Alberta and Canada have lifetime Master Student Financial Assistance Agreements (MSFAAs) that will cover you for all of the time you are in full-time post-secondary studies. Generally, you only have to provide these agreements once. You will receive a notification email/letter asking you to print, complete and sign two financial assistance agreements: 1. Step 1: Review your MSFAA Agreements

Check Your Personal Information Ensure the personal information on your Alberta and Canada MSFAA is correct. Follow these instructions if you need to correct your personal information or update your contacts:

1. If there is incorrect personal information on your MSFAAs – do not adjust directly on your MSFAA agreements. You must contact the Alberta Student Aid Service Centre and request that you receive 2 new MSFAA agreements. The personal information you may need to correct could include:

• Legal name change to your First/Last Name • Date of Birth (DOB) • Social Insurance Number (SIN)

Note! You will be asked to provide a proof of name change, or verify your DOB/SIN to Alberta Student Aid.

Step 2: Provide Your Canadian Banking Information

Canada MSFAA agreements.

Step 3: Sign and Date Your Agreements

You must provide an original ink signature on both of your Alberta MSFAA and Canada MSFAA agreements. Note! You must also sign and date your Alberta MSFAA agreement in two places.

Step 4: Take Your MSFAA Agreements to a Designated Canada Post Outlet

Take your Alberta and Canada MSFAA agreements to a designated Canada Post outlet right away. Canada Post will verify your identification, stamp your agreements as received and return to you copies for your records. Keep these pages for your reference as they outline the terms and conditions of the loan.

You must present the following 2 pieces of documentation to prove your identity: Note! You must always complete and return both MSFAA agreements (Alberta MSFAA + Canada MSFAA) even if you only receive one type of loan. Always provide banking information for both agreements (Alberta MSFAA + Canada MSFAA) - even when you are mailing your MSFAAs separately. Can’t Get to a Designated Canada Post Outlet

You can mail page 1 of both your MSFAA agreements (Alberta MSFAA + Canada MSFAA) directly to the Alberta Student Aid Service Centre with clearly readable photocopies of your 2 pieces of documentation to prove your identity (government-issued photo ID AND social insurance number (SIN) card/letter. Do not send originals! Ensure you either attach 2 VOID cheques or fill in the banking information section on each of your MSFAA agreements.

Your MSFAAs - Requirements Check List

Personal Information appearing on Alberta and Canada MSFAA agreements Surname and given names: Your name must be written exactly as it appears on your two pieces of

identification.

Date of birth / Social Insurance Number (SIN): The specific year, month and day of your birth and your SIN number must be written exactly as appears on your two pieces of identification.

Signature in ink: You must sign your usual signature on both of your Alberta and Canada MSFAA agreements.

• Do not authorize direct deposit of your funding into another person’s account. You can only use your own personal bank account to receive direct deposit funds.

• Submit 2 VOID cheques – one for each of your MSFAAs, or fill in your banking information on each separate Alberta/Canada MSFAA.

• The name on your VOID cheque must exactly match your surname and given name as appearing on your two pieces of ID PLUS as shown on both of your Alberta/Canada MSFAAs.

• Ensure both MSFAAs are signed – even when only receiving one type of funding or when submitting your agreements via separate mail packages.

Note: Alberta MSFAA agreement requires two signatures:

• Be sure to submit 2 clearly readable copies of each required type of ID for both MSFAA agreements! i.e. one copy of your Proof of SIN PLUS a copy of your Photo ID for Alberta MSFFA and a second copy of your Proof of SIN PLUS a copy of Photo ID for Canada MSFAA. Note: This is particularly important when submitting your MSFAA agreements in separate envelopes.

• Do NOT send your original ID, send photocopies only!

• The name on your VOID cheque must exactly match your surname and given name appearing on your two pieces of ID, PLUS as appearing on both of your Alberta and Canada MSFAA agreements.

• Faxed or Electronic Upload Documents are NOT accepted.

Part B Terms and Conditions1. This Master Student Financial Assistance Agreement –

Alberta (MSFAA-Alberta) is between the applicant for financial assistance, as identified in Par A (“Student”) and Her Majesty the Queen in right of Alberta as represented by the Minister of Advanced Education (“Minister” or “Advanced Education”), made pursuant to the Student Financial Assistance Act (Alberta).In consideration of the Minister providing an Alberta Student Loan under this MSFAA-Alberta, and by signing Part D below, the Student agrees to the Terms and Conditions of this loan agreement.

This MSFAA-Alberta is comprised of: Part A: Your Information;Part B: Terms and Conditions; Part C: Electronic Funds Transfer; Part D: Your Acknowledgement and Signature; Part E: Additional Terms and Conditions.

2. Until such time as the Alberta Student Loan, together with interest and any other amounts to be paid by the Student to the Minister are paid in full, the Student agrees:

a. that this Alberta Student Loan, together with any and all funds or other financial assistance received by the Student from the Minister pursuant to the Alberta Student Finance Acts shall be subject to, governed by and form part of this

MSFAA-Alberta, as amended from time to time, entered into between the Student and the Minister;

b. to use the Alberta Student Loan exclusively for the purposes of obtaining Necessities;

c. that the Minister may pay a portion or all of the Alberta Student Loan directly to the educational institution listed on the Student’s Application for payment of the Student’s tuition and any mandatory fees;

d. to provide notice of any changes to the Student’s name, address, marital or common law partner status, financial circumstances, academic status or study period, any change in the address or financial circumstances of the Student’s spouse/partner, or to any other information contained in the Application, in accordance with the instructions relating to changes of information found on the Alberta Student Aid website, studentaid.alberta.ca;

e. to provide all information requested by the Minister which, in the Minister’s sole discretion, is required to verify any statements made in this MSFAA-Alberta;

f. to abide by all applicable provisions of the Alberta Student Finance Acts. If any of the terms of this MSFAA-Alberta conflict with any of the Alberta Student Finance Acts, then

the provisions of the applicable Alberta Student Finance Act will apply;

g. to repay the full amount of the Alberta Student Loan, together with interest and any other amounts to be paid by the Student to the Minister, in accordance with the provisions of this MSFAA-Alberta; and

h. that this MSFAA-Alberta shall revoke and replace all previous agreements that the Student may have entered into with the Minister pursuant to any of the Alberta Student Finance Acts.

3. The Student declares that:

a. all information provided in this MSFAA-Alberta and in all other forms and communications submitted by the Student to the Minister is true and complete and the Student understands that the information is subject to review and assessment by the Minister in accordance with the Student Financial Assistance Act (Alberta); and

b. the Student has read, understood, and agrees to be bound by all the terms and conditions of this MSFAA-Alberta including those in Part E, which form part of this MSFAA-Alberta, and acknowledges having retained a copy of this MSFAA-Alberta.

Part C Electronic Funds Transfer

Part D Your Acknowledgement, Consent and Signature - You must sign in both locations.

For the purpose of verifying the data provided in the Application and the Master Student Financial Assistance Agreement – Alberta, I hereby consent to the release, by the Canada Revenue Agency to an official of Alberta Advanced Education, of information from my income tax returns, and, if applicable, other required taxpayer information about me, whether supplied by me or by a third party. The information is necessary for and will be used solely for the purpose of determining and verifying my eligibility and entitlement for the student financial assistance programs under the Canada Student Loans Act, the Canada Student Financial Assistance Act, and the Student Financial Assistance Act (Alberta). The information will not be disclosed to any other person or organization without my approval. This authorization is valid for the taxation year prior to the year of signature of this consent, the year of signature of this consent and any other subsequent taxation year for which assistance is requested.

This consent may be revoked, in writing, by contacting Director, Student Services, Alberta Student Aid, Box 28000, Station Main, Edmonton AB T5J 4R4.

The approved amount of any Alberta Student Loan disbursed under this MSFAA-Alberta will be electronically deposited into your Canadian bank account that you currently hold or jointly hold, as entered below. Electronic withdrawals may also be made from this bank account when payment is triggered, as per section 16, subject to your right of revocation, as per section 22. If you fail to provide your correct bank account information, disbursement of your Alberta Student Loan will be delayed and may not proceed.

1. Agreement: This Master Student Financial Assistance Agreement is between you (“you” or “your”), as identified in Part A, and Her Majesty the Queenin Right of Canada, as represented by the Minister of Employment and Social Development (“Canada”), made pursuant to the CSFAA and called theMaster Student Financial Assistance Agreement for Canada (“MSFAA-Canada”).

This MSFAA-Canada is comprised of: Part A: Your Information; Part B: Terms and Conditions; Part C: Electronic Funds Transfer; Part D: YourAcknowledgement and Signature; Part E: Definitions; Part F: Additional Terms and Conditions.

2. Agreement to Repay: You promise to pay your total Outstanding Loan Balance in accordance with the Terms and Conditions of this MSFAA-Canada.

3. Certification You certify that all information provided in your application(s) for Financial Assistance and on this MSFAA-Canada is true and completeto the best of your knowledge.

4. Authorization: Where required by law, you authorize Canada to collect, use and disclose information related to any of your Canada Student Loan(s)or Student Grant(s), as applicable, (i) for the purposes of carrying out the administration and enforcement of the CSFAA or CSLA, or (ii) in accordancewith sections F.10(c) and F.10(d) of this MSFAA-Canada.

5. Ratification of erms and Conditions: At any time, Canada may amend the Terms and Conditions of the MSFAA-Canada. You should reviewthe Terms and Conditions at: CanLearn.ca/MSFAA upon each application for Financial Assistance. You acknowledge that your acceptance of anydisbursement made under this MSFAA-Canada will ratify your acceptance of any revised Terms and Conditions.
