la presencia o formación de cálculos en vesícula biliar (VB)
Prevalencia variable Hay diferencias según el país y
la población estudiada. Aumenta con la edad y es mayor
en mujeres que en varones

COLELITIASIS ASINTOMATICA
SINTOMAS «cólico biliar», dura menos de 6 horas, cede
espontáneamente o con fármacos y no deja secuelas.
Frecuentemente en el hipocondrio derecho, también en mesogastrio o hipocondrio izquierdo.
Más de 1/3 se irradia a la espalda, frecuentemente a la escápula/hombro derecho.
Suele suceder a las 2-3 horas de haber comido, Vómitos, suceden al comienzo y no alivian el
cuadro.

Plenitud posprandial, intolerancia a grasas, halitosis, lengua saburral, «dispepsia biliar».
Estos inespecíficos síntomas no son justificación para tratamiento de la colelitiasis.
La litiasis de la VBP también puede ser asintomatica

Entre el 10% y 15% de la población occidental adulta presenta cálculos biliares (NIH 1992. )
Entre el 15 y el 25 % de las mujeres adultas y el 7-15 % de los varones tienen colelitiasis.
4% de los pacientes con cálculos biliares asintomáticos presentan síntomas, incluidos colecistitis, ictericia obstructiva, pancreatitis y cáncer de vesícula biliar.

La incidencia de cálculos de la vía biliar común (VBC) en el momento de la colecistectomía varía entre el 5% y 11% (3,4)
La VB "de porcelana" (calcificada) se asocia con un riesgo del 7% de Ca (5) y se recomienda la colecistectomía profiláctica (1)
los pólipos de más de 10 mm, se asocian con el cáncer de vesícula biliar (6,7,8)

La colecistectomía en ausencia de síntomas también es polémica en los diabéticos, los pacientes inmunosuprimidos y los niños.
