s we continue to advance the ways we provide exceptional care to our patients and their families, Centegra Health System keeps the needs of our community at the center of all we

do. We develop new services and expand existing programs to stay ahead of community need and to adapt to the changing landscape of the health care world.

In 2015 we passed the 75 percent completion mark at Centegra Hospital–Huntley. With less than a year until we welcome our first patients, Centegra Associates and Physicians continue their efforts to ensure our newest facility meets the highest standards for clinical quality and service.

Centegra continues to integrate services so community members receive seamless care. We do this at the Centegra Hip & Knee Replacement Center, where patients benefit from collaboration between orthopedic surgeons, nursing and rehabilitation teams that walk patients through every step of their journeys.

Last year, Centegra Physician Care welcomed new primary care and specialty providers to improve the community’s access to expert care. New pharmacies at Centegra Hospital–McHenry and Centegra Hospital–Woodstock make it easier for patients to get the medications and supplies they need before they leave the hospital. A new inpatient Behavioral Health unit at Centegra Hospital–Woodstock gives patients increased access to additional hospital services so all their needs can be addressed.

The pages of this report are filled with the highlights of 2015 and with the names and faces of individuals who are making a difference in our community. Whether they give of their time, their

talents or they make gifts to the Centegra Health System Foundation, each of these people helps us provide our patients the care they deserve.

2015 AccomplishmentsCentegra Hip & Knee Replacement CenterThe Centegra Hip & Knee Replacement Center launched at Centegra Hospital–McHenry in July 2014. The center is unique, with joint navigators guiding patients through every step of the joint replacement experience. Patients receive surgery from highly skilled and experienced surgeons. They recover in private rooms on a unit dedicated to total hip and knee joint replacement patients. The new center has experienced increased volumes and outstanding patient satisfaction scores.

Obstetrics EnhancementIn September 2014, obstetrics services were centralized to Centegra Hospital–McHenry. To provide the safest care for patients, in October, obstetrics hospitalists started providing care at Centegra Hospital–McHenry. This service provides 24/7 access to physicians who have specialized training in obstetrics and gynecology. As a result of the OB hospitalist service, two rooms in the Emergency Department are dedicated to obstetrics care so women with obstetrics emergencies can receive assessment from emergency department physicians and OB hospitalists without having to be admitted to the Family Birth Center.

Centegra Care ConnectionsCentegra Care Connections retail pharmacies opened at both Centegra Hospital–McHenry and Centegra Hospital–Woodstock in early 2015. The pharmacies help ensure patients have the medications and supplies they need before returning home.

MetroChicago Health Information Exchange (HIE)Centegra, a founding member of the MetroChicago Health Information Exchange (HIE), joined 33 hospitals and health care organizations, including seven other major Chicago-area health systems, in secure, electronic sharing of patient information to improve the quality, safety and cost of health care in the region. In February 2015, a patient at Centegra Hospital–Woodstock was the millionth person to have a secure patient record become part of the MetroChicago Health Information Exchange.

Centegra Cardiovascular & Thoracic Center There have been more than 2,300 visits to the Centegra Cardiovascular & Thoracic Center since it opened in October 2014. The center includes Centegra Physician Care Cardiology, Cardiac Surgery Associates, S.C. and a heart failure clinic. Over 580 of these visits were for heart failure management. Centegra’s new partnership with Loyola University Health System provides access to heart failure specialists.

Centegra, Huntley School District Announce PartnershipCentegra Health System and Huntley Community School District 158 Board announced they will partner to bring a new High School Residency Program and enhanced sports care services to the district’s students. The High School Residency Program will educate students who are interested in health care careers. The program will be the first in the state of Illinois and one of just a few in the country. Centegra has also established an exclusive agreement with the district to provide sports care services.

Grant Received for Advance Planning ConversationsCentegra Health System was awarded the Preferencing Aligned Communication and Treatment (PACT) grant, a collaboration between Northwestern University, Telligen and Blue Cross Blue Shield of Illinois. This collaborative initiative led by the Care Coordination and Clinical Effectiveness teams will run from FY15 through FY18 and support facilitation of standardized patient-centered advance planning conversations for patients facing serious illness.

Rosalind Franklin University PartnershipCentegra Health System and the Chicago Medical School at Rosalind Franklin University of Medicine and Science will partner to offer an Internal Medicine Residency Program that will begin in July 2016. The three-year Internal Medicine Residency Program will begin with eight residents, then add eight additional residents in 2017 and eight more again in 2018.

hen an oncologist referred Janet Luken to the Centegra Survivorship Training and Rehabilitation (STAR) Program

for rehabilitation services, her immediate reaction was to question the recommendation.

“I don’t think I need that,” said Luken, a Crystal Lake resident and colon cancer survivor, to her husband. Months later, Luken admitted she could not have been more wrong.

“Immediately following my six months of chemotherapy, which was prescribed after colon cancer spread to four of my lymph nodes, I thought I’d felt so much better than I had during my treatment or even before when I didn’t realize I was ill,” Luken said. “I guess I had forgotten what ‘normal’ felt like.”

The Centegra STAR Program provides patients coordinated cancer rehabilitation by a team of certified STAR clinicians and providers. The specially trained team includes physicians, nurses, physical therapists, occupational therapists, speech-language pathologists, registered dietitians and mental health professionals. Each team member provides expertise, guidance, training and support to help cancer survivors thrive.

In the end, Luken took her doctor’s advice and scheduled an appointment with physical therapist Maria “Jingle” Ariston. “When I met with Jingle, she was testing my arm and leg strength by putting pressure on them,” Luken said. “I couldn’t even lift my leg. I was shocked. I didn’t realize how weak I was. I would have gone through life with no muscle tone had I not gone through the STAR Program. Jingle helped me build up my strength and endurance.

“With the STAR program and Jingle’s help, I was able to get stronger, be healthy and be myself again,” Luken said.

Centegra Physician Care expands employed practiceOn October 1, 2015, Centegra Health System welcomed McHenry County Orthopaedics to Centegra Physician Care. This significant addition to the employed physician group demonstrated Centegra’s continued dedication to partnering with exceptional providers. The orthopedic surgeons and Associates of CPC-MCO will continue to provide the same services their patients have come to appreciate at their offices in Crystal Lake and Huntley and the team will support Centegra’s growth to meet the needs of the community.

n the year since it opened, the Centegra Hip & Knee Replacement Center has become the region’s trusted destination for patients like Mike

Boho. Boho chose Centegra for his knee replacement surgery and he said it seemed like he was the team’s top priority during his stay at the center.

“Centegra’s got their act together,” Boho said. “I think they are very concerned about the patient. They are very concerned about results and with how you feel about your situation.”

The orthopedic surgeons and Associates at the Centegra Hip & Knee Replacement Center understand that their patients are seeking more than pain-free joints – they’re looking forward to enjoying life again.

To make sure patients have the best-possible experiences and outcomes, they are assigned a joint navigator who helps them prepare for surgery. A special class teaches patients what will happen during and after surgery so they know what to expect. This special focus on preparation reduces their anxiety and sets them up for a successful recovery at home.

From the moment the new unit opened, it was clear that patients enjoyed the personalized attention and expert care they received.

“As far as Centegra’s facilities, the one aspect I liked the most was the private room,” Boho said. “It gave me a feeling of comfort. It was like being at a hotel room. Nothing was too much trouble. Anything I needed, they were right there.”

The spacious patient rooms provide space for in-room physical therapy, which patients receive twice a day after joint replacement. Centegra’s physical and occupational therapy teams help patients get on their feet and start down the path toward recovery so they can return home to their families and favorite activities.

From start to finish, the Centegra Hip & Knee Replacement Center walks patients through the joint replacement experience. It’s another way Centegra ensures community members have access to the highest level of care close to their homes.

entegra Health System’s distinctive health campus in Huntley came alive in 2015 when hundreds of tradesmen arrived daily to build the community’s new hospital. As walls were

raised and systems installed, supporters had visible proof that Centegra Hospital–Huntley would soon be open to serve patients and families.

Centegra executives, leaders, Physicians and Associates worked alongside contractors to ensure the hospital was built to meet patient needs. Doctors and nurses visited regularly to share their thoughts and enhance the design of patient rooms and work areas. Their top priority was to create a private, healing environment that would provide the exceptional experience community members deserve.

In early 2015 the Illinois Health Facilities and Services Review Board unanimously approved Centegra’s certificate of need application to add cardiac catheterization services at Centegra Hospital–Huntley. The approval meant the people of northern Kane County and southern McHenry County will have access to advanced emergency and routine cardiology services the day the new hospital opens.

The health system’s medical staff grew with the addition of new primary care and specialty providers. Centegra Physician Care welcomed new pediatricians, family practitioners, internists and cardiologists to care for the growing community and in anticipation of the new hospital’s opening.

As Centegra Hospital–Huntley passed the 75 percent completion mark, the Centegra Health System Foundation continued to focus on funding for the new hospital. Individual investors and corporate partners expressed their support of Centegra by sharing generous gifts to help enhance the services that will be available to the community.

Centegra Hospital–Huntley became the symbol of the region’s growth and its future. For many community members, the hospital generated excitement that world-class health care would soon be just minutes from home. To everyone in southern McHenry County and northern

Kane County, it became clear that Centegra Health System was preparing to serve patients and families for generations to come.

Although it was already the largest employer in McHenry County, Centegra Health System planned to recruit and welcome about 500 additional employees in 2016 and prepared for more growth when the hospital opens. The health system announced it will partner with Huntley Community School District 158 to develop local health care talent by creating a High School Residency Program. An Internal Medicine Residency Program in partnership with the Chicago Medical School at Rosalind Franklin University of Medicine and Science was announced and will bring physicians in training to the region beginning in 2016.

advanced heart procedures save life

hen Beth-Sophia Thompson listened to an interventional cardiologist speak

at the Centegra Health Strong Woman Event in February 2015, she never dreamed she’d meet him in person that evening when he performed a procedure to save her life.

WCentegra’s cardiology team saved the life of a woman suffering a heart attack.

Thompson was one of more than 400 women who attended the annual event in Crystal Lake to learn about heart health and connect with Centegra’s many services. There, she heard Douglas Tomasian, MD, director of cardiology at Centegra Health System, speak about heart attacks in women. As Tomasian described common symptoms, Thompson realized she was experiencing many of them at the same time.

During her drive home, Thompson knew she needed to seek help.

“God told me ‘you turn yourself around and go to the ER,’ so I did.”

When Thompson arrived at Centegra Hospital–Woodstock’s Emergency Department, tests revealed she was having a heart attack. She was immediately transferred to Centegra Hospital–McHenry, where Tomasian performed an emergency cardiac catheterization to reopen an artery that was 100 percent blocked. Shortly after, Tomasian performed a second procedure to address a 90 percent blockage in another artery. After the procedures, Thompson stayed in the hospital to receive follow-up care.

“The intensive care unit nurses were very good,” she said. “They were knowledgeable on what to expect and how to treat a patient like me who had a heart attack. They were very caring and attentive to my needs.”

Thompson and her husband, who are parents to young children, are convinced the doctors and nurses at Centegra Hospital–Woodstock and Centegra Hospital–McHenry saved her life.

“Dr. Tomasian told me maybe if I’d waited a week or two or maybe just a few days I might have not been sitting here today, so I really want to say thank you,” she said. “It was just a beautiful experience when I came here, from admission to discharge. I had a wonderful experience.”

3-D Mammography catches cancer early

ach year, Jeanine Katzel, 67, faithfully schedules and receives a mammogram

at Centegra Health System. Her health is important to her, so between appointments, she performs regular breast self-exams and sees her doctor for a clinical breast exam.

“Getting a mammogram isn’t my favorite thing to do, but I know that it’s important,” Katzel said. “So I grit my teeth and go. When my last mammogram was irregular, I was glad that I’d gone in.”

Donations Save LivesPhilanthropic investors of the Centegra Health System Foundation funded the purchase of two new 3-D mammography units to ensure the most advanced technology is available for the community. These community members, who are dedicated to women’s health in the region, have ensured that women can now receive this state-of-the-art imaging at Centegra Gavers Breast Center, Centegra Hospital–McHenry and Centegra Hospital–Woodstock.

Ideal for women with dense breasts, 3-D mammography technology captures multiple images (slices) of the breast at different angles, allowing radiologists to examine each one-millimeter slice at a time, providing a 3-D reconstruction of the breast.

“We are finding greater accuracy in detecting breast cancers when compared with standard 2-D mammography,” said Elissa Brebach, MD, Centegra’s section chief of mammography. “3-D mammography benefits women because it allows us to examine breast tissue in greater detail than with standard mammography, enabling us to detect small cancers even earlier.”

Brebach said more than 7,000 community members have received 3-D mammograms at Centegra Gavers Breast Center since it was installed there in May 2013.

“Not only are we finding greater accuracy but we’re also finding a decrease in patient anxiety, expense and inconvenience since our patients are less likely asked to return for follow-up screenings,” Brebach said.

Center. The scan used X-ray technology to capture images of each layer of her breast tissue. Special software merged those images, creating three-dimensional renderings of her breasts. When Katzel’s doctors reviewed those images, they discovered a small lump. Additional tests revealed the Cary resident had cancer – a one-centimeter tumor.

“It was so small I couldn’t feel it,” Katzel said. “There were no lumps or changes in my breast tissue. I probably wouldn’t have discovered the cancer on my own for a long time. But the 3-D mammogram found it early. This technology is revolutionary.”

Katzel underwent presurgery chemotherapy to shrink her tumor. That was followed by a lumpectomy in April. By the time she had surgery, Katzel’s tumor was considerably smaller. She is now cancer free and following a postsurgical radiation therapy regimen to help prevent the cancer from returning.

“It felt good knowing that the technology to find and treat this cancer is available here, close to my home,” Katzel said. “The medical team I worked with was helpful, caring and reassuring. I even had the same oncologist that my father used when he had cancer. It was a positive experience, and I’m optimistic about my future.”

improves patient access to the medications, services and equipment they need to safely recover at home. Centegra Care Connections extends the health system’s commitment of caring beyond the walls of the hospital and provides convenience and peace of mind to patients and families.

Outpatient pharmacies at both hospitals were opened to make it easy for patients and Associates to get medications and supplies when they were on campus for doctor’s appointments or hospital visits. Another major component of the program was the introduction of a new role at Centegra called “transition specialist.” These new Associates provide one-on-one interaction with patients and family members to coordinate medications, supplies and other health care needs and services. They deliver medications to the bedside to ensure patients leave the hospital prepared for a successful recovery. If a patient needs medication counseling, the transition specialist arranges for a pharmacist to visit with him or her before hospital discharge. The team also provides curbside delivery of medical equipment so patients do not have to shop for supplies when they leave the hospital.

Since the program began, transition specialists have transformed the lives of their patients by providing support and paving the path to successful recoveries at home.

“This week I helped an elderly man who was being discharged,” a transition care specialist shared. “Both the patient and his wife were very concerned with the cost of his medications. I was able to get them the prices of his six medications through his Medicare Part D, and was also able to inform the nurse that one of them, which was expensive, wasn’t covered. She contacted the doctor and got it changed to a preferred medication. The couple was very thankful that we were not only able

to fill their medications here, but that we were able to catch that a medication wasn’t covered and have the problem rectified before they left.”

Another transition specialist shared how she supported a patient who was concerned about paying full price for prescriptions at a local pharmacy.

“I noticed that she had Medicare Part B, but she claimed that she didn’t have any prescription coverage. I brought her prescriptions downstairs for prices, but asked the technicians if they would run a Medicare Part D check for the patient, just in case she had coverage. Sure enough, the patient had prescription coverage that she was unaware of and had been paying for it without using it. She was able to get her three prescriptions for no charge. entegra Health System improved the region’s access to Behavioral Health

services when it relocated inpatient care from South Street in Woodstock to a newly constructed unit at Centegra Hospital–Woodstock in June 2015. The new unit was funded in part by community members who made generous donations to the Centegra Health System Foundation.

The move allowed Centegra to expand inpatient Behavioral Health from 24 to 34 beds and to add additional private rooms for patients. By relocating to the Doty Road hospital, Centegra Hospital–Woodstock is now better able to serve patients with enhanced facilities and improved

medical services. If patients need access to acute care medicine, consultations and services, they are now all available on one campus.

Centegra Hospital–Woodstock also opened a Psychiatric Emergency Service at Centegra Hospital–Woodstock to support patients who need crisis assessment, intervention or evaluation for inpatient psychiatric hospitalization. The service also provides mental health and substance abuse treatment referral information to patients.

“As the region’s leading provider of Behavioral Health care, Centegra understands the needs of community members who turn to us for intensive psychiatric care,” said Sheila Senn, senior vice president and chief operating officer, Centegra Hospitals-Woodstock and Huntley. “Our new unit improves the level of service we provide our patients and increases their opportunities to receive care close to home.”

To make room for the Behavioral Health move at Centegra Hospital–Woodstock, the health system centralized obstetrics services to an expanded and renovated Family Birth Center at Centegra Hospital–McHenry. By adding 24-hour obstetrics hospitalist and neonatology services, Centegra enhanced the level of care available to all mothers and babies. As a result of these additions, Centegra Hospital–McHenry also began to offer an Obstetrics Emergency Department to women who arrive at the hospital with obstetrics emergencies.
