ACCP assists Aboriginal andTorres Strait Islander people in

South Western Sydney who haveor are at risk of developing a

chronic condition throughproviding holistic, culturally

appropriate health care.ACCP operates from Budyari

Aboriginal Community HealthCentre at Miller.

WHAT IS A CHRONIC CONDITION?

A chronic condition that affects aperson for longer than 6 months. Some examples of chronic conditions include:

• Diabetes

• Cancer

• Cardiovascular Disease

• Respiratory Disease

• Kidney Disease

• Mental Health

HOW CAN THEACCP HELP

Your Aboriginal Health Worker and Care Coordinator will support you to access the necessary services and to support you in controlling your health.

HOW TO ACCESSTHE ACCP

• General Practitioner referral

• Specialist referral

• Health Professional referral

• Self-referral

WHAT WILL HAPPEN?Once your referral has been received,an Aboriginal Health Worker or CareCoordinator from ACCP will contact you and arrange a visit.

Our Team will meet with you to discussyour needs and work towards developing a plan with your GP.

WHO IS OUR TEAM?ACCP is staffed by a range of healthprofessionals including:

• Aboriginal Health Workers Nurses

• Allied Health Professionals

Our clinical staff and Aboriginal HealthWorkers work collaboratively to ensureyour needs are met holistically.
