Scope This corporate standard applies to any work location where work involving the erection, dismantling or use of scaffolding is required within a SCL operated and/or maintained site or any situation in which an SCL employee or contractor is required to undertake specific work tasks from, or involving, a scaffold.

Definitions Certified scaffolder: a person who holds a certificate of competency as a scaffolder. Competent person: a person who has through a combination of training, education and experience, acquired knowledge and skills enabling that person to perform correctly the specified scaffolding task. Fall arresting platform: a platform installed to arrest the fall of a person who falls from a building or other structure. Guardrail: a horizontal structural member to prevent persons from falling off any platform, walkway, stairway or landing. Mobile scaffold: an independent free-standing scaffold that is mounted on castors. Perimeter containment screening: a screen (i.e. made of mesh, timber, plywood or metal sheeting), which is designed to: stop objects falling on members of the public from a level of a building; or to redirect a falling object onto a catch platform. Prefabricated scaffold: a scaffold assembled from prefabricated components and manufactured so that the geometry of the scaffolding is predetermined. Prescribed lining: intact shade cloth, or another intact lining, that when tested, wet or dry, in accordance with method A in AS 2001.2.4, has a mean bursting pressure of at least 1000 kPa. Registrable plant design: plant design(s) required to be registered with the OH&S statutory authority for a particular state / territory. Scaffold: a temporary structure for supporting access or working platforms. Scafftag: a specific card holder and card, that, when fixed to a scaffold, identifies the status of the scaffold, details with respect to its use and inspection details. Refer to Scafftags – HB# 560077. Tie: a member or assembly of members used to tie / secure a scaffold to a supporting structure. Tube and coupler: a description of scaffolding that consists of steel tubing (tube) and joining or fixing components (couplers) that are fixed together to form a required scaffold design. Worker: SCL employee or controlled contractor. Working load limit: the maximum safe working load that may be applied to any component or system. Working platform: a platform that is intended to support persons, materials and equipment.

Station / Site Manager To monitor the implementation of the Scaffolding Corporate Standard and allocate responsibilities and resources to ensure site-specific practices / procedures are developed to satisfy the Corporate Standard. Scaffolders To ensure scaffolding is erected in accordance with statutory, Australian Standard and manufacturer requirements. To comply with specific SCL scaffolding work procedures / standards. Workers To comply at all times with the requirements specified within this corporate standard and any site-specific procedures.

Hazards In relation to activities involving the erection, modification, dismantling or use of scaffolding, a safe system has been implemented to control risks to health and safety arising from hazards such as, but not limited to: scaffold collapse; manual tasks; impact of mobile plant / traffic; electricity / induced voltage; hazardous areas; falls from heights; and falling objects.

Risk Assessment Scaffolding hazards are to be assessed and managed using the following where relevant: scaffold plans; inspections; work method statements for the relevant work activities.

Controls Planning 1. A scaffold plan is to be prepared and provided by the organisation (e.g. scaffolding company, Stanwell

Corporation) doing the scaffold work. Note: Where prefabricated scaffolds are erected strictly in accordance with manufacturer recommendations,

and used regularly on site, the manufacturer’s instructional erection diagrams and associated documentation may suffice in regards to scaffold plan requirements. A scaffold plan is not required in circumstances where the scaffold being erected is of a basic nature (e.g. not a hung or complex scaffold) and the potential fall distance from the scaffold is less than 2m (e.g. work benches).

2. All scaffolds are to be designed by a competent person. 3. The scaffold plan is to be available for inspection on site.

4. When designing scaffolding, consideration is to be given to the following types of issues:

the environment, such as the design, shape and location of the building or structure and safety of other workers and the public, including pedestrians and road users;

the scaffold systems being selected to ensure that they are the most adaptable to the contour of the building or structure;

means of access which includes the supply and transportation of scaffold components close to the work area to assist in ease of erection;

the availability for tying a scaffold to a structure and the methods which are most suitable, including the design of additional bracing within a scaffold where only minimal tying positions are available on existing structures;

the location and length of time the scaffold is to be erected; making allowances for anticipated climatic conditions such as wind and rain; the proximity of the scaffold to an environment that contains hazardous substances which could react

with the scaffold; if the area is a designated hazardous area; controls to prevent plant from being activated where scaffolds are constructed on, over or inside items of

plant; clearances between boilers and scaffolding to allow for expansion and contraction of the boiler; the purpose for which the scaffold is to be used, be it for access only, construction related activities,

demolition activities, support of specific materials such as blocks or plant items, specific electrical work, etc;

the expected weight to be placed on the scaffold and specific loading bays, if relevant; the capability of the structure to support a scaffold, for example the structural integrity of walls, floors,

parapets, awnings and the capability of the structure to withstand any additional loads imposed; the intended use and modification needs of the scaffold, including the alteration and/or removal of any

part of the scaffold; and any other issues described in relevant codes of practice or Australian Standards.

5. Information regarding the design of a scaffold is to be provided by the scaffold designer to the scaffold

erector and detailed in a scaffold plan, or documented information provided by the supplier. 6. The following information is to be detailed in the scaffold plan:

instructions for erecting, dismantling, transporting, storage and maintenance; guidance as to safe work practices to ensure stability of the erected scaffold; guidance as to where safety fittings are to be used in either a compression or tension manner. Examples

of this include where an access opening is made through scaffolding or in the construction of spur or hung scaffolding;

the duty classification of the scaffold including maximum platform capacity; the maximum number of working platforms; maximum height of the scaffold; and location of bracing and scaffold ties.

7. The scaffold erector is to erect the scaffold in accordance with relevant scaffolding Australian Standards and

the scaffold designer’s / supplier’s specifications, which have been detailed in the scaffold plan. 8. Scaffold plans are to be maintained in accordance with the following:

owned prefabricated scaffolds (mobiles, purpose built scaffolds, etc) – life of scaffold; designed tube and coupler scaffolds used regularly – as long as scaffold design is being used; and contractor supplied plans – life of outage/overhaul or duration of the construction period.

Controls 1. All scaffolding to be erected on-site is to be of the appropriate / suitable type for the particular activity for

which it is to be used. 2. All scaffolding to be erected on-site is to be in compliance with the scaffolding requirements detailed in

Attachment 1. 3. Scaffold components from different manufacturers or suppliers are not to be used together unless the

requirements for using components from different systems detailed in the QLD Scaffolding Code of Practice and AS/NZS4576 – Guidelines for Scaffolding are met.

4. Where scaffolding has been erected, altered or modified by a contractor, the contractor is to provide a

handover certificate. 5. The handover certificate is to:

a. be kept onsite until the scaffold has been dismantled; and b. meet the requirements detailed in Attachment 2: Handover Certificate Requirements.

6. Workers are only to undertake scaffolding activities for which they hold the appropriate licence / certificate

(e.g. basic scaffolder, intermediate scaffolder etc.). 7. To undertake the following activities associated with scaffolding from which a person or thing may fall more

than 4m or any tube and coupler scaffolding of any kind, the competent person must be a certified scaffolder: to erect, modify or dismantle; and to undertake specific inspections and alterations of scaffolding for the purposes of ensuring scaffold

compliance. Access 8. Scafftags are to be used to designate the status of scaffold, inspection and use information.

Note: Refer to Scafftags – HB# 560077. Falls and Falling Objects 9. During erection / dismantling / use / modification of scaffolding controls are to be implemented to prevent a

person being hit by a falling object. 10. During erection and dismantling of scaffolding, the person(s) erecting and dismantling must be prevented

from being able to fall to the ground or the next platform of scaffolding by means of: edge protection measures to prevent a fall; use of a fall-arrest harness system (only suitable for some situations, refer next point); or use of a prescribed and detailed work method (immediate installation of platform and edge protection,

and retention of full deck).

Note: This includes situations where scaffolds or platforms are erected and used on catwalks, near handrails or in other elevated locations where a fall height may be in excess of the scaffold or platform being erected or used, creating an increased height safety risk.

11. The only situations where personal fall arrest equipment may be used during erection and dismantling is:

where a person falling cannot hit an object prior to the fall being arrested, taking into consideration the lanyard length, anchorage height, etc (an example being the erection and dismantling of a hung scaffold);

where anchorage loadings (such as 15kN) are able to be confirmed for scaffolding anchorage points or structural anchorage points; and

where there is not a high likelihood of trip hazards being presented for users of the fall arrest equipment.

12. Where the installing of a fall arresting platform is being used as a control measure to catch a falling person,

the fall arresting platform must be located not lower than: where the slope of the work surface is less than, or equal to 26° – 1 metre below the work surface; or where the slope of the work surface is over 26° – 300mm below the work surface.

13. The platform of the fall arresting platform must:

provide an unobstructed landing area, for a falling person, at least 675mm wide for the length of the platform;

be able to withstand the impact of a fall onto the platform; have edge protection erected along the outer edge of the length, and along the edge of each end of the

platform; and where a gap between the inner edge of a platform and the building/structure exceeds 225mm, control

measures are to be implemented to ensure that a person cannot fall off the inner edge. 14. Where a prescribed and detailed erection / dismantling method is used for scaffolding such as perimeter and

birdcage scaffolding, the scaffolding is to be provided with a full width deck of planks every second lift, as a minimum. It should also be noted that a full set of planks at every second lift is based on the requirement that the scaffolder installs planks from under the deck, while standing on a minimum of two planks.

15. Materials, equipment, scaffold components or other items are not to be dropped from scaffold. Specific Scaffolding Hazards 16. During erection and dismantling of scaffolding, planning and precautions are to be taken to ensure scaffold

components cannot come into contact with nearby plant items, components, overhead electrical conductors, etc. That is, overhead clearance distances, plant protection and handling methods need to be considered as part of scaffold planning processes. Note: Refer to Electrical Safety – HB# 560790, for information regarding exclusion zones for overhead

powerlines. 17. Controls are to be put in place to prevent or minimise the risk associated with mobile plant or traffic operating

in the vicinity a scaffold. Refer to Traffic Management – HB# 562513 for further information. 18. Hazardous areas hazards are to be controlled as per Hazardous Areas – HB# 625195. 19. Lighting of approaches, access ways, walkways, working platforms and exits is to be provided where

required. Inspection, maintenance, servicing and repair 20. All scaffolding and associated components are to be kept in a safe and well-maintained condition. 21. Inspections are to be undertaken using the Scaffold Inspection Checklist Template or equivalent to assess

compliance with this standard. The intervals for inspection are to be allocated at the site level and appropriate for the frequency of scaffold use, types of scaffolds and types of activities undertaken from the scaffold. Corporate Standard compliance elements are marked as such on the Scaffold Inspection Checklist.

22. Regular inspections by a competent person are to be undertaken of all scaffolding onsite regardless of type

or size. Scaffolds and their supporting structures that are at a height of four (4) metres or more (or any tube and coupler scaffolds) are to be inspected by a certified scaffolder. The Scaffold Inspection Checklist Template may be used for this purpose.

23. Inspections are to be undertaken:

before the scaffold’s first use; immediately following any disturbance or major weather impact that might reasonably be expected to

affect the stability of the scaffold or supporting structure; following any repairs and modifications; and at intervals not exceeding thirty (30) days. Note: Intervals for inspection should be decided in consultation with the supplier and with consideration of

the manufacturer’s information, weather and site conditions, the type and size of scaffold and the risks associated with scaffold collapse.

24. For SCL owned scaffolding, full fitness for use inspections in accordance with AS 1576 and AS/NZS 4576

are to be undertaken on all components at not less than a 12 monthly basis. During this inspection, non-compliant components are to be identified, repaired or replaced as applicable.

25. The Scaffold Inspection Checklist Template has been provided as a guide and may be used by competent

SCL personnel and/or contractors during inspections to determine the following key factors: the scaffold structure is adequate; the supporting structure is adequate; working platforms are secured and protected; access and egress is sufficient; and the scaffold will enable the relevant work tasks to be performed adequately and safely. Note: Scaffolding contractors may have their own specific scaffold checklists by which inspection processes

are recorded. Where this is the case, SCL is to request and ensure that a copy of such inspection checklists are signed by the scaffolder and forwarded to SCL.

26. Specific scaffolding components and hardware (i.e. couplers, hoists, protective devices, load limiting

devices, wire ropes, scaffold tubes, planks, access ladders, props, etc) are to be maintained, inspected, serviced, cleaned, repaired and tested by a competent person in accordance with the manufacturer’s instructions and relevant standards.

27. Where a scaffold or supporting structure is identified to be in an unsafe condition, personnel are to be

restricted from accessing the scaffold, and necessary repairs, alterations etc are to be made. 28. Where scaffold components are identified as being in an unsafe condition, the items are to be removed from

service and reported to the relevant supervisor. 29. Scaffold components are not to be modified. 30. If a scaffold is incomplete or unsafe, and left unattended, appropriate controls, including the use of signage

and barricading are to be used to prevent unauthorised access onto the scaffold.

Note: Refer also to Scafftags – HB# 560077, for further information regarding the requirements for the use of Scafftags to signify incomplete or unsafe scaffolding.

31. Records of all tests, maintenance, inspections (including fitness for use inspections and during use

inspections), commissioning and alterations of / to scaffolding and scaffold components are to be kept and maintained on site as per AS/NZS 4576 and this Corporate Standard.

32. The number of the ‘certificate of registration of registrable plant design’ (i.e. registration number) is to be

permanently marked on all SCL owned prefabricated scaffolds, so as to be clearly visible (e.g. marked on a scaffold component located at a main scaffold access point). Note: The registration number does not have to be marked on all components of a prefabricated scaffold.

Training and Competency 1. Workers involved in erecting, modifying or dismantling scaffolding from which a person or object could fall

less than 4m in height are to be deemed competent to undertake such activities. 2. Workers involved in erecting, modifying or dismantling scaffolding from which a person or object could fall

more than 4m are to be certified scaffolders. 3. All workers erecting, modifying or dismantling scaffolding are to be provided with training as per Scaffolding

Corporate Standard Awareness - HS067 – HB# 550989. 4. Workers using scaffold as a means of access are to be provided with training as per Accessing (Scafftag)

and Working on Scaffold Corporate Procedure Awareness - HS102 – HB# 751603.

Attachment 1: Scaffolding Requirements 1. Edge protection which includes handrails, mid-rails and bottom rails / toeboards is to be erected on all

scaffolds over 2m and on any scaffold under 2m where there is a likelihood of a fall occurring. The requirements for each of the rails / boards is as follows: a. The top / guard rail of the edge protection is to be at least 900mm high, but not higher than 1100mm,

above the working surface. b. Mid-rails are to be installed such that the spacing between the mid-rails and all other rails or toeboards,

does not exceed 450mm. c. Toe boards are to be a minimum of 150mm high and bottom rails are to be 150 – 250mm high.

2. Where the surface from which work is to be done is sloped, the top rail of the edge protection is to be 900

mm higher than where that surface, if extended downwards at that slope, would intersect with the edge protection.

3. Where the slope of a work surface is over 26 degrees, sturdy mesh, sheeting or other suitable material, is

to be extended at least 900mm high from the base of the edge protection or toe board (where applicable). 4. Scaffolds and scaffold members are not to be used as anchorage points for any chain block, rope block,

purchase, snatch block, deflective sheave, barrow hoist or other lifting device, unless specifically designed by a competent person to withstand the forces involved.

5. Where perimeter containment screening is to be used on scaffolds, it is to comply with the following

requirements: screens are to be made of mesh, timber, plywood or metal sheeting; the framework supporting a screen must be able to bear the load of the screen; each gap measured horizontally and / or vertically between screens, and the framework supporting the

screens, is not to be over 25mm; and screens are to be installed no higher than the upper most tie.

6. Where mesh is to be used as perimeter containment screening, it is to be made of at least 2.5mm diameter

steel with a tensile strength of at least 380 Mpa. 7. Mesh is to also comply with the requirements detailed in the table below:

8. Loads placed upon scaffolds are not to exceed manufacturer’s recommended working load limits. Factors

to consider to avoid overloading of a scaffold include: the number of working platforms loaded are not to exceed the number specified in the scaffold plan; scaffolds are not to be used to support formwork and plant, such as hoist towers, unless the scaffold is

Mobile Scaffolds 9. The following minimum requirements for mobile scaffolds are to be implemented:

access only via an internally placed ladder (i.e. within the scaffold); maximum height to be no greater than 9 metres or 3-times the least base dimension; edge protection to be fitted inclusive of top rails, mid rails and toe boards (i.e. where a risk of falls /

falling objects exists); castors with adjustable legs to be used and adjusted to keep the platform level when supporting

structure is at different heights; castors are to be locked prior to accessing scaffold; plan bracing is to be used at base of scaffold to provide greater stability (where relevant); prior to moving mobile scaffold check that:

- no persons are on the scaffold; - the ground is firm / level; - the supporting structure is free from obstructions; - there are no overhead obstructions (e.g. power lines etc.); - no equipment and material can be dislodged from the platform;

always push or pull mobile scaffold from the base – never use powered vehicles to move mobile scaffolds.

Attachment 2: Handover Certificate Requirements Handover certificates are to include: Identification of Stanwell Corporation as the person requiring the scaffold (including site address and phone

number). The name and address of the contractor that erected or altered the scaffold. Relevant certificate number/s of persons that erected or altered the scaffold. The address and specific onsite location of the scaffold. A description of the type of the scaffold (including drawings (attached to certificate), top working platform

height and duty classification). The size of the scaffold (including bay sizes). The duty loadings applicable to the scaffold. The maximum number of platforms that can be loaded or worked from at any one time (number of working

decks). The intended purposes of the scaffold. The date and time of handover. Confirmation that the scaffold complies with any design specifications and provided drawings, any supplier’s

information, the Queensland Scaffolding Code of Practice, AS 1576 (1-5) and AS 4576 and is suitable for its intended tasks.
