you will NOT be permitted to visit at this time Contact HealthLine 811 and

self-isolate until you receive further direction

If you answer YES to question 3 you will NOT be permitted to visit until 14 days:

after your return to Canada after your last contact with confirmed or probable case

You must self-monitor and, if you develop symptoms, isolate and call HealthLine 811 immediately

If you answer YES to question 4 or 5 you MAY visit if you don’t have symptoms but

need to wear a mask

Please use the provided mask for your essential visit to help keep everyone safe.*

Please be advised that as of June 5, a COVID-19 test will be offered to all patients.

*If patients/clients or family member/support person are unable to tolerate wearing a procedure mask (examples include but are not limited to: allergies, claustrophobia, children not keeping a mask on), encourage the following alternatives:

Personal mask – they can continue to wear their own provided it covers both nose and mouth

Maintain physical distance of a minimum of two metres at all times Perform hand hygiene regularly
