Form
A few key things to help you be
successful in processing the form.

Contract Review
Basic Required Information
• The basic required
information to
submit a CRAF is
shown in yellow
highlight.
– Department type will
prepopulate based
on the Department.
Contract
Number Lookup
To avoid double entry and
duplicate CRAFS, when a
Contract Number is entered
a check is performed to
ensure that another CRAF
does not have the same
Contract Number.
If a CRAF has already been
entered using the same
Contract Number, you will
be given the option to open
the CRAF that has already
been created.
New Approval Fields
• The approvals are no longer individual cells but are
located in the table as shown below. Dean and Dept.
Head name and email will be provided by the
requester.
– Please note that the form will give you the correct number of
rows in the table when Department Type, Contract Type,
Amount, and whether the contract is Grant or Non-Grant
Related. You will NOT be able to provide routing information
until all fields are provided.
Approving the CRAF
• Approvers no longer mark checkboxes
but click either the “Approve” or “Return”
button. The approvals are logged in the
table.
Download Attachments
Click on the Attachment name that is underlined to download and view
the attachment.
Save Changes and Submit
Save Changes will send you a link to access the form to finish. Submit
sends the form on for processing.
Did I properly submit the form?
Common Errors
A message similar to the one above indicates that someone else is currently viewing
the form. Until they exit the form, you will be unable to make changes. This message
also appears if someone exited the form incorrectly. The form will generally “free”
itself in 30 minutes.
Common Error Messages
Please make sure to only open the form once and exit the
form correctly. Links provided in email notifications need
only to be clicked once to open the form instance.
Correct Ways to Close Form
Common Issue
• The request is not in my IRIS inbox.
Links to the requests are sent through email.
The email will be from UTMartin eForms
([email protected]). Emails are
sometimes in the “Junk” folder.
Common Errors
My form will not print or the print is very small.
– PerfectForms will only print correctly by using the Print
button located on the form.
Common ErrorsI am using the Print button but the form will
still not print.
– If you have pop-ups blocked on your computer, the print
