Take the Health Assessment on myuhc.com. You and your covered spouse will each receive a $75 reward¹. The deadline to take the Health Assessment to earn the ireward is December 31, 2010.

Individuals can earn rewards for specific activities once every 365 days. In other words, an individual is eligible to earn a reward for completing the Health Assessment 365 days from the last time they earned a Health Assessment incentive reward.
