Tujuan
· Studi ini mengukur fraksi Hb pada pasien hipokromik mikrositik untuk mendeteksi thalassemia dan Hb varian. CE dibandingkan dengan elektroforesis selulosa asetat (CA) untuk menggantikan CA dengan CE di laboratorium klinis.

Elektroforesis Kapiler (CE )
· CE menggunakan sistem Minicap (Sebia, Norcross, Prancis) menggunakan prinsip CE dalam larutan bebas
· Berat molekul dipisahkan dengan mobilitas elektroforesis dalam buffer basa dengan pH tertentu.
· Pemisahan juga terjadi menurut pH elektrolit dan aliran elektro-osmotik.
· Sistem ini memungkinkan analisis multipel dan simultan. Electropherograms diekspresikan dengan zona dibagi dari Z1 sampai Z15.
Analisis statistik
· Perhitungan statistik dilakukan dengan MedCalc 11.3.0.0 (Med Calc Software Company, Mariake, Belgia).
· Student t-test untuk membandingkan parameter RBC dan fraksi Hb antara kelompok kontrol dan hipokromik mikrositik.
· Perbandingan metode dengan Student-t test dan regresi linier. Nilai P <0,05 dianggap signifikan secara statistik.
HASIL
1. Pemisahan hemoglobin normal dan varian oleh CE· Dilakukan kontrol terhadap hb A,F,S,C (Gbr. 1A). · Hemoglobin utama, termasuk A, F, S, dan C dan Hb
normal dipisahkan dengan baik. · Dalam sistem Minicap, pola fraksi keluar dari kanan ke
kiri dengan urutan sebagai berikut: dari HbA2 sampai HbA (Gambar 1B).
· Fraksi terglikasi tidak diseparasi dari HbA pada CE.

3. CBC dan fraksi Hb pada kontrol normal dan kelompok hipokromik mikrositik
· Pada kelompok hipokromik mikrositik, Hb, Hct, MCV, MCH, dan MCHC menunjukkan perbedaan yang nyata dibandingkan dengan kelompok kontrol (P <0,0001) (Tabel 1).
· Tidak ada perbedaan signifikan untuk kuantifikasi fraksi Hb antara kedua kelompok (P> 0,05) (Tabel 2).
4. Perbandingan dua metode· Ada korelasi yang baik untuk pengukuran HbA (r = 0,9370, P
<0,0001), HbA2 (r = 0,8973, P <0,0001), dan HbF (r = 0,8010, p = 0,0304) antara kedua metode (Gambar 2) .
· Kedua teknik menunjukkan tingkat yang sama untuk HbA (97,4 ± 0,7% oleh CE; 97,4 ± 1,1% oleh CA) dan HbA2 (2,5 ± 0,6% oleh CE; 2,5 ± 0,7% oleh CA)dan juga terhadap HbF (0,4 ± 1,1% oleh CE; 0,5 ± 2,7% oleh CA).
· Tidak ada perbedaan signifikan secara statistik untuk kuantifikasi Hb (P> 0,05) (Tabel 3).
Kesesuaian keseluruhan dari 2 metode untuk fraksi Hb adalah 89,5% (128/143) (Tabel 4).

Diagnosis β-thalassemia

· Penelitian sebelumnya menunjukkan CE memisahkan HbA2 dari HbE, HBc, dan HbS.
· Metode ini : efisiensi tinggi (beberapa sampel dapat dijalankan secara paralel), akurasi tinggi, dan otomatisasi penuh.
· Pada studi ini, terdapat hubungan yang baik untuk pengukuran HbA, HbF, dan HbA2 antara CE dan CA.
· Persentase kesepakatan fraksi Hb,CE dan CA : 89,5%. · CA tidak mengidentifikasi beberapa penurunan HbA2 dan
kenaikan HbF. Ini menunjukkan bahwa CE lebih sensitif dari CA untuk mendeteksi fraksi Hb.
· CA hanya mendeteksi 25% varian Hb.· Beberapa studi menunjukkan presisi yang rendah untuk
kuantifikasi HbA2 berdasarkan elektroforesis.· Fitur menonjol metode CE kemampuan memisahkan

· Frekuensi gen β-thalassemia di Korea sekitar ≤ 0,1% . · Pada studi ini, total fraksi hemoglobin abnormal : 1,4%
dideteksi oleh CE dan CA. · Prevalensi hemoglobinopathies sangat rendah di Korea,
sehingga elektroforesis Hb belum diperiksa rutin.· CE disarankan sebagai alat skrining untuk gangguan
hemoglobin pada populasi Korea.
Kesimpulan
· CE sebanding dengan CA dalam hal pengukuran fraksi Hb, dan cocok untuk skrining.

jumlah rata-rata hemoglobin dalam eritrosit· MCHC: adalah perhitungan rata-rata konsentrasi
hemoglobin di dalam eritrosit. MCHC menurun (hipokromia) dijumpai pada kondisi di mana hemoglobin abnormal diencerkan di dalam eritrosit, seperti pada anemia dan kekurangan zat besi dalam talasemia. Electrophoresis Principle.
►Separation of haemoglobins with electrophoresis at pH 8.4 (alkaline) and pH 6.2 (acid).
►Scanning allows quantification of the hemoglobin present, bands are seen by staining.
►At alkaline pH Hb C, E, A2 and O migrate together to form a single band, Hb S, D and G also co migrate.
Electrophoresis Principle (2).
►At acid pH Hb C separates from E and O and Hb S separates from D and G.
►Hb E and O cannot be separated by electrophoresis neither can Hb D and G.
HPLC Disadvantages (2).
► Capillary zone Electrophoretic method can be used to quantify Hb A2 in the presence of Hb S by eliminating interference from these adducts.
► Interference can also be eliminated by the use of micro anion-exchange column methodology.
► Integration errors can result in false decreases in the values obtained, although this can be minimized by applying known corrections.
Capillary electrophoresis CE· Capillary electrophoresis (CE) was developed from
combining several features of different methods including the principle of gel electrophoresis, the fused silica capillary of gas chromatography (GC), and the highly sensitive detectors of high-performance liquid chromatography (HPLC).
CESeparation in CE is based on different mobility of analytes under an electric field, which occurs in a capillary filled with buffer. Unlike other media (e.g., paper, agarose, and polyacrylamide gel), electrophoresis in a capillary can use a high voltage (up to 800 V/cm) due to the physical properties of fused silica capillaries. A large surface area to volume ratio of a capillary provides effective heat dissipation. The small dimension of a capillary requires small amounts of samples and buffer and the automation of CE requires less time and labor.
· In 1989, Karger organized the first International Symposium on High Performance Capillary Electrophoresis (12) and, in the same year, the first commercial CE instrument was available. Since then, the number of publications on CE have been rapidly increasing due to its versatility, simplicity, and high efficiency.
The principle of CEThe principle of CE is based on the different
migration of solutes in an electric field, and electrophoresis is performed in narrow-bore capillaries filled with electrolyte. The mobility of
analytes depends upon their sizes, charges, and degree of ionization, viscosity, temperature, and dielectric constant of the background electrolyte (BGE). Upon application of voltage, analytes
are driven by two forces, the electrophoretic migration and the electro-osmotic flow (EOF).

· Several acquired conditions are associated with modest elevations of HbF. They include pregnancy, recovery from marrow hypoplasia, aplastic anemia, leukemia, thyrotoxicosis, hepatoma, and juvenile chronic myeloid leukemia.4 The latter condition is exceptional in that it seems to reflect a genuine
reversion to fetal erythropoiesis.5 The remainder seem to be examples of the transient reactivation of HbF under conditions of acute erythropoietic stress, that is, rapid expansion of the erythron.6
