Introduction

This is the annual review report for the October 2017/2018 – October 2021/2022 Citrus Community Health Improvement Plan (CHIP). The report is a progress review of the strategies that were developed and the activities that have been implemented or otherwise changed. While the CHIP is a community driven and collectively owned health improvement plan, the Florida Department of Health in Citrus County is charged with providing administrative support, tracking and collecting data, and preparing the annual review report.

Overview of the Community Health Improvement Plan (CHIP)

The Florida Department of Health in Citrus County (DOH-Citrus), with the help of WellFlorida, Inc. prepared a new Community Health Assessment (CHA) in 2015. The document was received in December 2015, with revisions by the contractor submitted and approved by DOH-Citrus in January 2016.

On February 24, 2016, DOH-Citrus held the initial Community Health Improvement Plan (CHIP) meeting with interested community partners and members of the public. The purpose of the meeting was to introduce them to the CHA process and findings. In addition, the development of the CHIP was explained and that it would become a blueprint for use in identifying resources and opportunities to improve the health of our community.

The CHIP Partnership identified four (4) subcommittees to be created to address the findings of the CHA. The CHIP Partnership continued to meet as a group on a monthly basis, while the subcommittees met on their own to develop their goals, strategies, and objectives. In addition, the action steps, resources, responsibilities and timelines were developed by the subcommittees. Another subcommittee was added to the Partnership in October 2016 to include the Florida Healthy Babies programming to address infant mortality, bringing the total subcommittees to five (5).

The DOH-Citrus Community Health Improvement Plan (CHIP) was approved by the CHIP Partnership on October 13, 2017. On January 4, 2018, the CHIP Partnership noted two (2) glitches in the Plan and agreed to revise them at that time; 1) The cover page had 2 different dates on it, and 2) A county division name was incorrectly identified in the Plan.

Currently, the CHIP Partnership continues to meet every other month to oversee the projects identified in the Strategic Issue Areas. Annually, the CHIP Partnership reviews the CHIP, identify new information in the CHIP and CHA, changes to the strategies and other pertinent business to address the health needs of our community. Changes/updates to the Plan are to be done during the annual review meeting with subsequent update(s) to the document(s) as needed.

The CHIP Partnership Subcommittees identified the five (5) strategic issue areas and goals:

Summary of CHIP Annual Review Meeting

The CHIP Partnership met on February 7, 2019 to review the status of our Plan and update of the CHIP Charter. The CHIP goals were discussed along with the progress of each. To bring the Partnership up to date on the status of all the goals and strategies, they decided to review a draft copy of the CHIP Annual Report on March 7, 2019 to help guide the development of an updated Plan.

Strategic Issue Area #1: Access to Care

Socioeconomic factors continue to be a leading factor in Access to Care issues. Poverty levels, income levels and unemployment rates all remain higher than the State levels. Citrus County remains in the health professional shortage and the medically underserved shortage areas.

Goal 1: Provide all residents in Citrus County with quality, comprehensive, and affordable health care.

Strategy 1: Increase the capacity of Citrus County to provide healthcare to uninsured and underinsured adults.

Why this is important to our community: The We Care program would provide volunteer physician and diagnostic services to our uninsured and underinsured citizens. While there have been some positive efforts to move this program forward during this reporting period, there are continued struggles with the local diagnostics needed to provide services to qualified clients.

The hiring of staff to lead the We Care Program on board by December 30, 2017 to lead the program and develop partnerships with local diagnostic facilities and physicians to perform these tests. Skin cancer Screenings have been held at the FDOH-Citrus facility in 2018. 104 screenings completed; additional free screenings for the 100 persons that were unable to be seen during hours of operation were held by the volunteer physicians at their facility(ies). In addition, 21 Blood Pressure screenings were done at these events. Langley Health Services Medical and Dental vans currently visit Citrus 1 - 3 times/quarter.

Develop a partnership with the Citrus County Medical Society. Initial Meeting March 28, 2017. Follow up meetings to take place as needed. Identification of benefits to working together to meet access to care needs; said benefits will be used to determine our benchmarks & goals. Written report of success to CHIP Partnership and determination of goals and benchmarks to be prepared.

DOH-Citrus Health Officer, Medical Director and Director of Nursing (DON) have attended 3 meetings. Health Officer and DON asked to be part of a presentation to the Society by a free clinic in a neighboring county; Volunteer Provider Program was presented. . Goal 2: Public Education and Outreach

3.Identify and coordinate health services that can be done through ministry at local churches to raise awareness of risks for heart disease and strokes; education and prevention. Heart Health Plus/Million Heart 100 Congregations initiative. By June 30, 2017 have completed a local community event at a minimum of 2 churches; coordinated with local church; distributed health information using health equity data; developed 3 community organizations/partners. By December 30, 2017 have enlisted at least 2 additional churches to participate in the program through June 30, 2018. The Medical Society is exploring a free medical clinic modeled after successful clinic in neighboring county. Working with the Medical Society to open a free mental health clinic at DOH-Citrus.

Strategic Issue Area #2: Obesity

A leading cause of chronic disease is obesity and/or being overweight both for adults and children. DOH-Citrus continues to educate our citizens on healthy eating habits

This is important to our community as the CHA identifies obesity as a major problem in Citrus County – 62% of our adults are either overweight or obese.

The 2018 totals exceeded the 2017 goal of 10% increase. The combined total approximate number of adults & children reached during these outreach events was 3,050. Strategic Issue Area #3: Access To Healthy Foods

Access to healthy foods is directly tied to obesity and is of continued concern. The USDA Low Income & Low Access Layers have not been updated to date from 2015. This Subcommittee did not submit any additional information towards resolution of this strategic issue. Due to the similarities, it is the intention to combine this issue area with the Obesity subcommittee going forward.

Goal 1: Improve access to healthy foods among low income residents by addressing the key modifiable barriers identified through the Community Health Assessment.

Strategy 1: Work with local food pantries and farmers to increase the access, availability, and preparation of healthy foods.

Revisions

The DOH-Citrus Community Health Improvement Partnership approved the final CHIP Plan on October 13, 2017. At their January 4, 2018 meeting, it was pointed out that a correction needed to be made to one of the community partnership division titles, and that the plan as approved, carried two different dates on the cover. The CHIP Partnership made a motion and approved to correct these 2 sections as noted. The motion carried unanimously. There have been no other revisions to the strategic issue areas or other data during this reporting period.

Reach out to the citizens of Citrus County through community events and the educational system.

DOH-Citrus has attended numerous outreach events to promote healthier eating. During 2018, the combined total approximate number of adults & children reached during these outreach events was 3,050 and included health fairs, day cares and other community events.

How it’s important for our community: Healthy eating habits will aid in the reduction of chronic diseases through decreasing the overall number of overweight and obese citizens. While there have been no new data to determine a reduction in these numbers at this time, it is believed that teaching these healthy habits to young children will result in their bringing the lessons home to the adults in the family.

Increase the percentage of adults in Citrus County who have a regular source of health care.

DOH-Citrus has developed a relationship with the Citrus County Medical Society and they continue to work towards the goal(s) of Access to Care for our citizens that are underinsured and/or uninsured.

How it’s important for our community: Through an overall coordinated effort of many partners, health care for the citizens in need will be provided. This would reduce the number of people going to the Emergency Department for non-emergency issues, and providing diagnostic testing, which is often unavailable, resulting in a healthier Citrus County.

Conclusion

The Community Health Improvement Plan (CHIP) is a map for the ongoing health improvement process for the local public health system by providing a framework for the identified strategic issue areas. This is a living document in that it is intended to evolve with completion of strategies to achieve the ultimate goals and to reflect new information as it becomes available. Progress is evaluated based on quarterly CHIP implementation reports and discussions with community partners to continuously improve the health of our community. Annual reports, such as this one, will be submitted in March every year.

In 2018, DOH-Citrus contracted with WellFlorida, Inc. to prepare an updated Community Health Assessment. The CHIPartnership plans to review the existing CHIP together with the new data and develop a new Community Health Improvement Plan in 2019.

In 2018, the Citrus County Board of County Commissioners submitted a formal request to have the county’s mental health and substance abuse provider services changed. The contract manager, Lutheran Services put the contract out for bid and LifeStream Behavioral Center was named as the Citrus County Mental Health provider with the transition date of October 1, 2018. However, all parties agreed that it would take time to fully make the necessary changes.

Open Floor Discussion • Renea Teastero Tampa Drug Summit – February 18-20, 2019, cover a wide variety of

topics. Will send out a link for the summit.▪ Drug education and prevention training provided by national

experts and local leaders. Network with professionals from acrossthe southeastern region of the United States dealing with theimpacts of substance abuse. 16 CEUs for prevention certification,substance abuse, mental health & marriage counseling will beprovided over the course of the three-day summit. Certificates willbe handed out at the end of each day.

o Event - “The Impact Game” in coordination with National Prevention Week(third week in May) – high school students to develop teams and competefor cups – this year’s host is Citrus High School – focus on prevention –kid zone – vendors – family event – Friday 5/10 5:30 to 9:30.

o PACE Center for Girls - Open House, Monday February 11th at 4:00.

• Chuck Higginso Citrus County Zero Discrimination Day Picnic – A family friendly event –

Saturday, March 2nd at Ft. Cooper State Park from 11am to 3pm. Willemail a flyer for the event.

o Democratic Equality Caucus of Citrus County social media is taking off.

• Blaire Peterseno Circuit 5 Substance Exposed Newborn Taskforce – April 12, 2019 9am to

4pm – Supporting Substance Exposed Newborn Communities –Presentation: Memorial Health System-Maternal Addiction Team – join theTaskforce to share and learn best practices in the Circuit 5 counties for thetreatment and prevention of the addicted pregnant patient and hernewborn – Wildwood Community Center

• Patrick Boydo Plugged Inn – a free speaker series open to members of the healthcare

community, fire/rescue, EMS and enforcement – focus on healthcare anddisaster preparedness or special needs population. February 14th –Dementia, March 14th Oxygen, April 11th Challenges of Sheltering, May 9th

HIPAA, June 13th Weather, July 11th Leadership CEUs offered. Will emaila flyer on the event.

• Nicole Kellyo GNAHEC is rolling out a new Tobacco Cessation Behavioral Health

Curriculum “Journey’s”. This 6-week program will be specifically forbehavioral health sites and private classes for the site (in-patient or out-patient). GNAHEC will hopefully partnering with The Centers andLifeStream to offer this program on-site. Participating clients will receivemore Quit Sessions and Nicotine Replacement Therapy (NRT) thanGNAHEC other cessation program for the general population.

o GNAHEC recently received state funds for Opioid prevention. The grantwill be offering CE and CME trainings throughout our 5 County servicearea. Community presentations will also be offered.

• Rebecca Martino YMCA will incorporate Fitness in Citrus into their programs next year -

unable to hold the program in February – will be open to the community.

• Carmen Hernandezo January was Human Trafficking Awareness Month – 2/7 is Shine a Light

on Slavery – posters on the back table for awareness of Human Trafficking– 5 reported cases for Citrus County and 7 reported cases for HernandoCounty – working with the YMCA for Safety Awareness

a scorecard.o All-encompassing DOH documento Will be able to share document via One Drive

Tito Rubio reported DOH is partnering with The Centers and Lifestream

Behavioral Center for addicted pregnant women andtheir care.

DOH working on a campaign “Mental Health Matters” inMay for Mental Health month.

Want to get DOH 800 number out in the community If you wish to be on the Behavioral Health

subcommittee contact Ellen – sending out a Doodle Pollfor best time to meet monthly.

o Healthy Behaviors Supported by Culturally Appropriate HealthEducation Pam Carnevale reported: Hope to be able to begin meeting again after Spring break.

o Child Health and Safety No report

o Access to Healthcare Tito Rubio reported The Medical Society is wanting to begin a free clinic in Citrus

County – going to approach the Medical Society about beingthe Lead for this subcommittee.

Suggest that they visit the “Connections” free clinic inHernando.

Rebecca Martin reported: Citrus 20/30 Committee – Carmen and Rebecca working on the

aspiration statements for “Health and Social Services” with plans to work “Health in All Policies” education into the statements.

Citrus 20/30 Committee will include focus areas being worked on in the CHIPartnership subcommittees – should subcommittee members want to also attend the Citrus 20/30 let Rebecca or Carmen know.

Rebecca and Dr. Ruben attended the WellFlorida meeting – learned that Alachua has a community health element in their COMP Plan. Jeff Feller / WellFlorida interested in addressing CHIPartnership.

Review Draft copy of Annual Report for 2018 o Draft copy of the 2018 Annual Report was sent via email for committee

members to review prior to this meeting. o The Draft 2018 Annual Report was brought up on the computer for

review at the meeting. o DOH requirement for accreditation – must be approved by the

CHIPartnership. o Will Bryant made a motion to accept the Draft Annual Report as

written, Kristi Lindke seconded, all in favor.

o The Department of Health in Citrus County now has 4 Tobacco Treatment Specialist on staff. Provide one-on-one counselling for smoking cessation. Tobacco Citation Classes – 14-15 kids per class (Court sent).

Kids share their situations. o Tobacco Free Program “Baby and Me” – help pregnant Mom’s who are

still smoking. o Nicole Kelly-Gulfcoast North AHEC provides Group Smoking Cessation

sessions and continuing education to providers. o Pam Carnevale - The School District-Student Services has reached out to

Anti-Drug Coalition and DOH-Tobacco Free Prevention to see what they can do to reach students that are vaping. 1 pod = 20+ cigarettes – smoking 1-3 pods/daily

Jim Rashley o DOH-Citrus now has a 800 number for conference calls and will be on the

next agenda for anyone wanting to call in to the meeting.
