RISE Community Health Centre

RISE Community Health Centre is the newest department of Collingwood Neighbourhood House. RISE CHC provides direct medical services and health promotion activities for Renfrew-Collingwood residents, focusing on those who experience barriers to accessing health care. Community Health Centres are different from standard doctor’s offices in that they provide team-based care (nurse practitioners, physicians, community health workers, nurses, social workers, counselors and others), and work holistically to address health needs, including supporting clients with social determinants of health (food, safety, housing, social connections).

RISE has a main location at Joyce Street and Crowley Drive (open April, 2020), and a secondary site within Collingwood Neighbourhood House at 5288 Joyce Street.

For more information about RISE Community Health Centre contact us via: email: [email protected] website: cnh.bc.ca/rise Phone: 604-558-8090
