Case Study - Hospital Client Brief

JPW Group were appointed by The Client in late 2013 and this continued through 2014 to investigate operational problems with the installed MEP systems.

A number of difficulties had become evident to the building operators and clinical staffregarding the operation of the HVAC systems (in particular to critical areas; Operating Theatres,ICU, CSSD departments and adjoining spaces).

The Client engaged JPW to investigate the initial design, equipment selection and installationof HVAC Plant in December 2013. The investigation and report was delivered on time, after 6weeks.

It became clear in the findings of this initial report that it would be of substantial benefit to theclient that a full investigation be undertaken with regard to all MEP systems.

JPW were subsequently engaged to deliver a full 3rd Party investigation and report detailing thefailing of the installed systems from the Initial design, installation and the implementation of theMV, LV and electrical distribution systems at the The Client Hospital facility. This detailed reportwas again delivered on time, in 8 weeks.

The ongoing review of the MEP systems operation throughout this period led the Client toagain engage JPW to deliver a full 3rd Party investigation for the Mechanical MEP, Fire Fightingand Plumbing Systems. This was closely followed by the Building Management Systems. TheMechanical report was delivered in 10 weeks, as contracted. The BMS Systems report wassubsequently delivered within the client requirement of 6 weeks.

23

Case Study - Hospital Outcome 1

The MEP investigations and reports delivered by JPW during 2014 each provided recommendations that advised, enabled and assisted The Client in remediating system deficiencies. This has reduced the Clients risk and assisted the Client in delivering the objective of first class healthcare. Each of the 3rd Party investigations and reports were split into analysis “blocks” with a view to obtaining a ‘whole picture’ from design intent through to installed system and system operability. These blocks consisted of the following interlinked analyses: • Original consultant design brief to design documents (incl. drawings, specifications, etc.).• HTM standards to design documents.• Design documents to as-built‘s.• HTM standards to as-built‘s.• As-built‘s of physical installation.• Best practice operability to physical condition.• Correlation between all the above. Two key deliverables were produced by JPW for each system. These were an investigative phase (phase 1) commencing with high level system analysis. Initially a desktop exercise of document evaluation was undertaken.

Site walkthroughs and inspections were carried out and documented, with a categorisation of each operational risk against patient welfare/safety.

24

Case Study - Hospital Outcome 2

The second deliverable (phase 2) was the detailed analysis of the results of the investigation, with a series of specific remedial actions recommended against each item identified. The Client contracted JPW to review and prepare each report generally in line with the Scope of Services stated as follows;

Phase 1JPW conducted complete and thorough inspections and reviews of the AHU, Electrical, Mechanical and BMS systems at The Client hospital.

Phase 2For each system JPW provided unbiased, independent third party reports that identified required standards, the status of the installed MEP systems, the discrepancies and shortfalls between the specified standards and the systems installed and the suitability of the system installed for the intended application.

These reports were in respect of:

• Compliance of the design and specifications with all relevant international codes.• Review of any proposed value engineering revisions at tender stage.• The review of the contracted terms, specifications and timescales.• Variations or site instruction (if any)• Site engineer design/specification comments.• Material submittals and approvals.• Factory acceptance testing reports (if any)• Site inspection reports.• Maintenance (record from contractor)• Site acceptance testing reports.• Commissioning and testing reports.• TOC’s• Punch lists

25

Case Study - Hospital HVAC Investigation & Report

The HVAC AHU systems were found to be not compliant with the requirements for a Healthcare facility. The AHU by AHU 3rd Party report was issued and identified the extensive deficiencies of both design and implementation. This report addressed each issue, resulting in a clear and comprehensive list of recommended actions enabling the Client to prioritise remedial works with the existing contractor / supplier. JPW have been appointed by The Client to manage remedial works planned during 2015, according to a schedule of priorities based on an analysis of risk to patient safety and comfort, and to The Client’s on-going business.

26

Case Study - Hospital Mechanical & Public Health Investigation & ReportThe Mechanical, Fire Fighting and Public Health systems were found to be not compliant with requirements in a number of key areas. An item by item report was issued on the due date noting deficiencies of both design and implementation. The report detailed variances between initial Design / supplied installation and relevant codes and Standards. The report addressed each issue, resulting in a clear and comprehensive list of recommended actions enabling the Client to prioritise remedial works. JPW have been appointed by The Client to manage remedial works planned during 2015, according to a schedule of priorities based on an analysis of risk to patient safety and comfort, and to The Client’s on-going business.

27

Case Study - Hospital Electrical Investigation & Report

JPW’s investigation into the electrical system was comprehensive, and covered all systems from MV and generation through distribution via conduit drops and switched socket outlets. The above is in respect of the following systems:

• MV Switch Gear • MV Monitoring and Control System• MV cabling and Control cabling• MV Transformers• LV Switchgear• UPS• IPS/ Generators/ • Electrical Services and Controls for Fuel Systems • ATS’s • Lighting Control Systems • Emergency Lighting System (Central battery) • External Lighting• Earthing and Bonding/ Lightning Protection• BMS interface signals. • Electrical supply configuration to mechanical plant• Vertical Transport Power Supply• Fire Detection and Alarm Systems The output of this brief was a series of reports and remedial actions which highlighted any issues at either the design, compliance, build, value engineering, commissioning or subsequent operations stages which posed a non-compliance in line with the client brief. JPW have been appointed by The Client to manage remedial works planned during 2015, according to a schedule of priorities based on an analysis of risk to patient safety and comfort, and to The Client’s on-going business.

28

Case Study - Hospital BMS Investigation & Report

The Building Management Systems were found to be not compliant with requirements and controlling the MEP plant incorrectly. An item by item report was issued on the due date noting deficiencies of design and implementation and analysing plant control deficiencies. The report detailed variances between initial Design / supplied installation and relevant codes and Standards. The report addressed each issue, resulting in a clear and comprehensive list of recommended actions enabling the Client to prioritise remedial works. Extensive remedial works are planned during 2015, according to a schedule of priorities based on an analysis of risk to patient safety and comfort, and to The Client’s on-going business.

29

Case Study - Hospital Summary

In summary the 3rd part investigation, analysis and structured reports prepared by JPW for The Client have identified the deficiencies and shortcoming allowing the Building Operator and The Client to modify their SOP’s (Standard Operating Procedures) to provide ‘work rounds’ to mitigate risk to this healthcare facility. The Client has also been able to assess the operational risk and its impact on revenue. The Client has been able to ensure that HVAC / MEP systems required to support critical clinical areas (Operating Theatres/CSSD/ICU etc) were modified to be fit for use before scheduled healthcare operations. The Client has recently appointed JPW to prepare relevant scopes of work for, agree programme and budget for, and manage the delivery of an inclusive list of the required MEP remedial works for delivery during 2015 and 2016. The Client has been able to schedule the required remedial works during suitable periods, rather than due to equipment or system failure.
