Cyst Type Premenopausal Postmenopausal*

Simple Cyst ≤ 3 cm: Don’t need to mention, no f/u 3 - 5 cm: “almost certainly benign,” no f/u 5 - 7 cm: “almost certainly benign,” yearly US f/u > 7 cm: difficult to assess, MRI or surgery

≤1 cm: Don’t need to mention, no f/u 1 - 7 cm: “almost certainly benign,” yearly US f/u >7 cm: difficult to assess, MRI or surgery

Hemorrhagic Cyst

≤ 3 cm: Don’t need to mention, no f/u 3-5 cm: Mention, but no f/u > 5 cm: Mention, US f/u in 6 – 12 weeks

Any hemorrhagic cyst in late postmenopausal is suspicious for neoplasm, requires surgical eval

Endometrioma Any size: f/u US in 6-12 weeks to distinguish from hemorrhagic cyst, then surgery or yearly US f/u for undetermined duration

Yearly US monitoring (indeterminate duration) vs surgery

Dermoid Surgery versus US f/u in 6 mos to 1 year No other imaging if classic appearance

Surgery vs US f/u in 6 mos – 1 yr if not removed

Complex cyst Thin septation ≤ 0.3 cm: Treat as simple cyst

Indeterminate type cyst

Any size: f/u in 6-12 weeks. Surgical evaluation
