C ommunity 2 0 1 3 D ear Community Members, You’ve undoubtedly heard me say how excited and blessed I am to work shoulder-to- shoulder with our employees and medical staff, as we try our very best to improve our patients’ and families’ quality of life, one patient at a time. My staff and I feel extremely confident about our ability to serve you, as is reflected by our latest Joint Commission quality rating (see article below), now hovering above 95 percent out of 100 percent. This means we have met more than 95 percent of nearly 600 best practice requirements for the last 12 years, according to the most credible healthcare auditor in our business. This achievement happened solely because everyone who works and practices here wants every member of our Northwest Connecticut community to have no doubt in our ability to care for you in Torrington, better than you might be cared for elsewhere, for those services which we offer. Very recently, I was faced with a decision about where I should undergo major surgery, something that was unexpected and very important to deal with, for the sake of my family and my quality of life, and the lives of others who to some measure depend on me. I know this sounds a bit “over-the-top,” but I realized at that decisive moment, that my staff had accomplished such terrific things over time that I was actually looking forward to having my own hospital family get me through this major surgical event. As you might guess, I was not able to play “undercover boss,” chiefly because our employees and medical staff have over time developed personal and professional interrelationships that now define us as a family of heartful and knowledgeable caregivers. As it turned out, my surgery and recovery went extremely well! Looking back at my inpatient experience, I can only feel a debt of gratitude to our employees, physicians and board members who together have created a truly enviable environment in which patients can be accu- rately diagnosed and can begin their healing process, in a place filled with love and talent. I have been leading hospital organizations for over 33 years. During that time, my con- nection with the CHH family has been the most unique and outstanding experience , wherein a group of dedicated people have made an unwavering commitment to work successfully with one another in “bringing hope and comfort to our ill and injured”! The accreditation award recognizes CHH’s dedication to continuous compliance with 600 Joint Commission’s patient safety standards, including infection prevention and control, hospital and medical staff leadership, and medication safety management. CHH under- went a rigorous unannounced on-site survey in August 2013 when a team of Joint Commission expert surveyors evaluated CHH for compliance with standards of care specific to the needs of patients. “Although we are extremely pleased with our accreditation results, we are doubly pleased in knowing that our patients and their families can truly depend on Charlotte Hungerford Hospital for the services we provide,” said Daniel J. The Community Outreach Team at CharlotteHungerford Hospital is dedicated to ensuring thatpeople young and old residing in northwestConnecticut make good nutrition and hand hygienea healthy habit.

In 2013, CHH created and delivered a series ofpresentations and outreach activities at local fairs,school events, and day care centers designed toeducate and entertain the public and promotepreventative health and active lifestyles.

Addressing the issue of childhood obesity is oneof the top priorities identified by the staff andcaregivers at Charlotte through its communityassessment surveys. Using educational materials,games and activities from myplate.gov and otherresources on good nutrition, the team promotes itsmessage by challenging and quizzing children andadults on what foods are healthy and what choicesshould be made when they are dining out or preparing food at home.

Good hand hygiene is another very importantlesson taught by the team. Children have a chance todiscuss what germs are and how they affect a person’shealth. Pretend germs are viewed under a speciallight to show children how they hide and can be

removed using hand sanitizer. The team has been very pleased with the

response, especially from the youngest partici-pants. Team members agree that the area’s localschools and day care centers have done a great

job educating kids about the importance of handwashing. As flu season approaches, the team’s goalis to increase awareness of how this simple proce-dure a few times a day can keep flu away fromfriends and family.

CHH and over 20 local community organizations, local non-profits, area agencies andschools formed a group called “Fit Together” andadopted a mission “to build the healthiest kids,families and communities through sustainablestrategies that foster healthy eating and activeliving.” The hospital’s recent outreach and publicevents have incorporated this mission, and rein-forced a united effort among the region’s leadersand caregivers to build healthier communities.

“An essential component of a successfulcommunity-based system of services is the pro-motion of health through good nutrition andphysical activity,” said Tim LeBouthillier,Director of CHH Community Relations andDevelopment. “We want to do all we can tomake sure our community members start off onthe right foot when it comes to health, nutritionand exercise.”

CHH CREATES PINWHEELS FOR PREVENTION ART GALLERY

Child Abuse Investigation Team of Northwest Connecticut (CAIT NW) recognized ChildAbuse Prevention Month with the creation of the first annual Pinwheels for Prevention Art Galleryat the Northwestern Connecticut Community College.

“We provide the community with information on how we can work together to keep kids safe whilehighlighting the spirit of our children with their artwork,” said Bethany Ferreira, CAIT NW Coordinator.

CHH FORMS NEW OSTOMY SUPPORT GROUP

CHH founded a new Ostomy Support Group, the first group of its kind in northwestConnecticut, to provide unify and strengthen its members for the benefit of people who have, orwill have intestinal or urinary diversions and their caregivers.

“I am very excited to offer this important resource to the community. The support group will allow individuals with an ostomy or continent diversion, their families and caregivers the opportunity to share theirexperiences and advice on related health and wellness issues,” said Suzanne Fox, RN, WOCN, the hospital’sWound, Ostomy and Continence Nurse.

CHH offered free admission to its Smoking Cessation Programs to assist residents looking to kick the habit learn how to stop smoking and start building a healthier lifestylethrough education, relaxation techniques, and methods of preventing weight gain.

“The program features sessions led by a trained, certified clinic leader who understands the motivationsand rationalizations of smoking, and uses a positive behavior change approach that teaches people how tobecome a permanent non-smoker,” said Sandy Markus, Program Facilitator.

CHH OFFERS NEW LUNG CANCER SCREENING SERVICECHH Early Detection Program’s offered a new Lung Cancer Screening Service utilizing a fast,

painless, low cost, low dose CT scan to assist current or former heavy smokers 55 or older that isreviewed by a Lung Cancer Navigation Team of radiologists, pulmonologists, oncologists and anurse navigator.

“You can’t put a price tag on the peace of mind that can come with a lung cancer screening. This testgives you access to a comprehensive program with the treatment and resources you may need to help you lead ahealthier life,” said Program Director Leslie Handelman, R.N.

CHH DEBUTS CHILD FIRST FAMILY CARE MODEL

Charlotte Hungerford Hospital’s Center for Youth and Families introduced “Child First,” aninnovative, home-based system of care designed to decrease serious emotional disturbance andabuse and neglect among the region’s most vulnerable young children.

“Two teams, each consisting of a developmental clinician and a care coordinator, work with pediatri-cians, teachers and other community providers to identify, assess and intervene where necessary to make thisprogram possible. It’s a team effort,” said Joan Neveski, Director of the Center For Youth & Families.

CHH BREAST HEALTH CENTER RECEIVES FIRST KOMEN GRANTEE AWARDRecognizing the efforts of the organization’s top sponsors, fundraisers and volunteers from

communities across Connecticut, Komen for the Cure CT choose the CHH Pink Rose Program asits “Grantee of the Year” for 2013.

“It is the first time Komen for the Cure, CT, has officially recognized an outstanding program, and it isan honor to be the first recipient of the award. We are very proud of all the work our program has done inthe community to educate women about the importance of breast cancer early detection,” said Project DirectorLeslie Handelman, R.N.

CHH CREATES HEALTHCARE COALITION OF MEDICAL PROVIDERSCHH formed the Greater Charlotte Hungerford Hospital Healthcare Coalition, a group that

brings together the hospital and a range of Northwest Connecticut healthcare organizations andpartners to develop and maintain critical emergency preparedness plans and strategies.

“The hospital and area healthcare stakeholders and agencies have been working together to improvecommunications and planning during disasters for several years. This new partnership formalizes thisrelationship and commitment ensuring we are all ready when needed most,” said Paul Rabeuf, CHH EMSand Emergency Management Coordinator.

CHH PARTNERSWITH LOCAL AGENCIES TO OFFER CPR COURSES

Charlotte Hungerford Hospital along with Campion Ambulance Service, the Torrington FireDepartment, Valerie Manor Health Care Center and the Elks Lodge conducted free CPR classeson Torrington’s Community Heart Saver CPR Day providing instruction on CPR and the use ofthe Automatic External Defibrillator (AED).

“Less than one-third of sudden cardiac arrest victims receive immediate CPR from bystanders. Thiscourse equips all people and communities to act in an emergency and ultimately save lives through early med-ical interventions,” said Tim LeBouthillier, Director of CHH Community Relations.

CHH CARDIOLOGY SERVICES RECEIVES ECHOCARDIOGRAPHY ACCREDITATION

CHH Cardiology Services and CHH Cardiovascular Medicine were granted a three-year termof accreditation in Echocardiography in the area of Adult Transthoracic and Adult Transesophagealby the Intersocietal Accreditation Commission (IAC).

their newest treatment option for cancer patients that works to significantly offers faster, more precisecancer treatments by reducing the time required for radiotherapy sessions to five minutes or less.

“It provides less treatment time and more quality time for patients,” said Elizabeth Whalen, M.D., CHHRadiation Oncologist. “We are committed to offering our patients a wider range of treatment options close to home.”

Our Volunteers And ChaplainsThe Charlotte Hungerford Hospital Volunteer Program

provides essential, supplementary and complementary services topatients, families, visitors and staff. The program has over 160volunteers in more than 30 non-clinical and clinical areas,including at the Information Desk, Gift Shop, Financial Servicesand as Patient Representatives. Our volunteers range in age from16 to 94 and log over 18,000 hours every year.

We welcome volunteers from all backgrounds, as eachvolunteer can make unique contributions to the hospital. TheVolunteer Program at Charlotte Hungerford Hospital hasapproximately 130 traditional volunteers and over 30 juniorvolunteers that make a profound and positive difference in thehospital’s daily operations.

Charlotte Hungerford Hospital Auxiliary

2013 proved to be a very productive year for theCharlotte Hungerford Hospital Auxiliary. Twenty-threeboard members and many more sponsors gave theirsupport and time by raising money through the operationof the CHH Gift Shop, membership dues, special vendorsales, the annual fashion show, the Artist of the MonthProgram and a holiday season greeting promotion.

The Auxiliary gave its fourth installment of its$50,000 pledge to the hospital’s information technologyinitiative. New chairs were purchased for the hospitalcafeteria and an infant warmer with a resuscitation systemfor Maternity Services was also delivered for immediateuse. In addition, the Auxiliary awarded 12 $1,000scholarships to students entering the medical field. Oneof the scholarships honored Dr. Alfred Fabro and anotherwas given in memory of Dr. Joseph Curi.

The 29th annual fashion show was a huge successwith 180 women in attendance. Fashions were providedby Dress Barn of Torrington. We are grateful to manyof our local and regional businesses that donate sogenerously to our fashion show and raffle.
