1. PURPOSE 1.1 A requirement exists within CSC for procedures and planning of safe entry and

operations in confined spaces. The aim of this SOP is to outline the steps that must be followed to ensure safe operations and emergency procedures in confined spaces. These operational procedures describe the hazards and define the rules and conditions under which persons will be permitted to work in confined spaces. Its main purpose is to ensure all necessary precautions are taken to protect the health and safety of all personnel.

1.2 Management and the employee(s) are both jointly responsible for the safety of all

personnel entering a confined space. The employees entering the confined space are also responsible to confirm that all the safety requirements have been met before entry.

2. REFERENCE STANDARDS

Confined spaces are not designed or intended for human occupancy except for the purpose of performing work. Entry into confined spaces presents special hazards to workers. These operational procedures are issued pursuant to:

Canada Labour Code (CLC), Part II;

3. DEFINITIONS 3.1 For the purpose of this SOP the following definitions will apply:

"Class of confined spaces"

A group of at least two confined spaces that are likely, by reason of their similarity, to present the same hazards to persons entering, exiting or occupying them.

“Confined space”

A tank, silo, storage bin, process vessel, underground vault, tunnel, or other enclosure not intended for human occupancy, except for the purpose of performing work, and:

has a limited number of openings for entry and exit;

has poor ventilation;

in which there may be an oxygen deficient atmosphere, or

in which there may be an airborne dangerous substance.

"Hot work" Any work where flame is used or a source of ignition (spark) may be produced.

"Competent person" (A) qualified because of such factors as knowledge, training and experience, to do

assigned work in a manner that will ensure the health and safety of persons,

(B) knowledgeable about the provisions of the Act and the regulations that apply to the assigned work, and

(C) knowledgeable about potential or actual danger to health and safety connected

with the assigned work; "Qualified person"

A person who, because of knowledge, training and experience, is qualified to perform safely and properly the duties specified under the directive in the following areas:

hazard survey

entry and emergency procedures

emergency requirement and equipment maintenance

entry permits

hot work

**Performance of these duties may be assigned to different qualified persons.**

"Lower explosive limit”

The lower limit of flammability of gas, vapour or dust or any combination of these at

ambient temperature. For gases and vapours, the lower explosive limit is expressed as a percentage in air by volume. For dust, the lower explosive limit is expressed as weight of dust per volume of air.

"Restricted space or access" Is usually applied to a space or area that does not meet all the requirements listed within that the actual confined space definition, but does exhibit (or has the) potential to be hazardous to persons who enter or occupy the space. “Oxygen deficiency”

With respect to the atmosphere of a confined space, a concentration of oxygen by volume in the atmosphere that is less than 19.5 percent at 101.3 kPa of atmospheric pressure (equivalent to a partial pressure of oxygen less than 135mm of mercury). “Oxygen enriched”

With respect to the atmosphere of a confined space, a concentration of oxygen by volume in the atmosphere that is more than 23 percent at 101.3 kPa of atmospheric pressure. There are numerous locations that are classified as confined spaces as defined by

the Treasury Board OSH Directive 2 -07 and the COSH Regulations, Part XI. Refer

to Annex A for a list of typical confined spaces, which may be found at

Dorchester Penitentiary

"Health Unit”

Means a consultation and treatment facility that is in charge of a person who is listed as a registered nurse under the laws of any province.

"Hantavirus”

Means a virus that is found in some rodents, especially deer mice in Canada. People can contract the disease when they breathe the virus that is found in the urine, saliva or droppings of infected rodents. The disease caused by hantavirus infection is commonly known as hantavirus pulmonary syndrome (HPS). Early symptoms of the disease are flu-like (fever, chills and muscle aches). The disease worsens rapidly and can be fatal. For prevention of hantavirus please check in Annex "

"MSDS”

A Material Safety Data Sheet (MSDS) contains pertinent information related specifically to a controlled product. Such as Product Identification and Use, Hazardous Ingredients, Physical Data, Fire and Explosion Data, Reactivity Data, Toxicological Properties, Preventive Measures, First Aid Measures and Preparation Date of the MSDS.

3.2 Class of Confined Spaces

The following criteria is used to identify a class of confined space: (a) the confined spaces may be of similar size and shape; (b) they may contain equipment and machinery that have the same or similar

purpose and use. (c) they may present similar hazards, such as:

restricted entry and exit;

probable exposure to the same or similar hazardous substances, such as: asphyxiants, explosive gases, biological hazards;

exposure to an oxygen-deficient atmosphere;

exposure to hazardous substances near or surrounding the confined space;

possible leaks from pipes, or water entering;

danger of drowning or being covered in flowing material; (d) the entry and/or emergency procedure can apply to every confined space in a

class.

4. GENERAL 4.1 The safety standards in this SOP are applicable to all CSC personnel as well as

contractors performing work in and around confined spaces. At the Institution, the Warden will be responsible to ensure compliance with the provisions of the standards as laid out in the above mentioned references and this SOP.

4.2 To ensure the effectiveness of the SOP, the Warden shall appoint a Program

Administrator. The Program Administrator will be responsible for overseeing the issue of and return of permits, the proper testing of the spaces before entry, delegation of responsibility to a Supervisor during the work and ensuring the security of the confined space upon completion of work.

4.3 No employee or contractor shall be permitted to enter a confined space without

receiving an evaluation, written in a language which is understood by the employee / contractor, concerning the level of hazard in the confined space. Entry will be made in compliance with this standard and the requirement as outlined in the references.

4.4 No employee or contractor shall enter a confined space without an entry permit. 4.5 Except for life and death situation that can only be authorized by the Warden or

his/her delegate, any deviation from this SOP shall call for an immediate cessation of work and re-evaluation of the conditions.

NOTE The Program Administrator for Dorchester Penitentiary will be during the Chief of Works or Chief of Engineering. After Hours the Program Administrator will be the on-duty delegate or the Correctional Supervisor.

5. RESPONSIBILITIES

5.1 Warden

The warden is the official responsible for the final review and approval of the plan. The warden will ensure that resources are available to properly follow the plan.

5.2 Program Administrator

The Confined Spaces Program Administrator at Dorchester Penitentiary will jointly be the Chief of Works and the Chief of Engineering. The administrator will evaluate the Confined Spaces Program on an on-going basis. Any changes that may be required due to deficiencies or operational considerations must be implemented or, if the changes are not within the administrator's authority, brought to the attention of the Warden of the Institution. The administrator will have knowledge of the codes and standards that are applicable to the program. The program administrator will have the responsibility of the implementation and maintenance phases of the program.

The administrator will follow all of the relevant regulations, codes, and standards. The administrator will also ensure that those persons that have confined spaces responsibilities are provided with the appropriate training. The administrator will ensure that all required protective equipment and devices are available and maintained. This includes appropriate personal protective equipment, locks and tags, monitoring equipment and rescue equipment. The administrator will ensure maintenance of all records associated with the program. They will also ensure that copies are provided to individuals as appropriate.

5.3 Supervisors A supervisor is any person who has direct authority or supervises another worker or service provider. Some workers will have supervisory responsibilities with respect to the inmates who assist them in their daily duties. Supervisors will ensure that no person enters a confined space unless this person has received approved training in confined space entry procedures. Supervisors of workers who enter confined spaces will ensure that the procedures outlined in this program are followed. Supervisors must have a thorough knowledge of the confined spaces program components

5.4 Workers For the purpose of this confined spaces program, a worker is anyone who is performing a duty that is within the scope of this program. Confined space entry workers are responsible for following confined space entry procedures outlined in this program and using protective equipment and devices supplied to them Reporting deficiencies or hazards to supervisors.

5.5 Joint Occupational Health and Safety Committee

The joint occupational health and safety committee will be responsible for coaching management, supervisors and workers as to the functioning of the program. It will be responsible for conducting hazard assessments, when required.

5.6 Occupational Safety and Health Advisor

The occupational safety and health advisor will be a resource person for the lock out / tag out program. Supervisors or other personnel may call on the occupational safety and health advisor to provide advice and guidance or to perform hazard assessments. The occupational safety and health advisor will provide advice and guidance relating to the confined spaces program to all the above mentioned parties. The occupational safety and health advisor will assist the JOSH committee in reviewing and evaluating the confined

spaces program annually. The occupational safety and health advisor will be responsible for liaising with RHQ to assure a consistent regional approach on the confined spaces program.

5.7 Confined Space Attendant The Confined Space Attendant is assigned and responsible for the well-being of workers required to enter a confined space. His / her duties include, but are not limited to: - Ensuring conditions as detailed in the Confined Space Entry Permit are met; and - Ensuring requirements and procedures as outlined in the Confined Space Entry

Documents are adhered to

5.8 Confined Space Rescue Personnel

The confined space rescue personnel is responsible to assist the confined space attendant in the event of an accident or other emergency. His / her duties include, but are not limited to: - must be trained and able to assist in emergency / rescue operations; and - must be in the vicinity of the confined space being entered

6. TRAINING 6.1 All personnel entering a confined space, acting as an attendant or a rescuer will be

trained in procedures as outlined below:

Training for Employees

Confined space entry and attendance procedures (including permit system) (Compulsory every 2 years, Certified instructor)

Gas detectors (Supplier)

Lock out / Tag out procedures (Every 3 years, Certified instructor)

Training for Rescue Personnel

First aid including CPR and artificial respiration

Note: Staffs have access to 24 hrs - 7 days health unit at the Regional Treatment Center. The Regional Treatment Center is a health unit that is in charge of a person who is listed as a registered nurse under the laws of the province of New Brunswick.

7. HAZARD IDENTIFICATION 7.1 Some of the hazards that could be encountered and should be scanned for in

confined spaces are:

Oxygen deficiency or enrichment

Infectious diseases

Insects and vermin

Ionizing and electromagnetic radiation

Combustibility of gases or vapors (i.e. fires/explosives)

Toxic gases, vapors, dusts or fumes

Difficult entry/exit

Shifting or collapse of bulk materials

Contents of pipes and supply lines

Mechanical equipment

Electrical shock

Working surfaces (i.e. slips/falls)

Residual chemicals

Temperature and humidity extremes

Drowning

Poor visibility

Noise

Psychological factors (i.e. claustrophobia )

Steam

Propane

Hantivirus

7.2 Combustible and Toxic Gases

Toxic gases present two kinds of risks in a confined space: irritation and chemical asphyxiation. Toxic gases that are especially dangerous in confined spaces include but are not limited to:

Hydrogen Sulphide ( H2S ) (PEL 10ppm)

Hydrogen Sulphide is a toxic, flammable and highly explosive gas, which is heavier than air, so it collects in low places. It is colourless at low concentrations or greenish yellow otherwise. A by-product of sewage treatment, petroleum and other industrial processes, it may be encountered in mines, gas wells, sewers and similar installations. In low concentrations, H2S smells like rotten eggs. However, this gas quickly deadens the sense of smell, leading to a false sense of security. In high concentrations of hydrogen Sulphide, collapse may occur with little or no warning. Hydrogen Sulphide can kill in minutes.

Methane (CH4) (LEL 10% or less)

Methane is highly explosive. It can leak from a gas line and can be a by-product of slow-moving sewers. It is a simple asphyxiant, suffocating an individual by displacing oxygen. It is lighter than air and is usually found at the upper level of a confined space.

Sulphur Dioxide (SO2) (PEL 2ppm)

Sulphur dioxide is colourless but has a strong suffocating odour. It is poisonous in small amounts.

Carbon Monoxide (CO) (PEL 25ppm)

Carbon monoxide is a toxic, colourless, odorless, tasteless, combustible and deadly gas that is slightly lighter than air. It is a product of incomplete combustion, and is commonly found in engine exhaust. Carbon monoxide enters the blood stream through the lungs. It has an extreme affinity for the hemoglobin in the bloodstream of about 200-300 times that of oxygen. As a result, carbon monoxide quickly replaces oxygen in the bloodstream and causes asphyxiation.

**There could be other gases and vapours that may be present within a confined space at any given time that will require the worker(s) to observe any safety guidelines set out for them**

8. PHYSICAL IDENTIFICATION OF CONFINED SPACES 8.1 All confined spaces must be identified and all workers at risk of entry must be

informed. Warning signs at entrance points are a means of preventing inadvertent entry and is required when the entrance to a confined space is easily accessible to non-authorized personnel and or the confined space is likely to contain a harmful atmosphere or other danger. Bilingual warning signs are placarded at every entry of every confined space and restricted area.

9. HAZARD ASSESSMENT 9.1 Where a hazard assessment required under this section identifies existing or

potential hazards, each confined space or class of confined spaces shall be identified as "permit entry only".

9.2 No employee or contractor will enter the confined space unless the appropriate entry permit has been issued and signed by a qualified person and, prior to entry, explained to, understood by and signed by the employee/contractor.

9.3 Where it is likely that a person will enter a confined space in order to perform work,

and that an assessment has not been carried out, the Program Administrator will appoint a qualified person to:

a) conduct an assessment of the physical and chemical hazards to which the

person is likely to be exposed in the confined space or the class of confined spaces; and

b) specify the tests that are necessary to determine whether the person would be

likely to be exposed to any hazards identified. 9.4 The qualified person will record the findings of the assessment and provide a

signed and dated report to the Program Administrator who will make a copy of any report available to the workplace Health and Safety Committee and will keep a copy on file.

9.5 A qualified person must review the assessment report at least once every three

years to ensure the assessment of the hazards is still accurate. The assessment report need not to be reviewed until it becomes likely that a person will need to enter the confined space. If the confined space has not been entered in the three years preceeding the time when the report should have been reviewed and no entry is scheduled.

10. SAFE ENTRY PERMIT 10.1 As outlined in the references standards, any confined spaces procedures developed

shall include an entry permit system which shall include a check list of entry requirements to be given to and signed by the employee(s) or contractors. Everyone has the right to a full explanation of the implications of entry into the confined space prior to signing the entry permit. An entry permit shall:

a) specify, in each case, the length of time for which the entry permit is valid;

b) record the name of the person(s) entering the confined space, the time of entry

and the anticipated time of exit;

c) specify the protection equipment that is to be used by every person who is granted access to the confined space;

d) specify any insulated protection equipment and tools that a person may need in

the confined space; and

e) specify the protection equipment and emergency equipment to be used by a person who takes part in the rescue of a person from the confined space or in responding to other emergency situations in the confined space.

10.2 A Safe Entry Permit shall be completely filled out each time a confined space is

entered for any reason.

** Legislation requires that these records must be retained for 10 years at the

facility ** 10.3 The Entry Permit system shall work in the following manner:

a) the Program Administrator must complete a “Confined Spaces Entry Permit”

and attach it to every work order for which entry into a confined space is required. A maximum of no more than eight hours will be allowed per permit;

b) the Supervisor and the employee(s) who will perform the work, review the permit

and, when the employee is ready to start the work, the Program Administrator shall be provided with Copy 2 to confirm the date and time of start of the work;

c) if the work is not completed within eight hours after the work has started, the

employee must return Copy 1 of the permit to the Program Administrator to request an extension. If the work is completed before the end of the eight-hour time limit, the employee must indicate on Copy 1 of the permit the time of completion. Copy 1 is to be returned to the Program Administrator as soon as the work is completed to allow the latter to ensure the security of the confined space is locked; and

d) if the Program Administrator has not received Copy 1 within eight hours after

the start time, he shall investigate at the confined space immediately.

Where Hot Work is to be performed in a Confined Space, a Hot Work Permit must be

obtained from the Program Administrator as part of the Hot-Work Permit Program.

All welding operations to be conducted in confined spaces shall be undertaken after

a Hot Work Permit is obtained.

11. ENTRY PROCEDURES 11.1 For any work that is CONTRACTED OUT, the Program Administrator must ensure

that the tender request or the work order clearly indicates the need for the contractor to comply with CLC, COSH, TB OSH Directives and Provincial Health and Safety Regulations' requirements. The following procedures shall apply to all contractors in the same manner as they apply to all CSC employees:

11.2 Before the confined space is entered a qualified person shall ensure/verify that:

a) the hazard evaluation section of the Confined Space Entry Permit is completed as per Annex "C";

b) any liquid in which the person could drown has been removed from the confined

space;

c) any free-flowing solid in which the person may become entrapped has been removed from the confined space;

d) the entry of any liquid, free-flowing solid or hazardous substance into the

confined space has been prevented by a secure means of disconnection or the fitting of blank flanges;

e) all electrical and mechanical equipment that may present a hazard to the person

has been disconnected from its power source, real or residual, and has been

locked out. All personnel shall follow the Dorchester Penitentiary Lockout /

Tagout Safety Program; and

f) after the isolation has been completed, it may necessary to purge or "clean out" the confined space before entering.

"Air" is the most common purging agent, but there are time when using air to purge a space can be hazardous. When flammable vapours are present above the U.E.L. and to rich to burn. Could create an explosive mixture. "Water" may be used, but can react dangerously with many chemicals. "Steam" is a good purging agent, but special procedures must be used with substances that have low ignition temperatures or flash points. "Inert Gases" such as nitrogen may be used to get rid of flammable vapours, but the atmosphere in the space will become oxygen deficient.

g) the opening for entry into and exit from the confined space is sufficient to allow

the safe passage of a person using protection equipment.

11.3 The qualified person shall verify that the specifications set out in this section are

complied with at all times that a person is in the confined space. 11.4 The qualified person shall, in a signed and dated report to the program

administrator, set out the results of the verification carried out, including the test methods, the test results and a list of the test equipment used.

11.5 To minimize hazards to personnel entering confined spaces, it is essential to

sample the atmosphere before anyone is permitted to enter. In addition to

monitoring prior to entry, it is equally important to continuously monitor the

atmosphere during entry and as long as people are in the space, since sudden fluctuations in atmospheric conditions can cause a change from a safe to a hazardous environment in a very short time.

11.6 An approved Multi-Gas detector/monitor shall be used to monitor the level of

oxygen and any combustible/explosive and/or toxic gases. Instructions for use of the Multi-Gas monitor will be given to all employees who will be entering the confined space either for work or as a member of the rescue team; monitoring the conditions of the confined space; or calibrating or servicing the monitor.

11.7 All monitoring equipment must be calibrated and shall be inspected before every

use. All batteries must be fully charged before each use. All equipment shall be maintained at frequencies prescribed by the manufacturer. The Program Administrator shall keep a log of the manufacturer’s recommended maintenance intervals and shall record on this log all maintenance performed on this equipment.

11.8 Where a person is about to enter a confined space, a qualified person (who could

be the same person) shall verify the following conditions; a) the concentration of any chemical agent, or combination of chemical agents, in

the confined space to which the person is likely to be exposed will not result in the exposure of the person

(i) to a concentration of an airborne chemical agent or combination of

chemical agents in excess of the value for that chemical agent adopted by the American Conference of Governmental Industrial Hygienists (ACGIH) in its publication entitled "Threshold Limit Values and Biological Exposure Indices for 2000-01, "as amended from time to time, or

(ii) to a concentration of an airborne chemical agent or combination of

chemical agents that is more than 50 per cent of the lower explosive limit of the chemical agent or combination of chemical agents, or

(iii) where a source of ignition may ignite the concentration of an airborne

agent or combination of chemical agents, to a concentration that is more than 10 per cent of the lower explosive limit of the chemical agent or combination of chemical agents.

b) the concentration of airborne hazardous substances, other than chemical

agents, in the confined space is not hazardous to the safety or health of the person, and

c) the percentage of oxygen in the air in the confined space is not less than 19.5

per cent by volume and not more than 23 per cent by volume, at normal atmospheric pressure.

Every test conducted shall be recorded on the “Confined Space Entry Permit.

Tests for the presence of explosive gases in confined spaces shall be carried out

before the covers, door, hatches are opened.

11.9 Similar tests shall be made whenever a confined space is re-opened, regardless of

the length of time it has been closed. 11.10 If the tests show a confined space to be unsafe to enter due to:

a) an unacceptable concentration of explosive gas, the conditions shall be reported to the Program Administrator immediately and the area barricaded. All planned work in the confined space shall cease until the condition is rectified; or

b) a concentration of hazardous/toxic gas and/or a lack of oxygen, acceptable

mechanical ventilation equipment shall be used until tests show that the confined space is safe to enter.

As a last resort, respiratory protection conforming to the CSA requirements listed in the reference standards shall be used.

11.11 Where it is not practical to reduce and maintain the concentration of toxic airborne

dangerous substances to a safe level or to correct an oxygen deficiency in a hazardous confined space or to completely remove a liquid or free-flowing solids, an employee may be permitted to enter such space if: a) the employee is wearing an approved breathing apparatus;

b) the employee is wearing a safety harness, or other similar safety equipment,

securely attached to a lifeline that is attached to a secure anchor outside the hazardous confined space and controlled by an employee trained in the emergency and rescue procedures;

c) the entry of material in the confined space is prevented;

d) additional trained employees, of which at least one is qualified in first aid, are

readily available in the immediate vicinity;

e) the concentration of any flammable or explosive substance is reduced to and maintained at a level below the lower flammable or explosive limit of that substance;

f) any additional precautionary measures are taken, including lockout/tagout

procedures.

** No person shall use respiratory protective equipment until adequately trained in

conformance with the manufacturer’s requirements and as outlined in the CSA

Standards **

12. HOT WORK 12.1 Pre-entry requirements

The Program Administrator shall be consulted for confined entry regulations. Inspections and authorization shall be required by writing "HOT PERMIT" as per Annex "C" before any confined space entry. The employer shall have written request procedures for confined space emergencies. Before a welder enters a confined space to undertake any welding, cutting, or allied process, the following requirements shall be met: a) flammable or hazardous solids or liquids shall be removed;

b) the space shall be ventilated to remove flammable or hazardous gases,

vapors, or dusts;

c) the space shall be tested and evaluated by a competent person before and during the work to ensure that the atmosphere is safe;

d) adequate ventilation shall be provided to protect all personnel entering the

confined space; and

e) a worker shall be stationed outside the confined space to call for assistance when rescue and removal from the confined space is required.

Before a welder enters a confined space, a visual inspection shall be done on all electrical cables and connections that will be taken into the confined space. Before a welder enter a confined space, a leak test shall be done on all joints of any gas lines in order to eliminate the possibility of introducing gases into the confined space.

12.2 Small-Opening Entry Requirements Where a welder must enter a confined space that has limited small openings for entry and exit, the following requirements shall be met: a) The welder shall use an approved safety belt, harness, or other safety

equipment that is securely attached to a rope or lifeline. The lifeline shall be attached so that the lines or the welder's body does not become jammed in passing through a manhole or other small opening during the rescue operation. The equipment shall have been inspected prior to use and deemed in good working order.

b) The free end of the rope or lifeline shall be tied off and held by the worker

permanently stationed outside the confined space and can be used by the welder to signal for help.

c) The worker outside the confined space shall have a radio to summon help when necessary and shall not enter the confined space to assist the welder

in the confined space until another worker is present and precautions are taken.

12.3 Where an airborne hazardous substance may be produced by hot work in a

confined space, no person shall enter or occupy the confined space unless: a) the person uses a respiratory protective device that meets the requirements of

the reference standards.; or

the following section pertaining to the ventilation equipment is complied with. 12.4 Ventilation in Confined Space

Adequate ventilation in a confined space shall be provided to reduce the level of air contaminants such as gases, vapors, dusts, or fumes. a) below that which may cause illness or disease;

b) to prevent accumulation of contaminant vapor greater that 10% of the lower

explosive limit or as required by the regulatory authority; and

c) to prevent oxygen deficient atmosphere in excess of 23 per cent.

Compressed gases of any kind, other than compressed air, shall not be used to ventilate a confined space. Only air shall be used to ventilate a confined space.

12.5 Where hot work is to be performed in a confined space that contains hazardous concentrations of flammable or explosive materials, a qualified person shall patrol the area surrounding the confined space and maintain a fire-protection watch in that area until all fire hazard has passed. As well, fire extinguishers shall be provided in the area.

12.6 Service Equipment

Compressed gas cylinders and welding power sources shall be located outside of the confined space when welding and cutting is being done in a confined space. All portable equipment shall be secured properly to prevent accidental movement before operations are started.

12.7 Safety Requirements

All electric power shall be disconnected at source when arc welding is suspended for any substantial period of time to prevent accidental electric contract or shock occurring within the confined space. Also, all electrodes shall be removed from the holders and the holders carefully located to prevent accidental contact. For oxyfuel applications, the flame shall be ignited and properly adjusted for the required operation before it is passed into the enclosed area and passed out of the enclosed area before it is extinguished in order to prevent the accumulation of flammable gases in the confined space. The torch valves shall be closed and the gas supply to the torch at the manifold outlet or cylinder positively shut off outside of the confined space. Whenever a torch

is not used for a substantial period of time, such as during lunch or overnight, where practicable the torch and hose shall be removed from the confined space.

13. VENTILATION EQUIPMENT 13.1 Access to a confined space shall not be granted to any person unless ventilation

equipment is used to:

maintain the concentration of a chemical agent or combination of chemical agents in a confined space at or below the concentration referred to in section 10.8(a), or

maintain the percentage of oxygen in the air of a confined space within the limits referred to in section paragraph 10.8 (c),

13.2 The ventilation equipment must be equipped with an approved alarm system or

monitored by a qualified person who is in constant attendance on the ventilation equipment and in constant contact with the worker(s) in the confined space.

14. PROVISION & USE OF EQUIPMENT 14.1 The "employer" shall provide:

a) each employee who is granted access to a confined space with the protection equipment specified in section 9.1(c) of this SOP; and

b) each employee who is to undertake rescue operations with the protection

equipment and emergency equipment specified in section 9.1(e) of this SOP; 14.2 The employer shall ensure that every person who enters, exits or occupies a

confined space follows all the procedures established pursuant to section 9 and 10 and uses the protection equipment specified in section 9.1(c) and (d) of this SOP.

14.3 Prior to any entry into a confined space the following safety equipment shall be

assembled:

multi-gas monitor

portable 2-way radio (where conditions indicate)

fire extinguisher as required (No CO2 portable fire extinguisher shall be used in a confined space, since carbon dioxide gas displaces oxygen).

alarm horn (or other communication device) to summon aid

safety harness and lifeline

ventilation system in place and working to ensure a fresh air supply to worker

personal protective equipment (PPE) 14.4 Specialized safety equipment, in addition to that normally provided, shall be

available depending on the hazards associated with the specific confined space. It shall be inspected before each use following the necessary training, and shall be worn or used in accordance with the manufacturer’s instructions.

14.5 All PPE identified on the entry permit and/or hazard assessment form shall be utilized during the entry into a confined space. The personal protective equipment provided must meets the requirements outlined in the Section 2 - Reference

Standards. It is stressed that PPE is not a substitute for proper ventilation. 14.6 Where the hazard assessment deems it necessary, workers shall wear emergency

escape units (egress bottle) and shall have an air-monitoring device with them at all

times while in the confined space. Emergency escape units are intended for

emergency escape use only and are not acceptable for work in confined

spaces Other PPE may include safety goggles, hard hats, safety footwear, coveralls and/or a suitable breathing apparatus.

14.7 Where atmospheric conditions are such that it is unsafe or unhealthy to breath the

air in the space, respiratory protection meeting the requirements of CSA Standard Z94.4-93 - Selection, Care and Use of Respirators shall be provided. If necessary, supplementary air meeting the requirements of CSA Z180.1 - Compressed Breathing Air and Systems shall be provided.

Rescue Equipment

14.8 A safety harness with an attached lifeline shall be worn by an employee entering a

confined space where:

a) the space is provided with only one manhole or opening at the top and rescue may be difficult; or

b) dangerous gases, vapours, mists, fumes, dusts, oxygen deficiency or extremes

of temperature are likely to be present:; or

c) where respiratory protection is necessary.

The body type harness is to be put on and adjusted before entering the confined space and worn during entry.

14.9 The free end of the lifeline attached to the harness should be secured outside the

enclosed space. It must be of sufficient length to reach from an outside support to any point of work inside the confined space, and shall be of sufficient strength to bear the weight of the employee and must meet the requirements outlined in the reference standards.

14.10 Lifting and lowering equipment (including fall protection) shall be in place prior to

and when work is underway in the confined space. Lifting and lowering equipment must be inspected, maintained and tested before each use and shall meet the requirements set out in the CSA Standards outlined in the reference standards.

14.11 Appropriate positive pressure air supplied respiratory protection shall be available at

the site for use in the rescue/extraction of personnel working in the space.

Other Equipment 14.12 When working in maintenance holes not equipped with permanent ladders, person

shall use a portable non-conductive ladder that is readily available and secured. Ladders must be inspected for defects before use, protected from any residues

inside the confined space, and be tied off outside to prevent slippage. Portable ladders shall meet the requirements set out in the reference standards.

14.13 If conditions in the confined space permit the use of portable electric tools, they

shall be CSA approved as outlined in the reference standards and protected by approved ground fault circuit interrupters or ground fault receptacles. Where electric power tools are not appropriate, pneumatic and hand operated tools may be recommended.

14.14 Lighting required in the confined space, including flashlights, shall be of the

approved explosion-proof type suitable for the hazardous atmosphere (explosive gases, dusts, mists, etc.) and not over 12 volts. When using low voltage lighting or ground fault interrupters, the low voltage transformer and the ground fault interrupter must be left outside the confined space being entered. The operation and maintenance of all electrical equipment shall meet the requirements set out in the reference standards.

14.15 All equipment must be inspected before every use. All equipment shall be

inspected and maintained at frequencies prescribed by the manufacturer. 14.16 The Program Administrator shall keep a log of the manufacturer’s recommended

maintenance intervals and shall record on this log all maintenance performed on this equipment.

15. SITE OBSERVATION 15.1 A minimum of one person shall be posted outside a confined space as an attendant

and shall:

Have no other tasking which could detract from his function of observing the person in the space;

Control the life-line attached to the person in the space and ensuring the life-line is attached to some solid object;

Be self equipped with a safety harness;

Maintain visual/voice contact with the worker and monitor the ventilation equipment and work area.

Have a means to summon assistance in case of any emergency situation; and

Be trained in rescue procedures 15.2 For two or more people in the confined space, the nature of the work and the

difficulty of escape or rescue must be carefully considered. The Program Administrator, the Supervisor and the employee(s) entering the confined space will decide if more than one rescue person is required based upon the considerations and upon the availability of additional personnel from the immediate vicinity.

15.3 In addition to the attendant, where conditions warrant, an additional individual (a

rescuer) shall be present at the entrance to the confined space and shall:

Wear all required PPE including harness, life-line and some form of positive pressure air supplied respirator (where required);

Be present at all times when a person is working in the space;

Be trained in rescue procedures and First Aid; and

Shall not enter the space unless to rescue the person in the space and only after additional assistance trained in rescue procedures has been summoned and all PPE is being worn.

Inform the Shepody Healing Center, Health Care Services at 4118 or 4119. 15.4 In the event that either the observer or the rescuer is required to leave the entrance

to the space, the space must be vacated by those working in it until such time as the observer or rescuer returns.

15.5 Should a person or persons become injured or otherwise overcome while working in

a confined space or restricted area, the following action is to be started immediately:

a) Contact the Works and Engineering Heating Plant via radio maintenance

frequency call sign 253 or by telephone at ext. 4041 and advise of an emergency situation according to Institution procedures. Indicate that there is a “person down” in a confined space located at “................”.

b) Receive confirmation that instructions are understood. Works and Engineering

Heating Plant personnel should repeat it back to you and immediately inform the Program Administrator by phone.

c) The Heating Plant shall contact the confined space emergency rescue team.

d) The Heating Plant personnel shall contact the Shepody Healing Center, Health

Care Services for assistance at 4118 or 4119.

e) Request assistance from the Dorchester Fire Department and/or the Sackville Ambulance please dial 9-911, if required.

f) Assemble confined space emergency rescue team.

g) Using the Multi-Gas monitor, verify that a hazardous concentration of explosive

or other toxic gas does not exist in the confined space.

h) Set up ventilation equipment and start ventilating the confined space or don SCBA, if ventilation is not possible.

i) Rescue team to enter confined space and rescue workers that are in danger.

j) Upon arrival of the Health Care Services Personnel, provide all necessary

information to the person in charge.

k) A Hazardous Occurrence Report (HRDC form 1070) and Workers’ Compensation Board Accident Report form shall be filled out to record the incident with all particulars.

16. COMPLETION OF WORK 16.1 When the work is completed in the confined spaces, the supervisor must ensure to

replace the cover on the manhole or lock the entrance to the confined space.

16.2 The supervisor must complete and must indicate on Copy 1 of the permit the time of

completion. Copy 1 is to be returned to the Program Administrator as soon as the work is completed to allow the latter to ensure that the confined space is secured and locked

17. RESTRICTED AREAS

Some spaces within Dorchester Penitentiary do not fit the definition of confined spaces, but present special risks for people working in them. One of these risks is simply due to the fact that employees may be required to work alone in these spaces. If an employee is required to enter a restricted area, he or she will: - Advise another employee of the location of this restricted area - Advise this same employee of the type of work to be conducted - Advise this same employee of the length of time expected to be in the space - Give any other directions to this employee, under the circumstances, to ensure that the notified employee is able to provide assistance if needed - Advise this employee when the work in the restricted area is complete - Under special circumstances, restricted areas may be considered confined spaces, e.g. during hot work. When this is the case, a hazard assessment is to be carried out and appropriate permits need to be filled out.

NOTES

This permit is to be made in duplicate. The Program Administrator must complete a “Confined Spaces Entry Permit” and attach it to every work order for which entry into a confined space is required. The completed hazard assessments will be used to prepare the entry permits. If the work is not completed within eight hours after the work has started, the employee must return Copy 1 of the permit to the Program Administrator to request an extension. If the Program Administrator has not received Copy 1 within eight hours after the start time, he shall investigate at the confined space immediately. If the work is completed before the end of the eight-hour time limit, the employee must indicate on Copy 1 of the permit the time of completion. Copy 1 is to be returned to the Program Administrator as soon as the work is completed to allow the latter to ensure the security of the confined space. The Supervisor and the employee(s) who will perform the work, review the permit and, when the employee is ready to start the work, the Program Administrator shall be provided with Copy 2 to confirm the date and time of start of the work. This completed permit must be displayed in a conspicuous place where the entry is to be made. If the person(s) who signed this permit are relieved, the person(s) relieving must countersign this permit in the presence of the person(s) relieved. When all work is completed and area secure, all copies including hot work permit must be sent to Central Records to be kept on file for a minimum of 10 years as per Treasury Board of Canada, Handbook of Occupational Safety and Health, Section 11.12.

Hantivirus

A virus that is found in some rodents, especially deer mice in Canada and the United States, causes Hantivirus infection. Examples of rodent locations are rubbish piles, wood piles, garbage, weeds and long grass, infrequently used equipment and storage sheds. People can contact the disease when they breathe the virus that is found in the urine, saliva, or droppings of infected rodents. The disease caused by hantavirus infection is commonly known as hantavirus pulmonary syndrome (HPS). Early symptoms of the disease are flu-like (fever, chills and muscle aches). The disease worsens rapidly and can be fatal.

PREVENTION Note it apply to pigeon, bat and rodents droppings. 1. CLEAN up infected area in a way that minimizes production of airborne dust. 2. USE personal protective equipment such as rubber gloves, rubber boots, coveralls,

goggles and a face mask (respiratory protection with high-efficiency particulate air (HEPA) filter, if ventilation is reasonable. If the area is confined, a full face air-supplied respirator should be used.

3. DISPOSE of used gloves and other equipment in the same way as you dispose of infectious waste.

4. SOAK dead mice, nests, droppings thoroughly with a diluted (1 part bleach in 9 parts water) household bleach before clean-up.

5. SCOOP UP the material instead of vacuuming or sweeping. 6. DISPOSE OF contaminated material in sealed heavy duty plastic bags. 7. CONTACT your local environmental or municipal authorities about approved disposal

methods. 8. WASH hands thoroughly with soap and water after removing gloves. 9. COVER all broken skin that may be exposed to rodent contamination.
