiling as I remember particular moments of victory or success.

T h e y d i d n ’ t w i n a championship; didn’t win a district title; didn’t even have a winning season – based on their win/loss record anyway. But after four years with three different head coaches, things were poised to head further south than they were last year. But that’s not what happened. The team doubled their wins, which is nice. But what made the season worth remembering was the attitude of dedicated players (especially the jaded seniors), crafted by invested coaches. Some people may view this as u n r e m a r k a b l e o r unworthy of note. But

when I caught myself smiling as I reminisced about my son’s losing season, I realized that their victory hinged on more than the light bulbs o f a h i g h s c h o o l scoreboard.Life is a hard place. Joys are sprinkled in with enough frequency to keep us going. But anyone who has buried a spouse or a child, watched their home burn to the ground, been overwhelmed with financial desperation or dealt with any number of t h i n g s w e h u m a n s encounter on our journey – they know that life can deal a haymaker with no warning.

Driving past the OKC Bombing Memorial the other day helped me put my own small situation into perspective. Helping an elderly parent get the c a r e t h e y n e e d a n d deserve proves to be a bigger challenge than it should be. Corkscrew requirements that limit payments allowed or type of care given are the i m a g i n i n g s o f a bureaucracy that seems to have lost sight of finding the best ways to care for individuals. So as I sat at a stoplight, furious that our hands were being tied in finding the best care for my dad, I glanced at the fence to my right. Teddy bears and flowers were crammed into the chain link fence. Among them was an 8x10 photo of a

slightly older man. A man who would be an elderly m a n t o d a y h a d h e survived that bombing. In that moment, I knew that his family would be overjoyed to trade places with me and be happy to deal with governmental red tape if it meant having their father back on earth with them. The victory I can claim is knowing I have the privilege of fighting the good fight on behalf of a father who sacrificed mightily for

me, our family and our country.We are starting another new year. My hope is that we are all able to find the little victories that are s o m e t i m e s h i d d e n underneath situations that stir our anger and our discontent. How better for us to remember that God claimed victory for all of us and we celebrated that fact last week. Now it’s up to us to remember it the whole year.

when subjected to the magnetic fields and radio waves that allow magnetic resonance imaging, or MRI, to see into the body.The new study in The New England Journal of Medicine “confirms that pretty much a n y b o d y w h o h a s a pacemaker or implanted defibrillator can, with very few restrictions, safely get an MRI scan if they need it,” as long as the devices are properly adjusted before the scan and safeguards are in place, coauthor Dr. Henry Halper in , professor o f medicine, radiology and biomedical engineering at Johns Hopkins University in Baltimore, told Reuters Heal th in a te lephone interview.To prevent problems, the researchers reprogrammed the devices to adopt a standard heart rhythm for people whose hearts won’t beat on their own and disabled functions that might cause the pacemaker to fire improperly if the MRI produced erratic signals in the heart. After the MRI, the devices were returned to their original settings.O n l y n e w e r d e v i c e s d e s i g n a t e d a s “ M R I -conditional” have been considered safe by the U.S. F o o d a n d D r u g Administration. The vast majority in use - about 6 million worldwide - do not have that designation. A 2005 analysis estimated that at least half of those patients will someday need an MRI.T h e U . S . C e n t e r s f o r Medicare and Medicaid Services (CMS), which administers the Medicare insurance program for the elderly and disabled, will only pay for MRI scans in p a t i e n t s w i t h “ M R I -c o n d i t i o n a l ” d e v i c e s , c o a u t h o r D r . S a m a n Nazarian, of the University of Pennsylvania Perelman School of Medicine, told Reuters Health in an email.Given the results of the new study and an earlier one published in February, which also showed the older devices to be safe, “it’s hard to understand the position” of the CMS, said Nazarian, a cardiac electrophysiologist.In that February study, a separate team of researchers r e p o r t e d i n T h e N e w England Journal that no serious problems were seen in 1,318 patients with a pacemaker or implantable defibrillator who received an MRI. But in those cases, t h e c h e s t a r e a w a s n’ t scanned by the MRI.In the new tests by the Nazarian-Halperin team, about 200 of the 1,509 pacemaker and defibrillator patients had their chest

scanned with a 1.5 Tesla MRI. Neither the chest scans n o r t h e s c a n s d o n e elsewhere in the body p r o d u c e d s i g n i f i c a n t problems, said Dr. Halperin.Nine of the devices were d i s r u p t e d b u t r e s e t themselves to a backup mode. In all but one case, the effect was temporary. There was one instance where the device had to be replaced, but it had a low battery and could not be proper ly reprogrammed.Thus, a nearly-dead battery is probably one of the few reasons not to have an MRI, D r . H a l p e r i n s a i d . “Pacemakers do funny things when the battery is low.”In five instances, doctors halted the MRI exam. In one case, the heart rate fell to under 40 beats per minute. In another, the patient’s heartbeat became too rapid. In the remaining three, the doctors decided that an MRI probably wouldn’t produce a useful image anyway.D r . H a l p e r i n s a i d p a c e m a k e r s a n d defibrillators cleared by the FDA since the 2000s are

much better protected from the magnetic and radio waves of an MRI. “And we program them to avoid most any problems that might happen.”Dr. Nazarian said he would

advise patients that even if they have a device that’s not specifically rated for MRI safety, “Many centers across the U.S. are capable of performing safe imaging despite your device.”

Compared to last week: Steer and heifer calves under 500 lbs not fully tested, comparable sales mostly steady. Steer calves over 500lbs 3.00-5.00 lower, heifer calves over 500 lbs 4.00-6.00 lower. Demand was mostly moderate. Overall quality plain to average. Buyers showed very little demand for short-weaned and fleshy calves. Heifers made up 44 percent and cattle over 600 lbs 22 percent of supply.

springIt is not too soon to begin to prepare for the spring calving season. Locating, obtaining, and storing

several doses of colostrum or colostrum replacer will come in handy before the first heifers start to go into labor. Calves born after a difficult birth are at a high risk of failing to receive adequate colostrum by natural suckling because of greatly decreased colostrum intake. Calves that are born to a prolonged stage II of p a r t u r i t i o n ( d e l i v e r y through the pelvic canal) very often suffer from severe r e s p i r a t o r y a c i d o s i s . Acidotic calves are less e f f i c i en t a t absorb ing colostral immunoglobulins even if artif icially fed co los t rum. The on ly disease protection baby calves will receive is via the p a s s i v e t r a n s f e r o f a n t i b o d i e s (immunoglobulins) from the colostrum that they

ingest. Therefore effort should be made to provide weak newborn calves with the best source of colostrum available via bottle suckling or tube feeding.Natural colostrum is still considered the best source of the immunoglobulins for disease protection for the calf. If there is still a dairy in your area, the opportunity may exist to obtain some natural colostrum from newly freshened dairy cows. Avoid obtaining colostrum from dairies that are known to have had an i n c i d e n c e o f “ J o h n e s Disease”. Fresh colostrum can be stored in 1 quart doses by putting that much (1 quart) in a gallon-size Ziploc bag. Lay the bags flat to freeze in the freezer. When it is time to thaw the colostrum, it

will be easier and quicker to thaw, compared to 2 quarts or more in a big frozen chunk. The amount of immunoglobulin ingested is also a major determinant of final blood immunoglobulin concentration. A practical "rule-of-thumb" is to feed 5 to 6% of the calf's body weight within the first 6 hours and repeat the feeding when the calf is about 12 hours old. For an 80 pound calf, this will equate to approximately 2 quarts of colostrum per feeding. Consequently, if the calf is quite large (about 100 pounds), then the amount of colostrum will need to be increased accordingly to 2 ½ quarts per feeding.If there is no source of natural colostrum available, purchase a few doses of a commercial colostrum

“replacer”. Colostrum replacers wil l contain greater than 100 grams of immunoglobulin per dose. Make certain to read the label before purchasing. Colostrum replacers may

seem expensive, but the value of a l ive calf at weaning strongly suggests that every effort to keep all of them alive is worth the investment.

Chesapeake Offers $30 million Settlement in Pennsylvania Royalty LawsuitsA $30 million settlement offer is being made by Oklahoma City-based Chesapeake Energy to landowners in federal lawsuits in Pennsylvania.The lawsuits were filed in a fight over gas-royalty payments. Despite the settlement, Pennsylvania Attorney General Josh Shaprio says it really depends on bringing an end to a separate lawsuit against the Oklahoma company according to The

Philadelphia Inquirer.Lawyers for Chesapeake told a federal judge in Scranton this month they had reached a settlement with several of the class-action suits. The deal would provide payments to all 14,000 Chesapeake gas l ea seho lde r s . In addition, the agreement w o u l d a l l o w t h e landowners to “reset” their leases to clarify the terms on which they are paid royalties.

However, there is an “if” i n t h e d e a l a n d Chesapeake won’t move f o r w a r d w i t h t h e settlements until a lawsuit filed against the company by former Pennsylvania A t t o r n e y G e n e r a l Kathleen G. Kane in 2015 is resolved. She sued claiming Chesapeake had cheated landowners by making big deductions from their royalties.T h e P e n n s y l v a n i a Attorney General’s office repor ted ly to ld U .S . District Judge Malachy E. Mannion it is still far apart with Chesapeake in the negotiations.“At this point in time, our major divide is on money,” said Joseph S. negotiations.“We all know, I will tell you, that these cases are going to resolve, and they aren’t going to go to trial,” said the judge. “It’s going t o b e a q u e s t i o n o f whether or not we spend significant amounts of m o n e y a n d t i m e , o r whether we kind of get on the real-life practical train on both sides and find a way to get to the end.”At the heart of the lawsuits by the landowners lay the price of deducting post-production costs from the royalties. For the life of me, I can’t understand why people drive without their headlights on in the fog. I saw many cars doing this. It should be a law to drive with your lights on in the fog.

Just got my Social Security statement advising me about an increase in benefits. I’m

getting an increase of $2.2 dollars. Two dollars; 4 half-dol lars; 8 quarters; 200 pennies. Social Security is a joke.

We have the worst drivers in the worst driving state in the nation in wet weather. Those who use their brain already knew that. The most important factor — tailgating. Just like in dry weather, besides careless driving, tai lgat ing is the greatest cause of wrecks. Every day you see people driving the highway only one or two car lengths behind other vehicles. I am asking for all store owners and managers to take a second and evaluate your accessibi l i ty opt ions for people with true disabilities. One of our family members was recently stricken with a long-term illness that requires the use of an electric cart to shop. Why would big (stores) not offer electric carts? Why would they not set up the display racks so the carts can maneuver around them? Also, why would the ones that offer a cart not have a way to guage the battery life so a person is not stranded in the back of the

store? Please look at what you offer for people with true disabilit ies because they would love to spend their money i n you r s to re i f accessibility was an option. Thank you.
