Community and Date of Birth certificate MeeSeva Introduces ‘Community & Date of Birth Certificate’ Service to the citizen, which is under Revenue Department. The main objective of Community & Date of Birth Certificate Service is to issue Community & Date of Birth Certificate`s to the citizens who are under SC, ST and BC Castes. This certificate is useful for the Students for their Educational Purpose, as a Proof of age (Date of birth) and Community. This Certificate is also useful for the Employment Purpose for the Job Holders and etc. By having the Community & Date of Birth Certificate, the Students and Job Holders can get the benefits from the governments, if he/she is eligible. The Citizen need to approach Meeseva Franchise for applying the Community & Date of Birth Certificate. The Issuing Authority will be routed to Tahsildhar (MRO). For Some Castes, the Request will be routed to RDO or District Collector. Category Type B User Charges INR 35/- Competent Authority MRO/RDO/COLLECTOR(Based on Community) Documents Required 1. Meeseva Application Form* 2. Community Certificate Issued to the Family members 3. SSC Marks or DOB Extract or Transfer Certificate 4. 1 to 10th Study Certificates or DOB Certificates Issued by Municipality/Gram Panchayath* 5. Ration Card/EPICCard/AADHAR Card* 6. MeeSeva Introduces ‘Community & Date of Birth Certificate’ Service to the citizen, which is under Revenue Department. The main objective of Community & Date of Birth Certificate Service is to issue Community & Date of Birth Certificate`s to the citizens who are under SC, ST and BC Castes. 1) In Meeseva home screen, under List of Services, select Revenue Department services

as depicted in Figure 1.

2) Select Community and date of Birth Certificate Service in Revenue Department as depicted in Figure 2.

3) Now Community and date of Birth Certificate Screen will be displayed. Select service Type

from Drop Down as depicted in Figure 3.

4) Enter Applicant Details such as Applicant Name, Aadhar No, Father Name, Mother Name, Date of Birth, Gender, and Age as depicted in Figure 4.

5) Enter Present Postal Address, Place of Permanent Residence Address as depicted in Figure 5.

6) Enter Place of Ordinary Residence ,Place of Birth and Service Specific Details as depicted

in Figure 6

7) Collect the necessary supporting documents as indicated in the Community and date of Birth Certificate request service page scan the documents and upload into the system, as depicted in Figure 7.

Note: All the mandatory fields have to be filled in; otherwise request will not be accepted by the system.

8) Click Show Payment. Another window appears requesting to Confirm Payment as

depicted in figure 8

9) Collect the amount from the Citizen and click Confirm Payment to submit the request as depicted in figure 9.

10) On confirmation, a receipt will be generated as depicted in the following figure 10. The receipt will contain the Certificate delivery date.

Note: Ensure that you have sufficient balance with your service provider (SCA), else the system will not accept the request.

Note: - On submission of the request, Citizen Receives Message. Like your Request for Community

and date of Birth Certificate Application has been entered vide Application Number, Transaction Id

and sent to Dept.

11) On submission, the request will be sent to the work flow of the respective Department officials

for further processing.

12) Once the request is approved by Department officials, the Community and date of Birth Certificate will be dispatched through courier to the citizen address if the Delivery Type is Speed Post Local/Non Local.

� If Delivery Type Is Manual, then the citizen need to collect the Certificate from franchisee

where he/she applied for the Certificate.
