MR Eligibility requirements:

Significantly sub-average general intellectual functioning with the overall IQ score at least two standard deviations below the mean andSignificant limitations in adaptive functioning areas of: communication, self-care, home living, social/interpersonal skills, self-direction, use of community resources, functional academic skills, work, leisure, and health & safety.Onset must occur before the individual’s 22nd birthday.

What is Early Intervention?

EI is a collection of supports & services designed to help families identify routines & activities that help their child’s development including making these routines & activities successful for the child & family. EI works with parents and other caregivers in learning how they can help the child grow & develop.

EI Services are provided at NO COST to the family. EI is funded through federal, EPSDT (MA), state & county dollars. Children who have Medical Assistance have their ACCESS card billed for services.

EI Eligibility requirements:

Birth to age 3 years

Significant delay (25% of chronological age) in one or more of the following areas:

• Cognitive development• Physical development (including vision & hearing)• Communication• Adaptive Development• Social/Emotional Development

Or have a high probability for developmental delays based on known physical or mental conditions.

How do I find out if my child qualifies for EI Services?

Contact is made with CMSU EI Service Coordinator who schedules an intake, gathers information, and answers questions about your child’s development.

A Multidisciplinary Evaluation (MDE) team completes an evaluation which determines the child’s eligibility for EI Services.

An Individualized Family Service Plan (IFSP) is written, which contains information from the MDE & outlines types & frequency of services to be provided, where the services will occur and the supports needed to achieve the outcomes.

The services occur within the natural environments of the child.

grievances investigated and complaints resolved.

PUNS (Prioritization of Urgency of Need for Services)

A standardized method to collect waiting list information regarding

persons in PA who have developmental disabilities. It is designed to categorize persons along a continuum of urgency of need for services. A person is placed in Emergency, Critical, or Planning for Need status based upon certain criteria established by OMR.

MA Funding & Medicaid Waivers

A medicaid waiver provides Federal Financial Participation for the provision of services for individuals with mental retardation in accordance with State assurances to the Federal Health Care Financing Administration:* Consolidated Waiver * Person/Family Directed Supports Waiver.

Eligibility: *Must choose home & community services in lieu of institutional care and be financially eligible for Medical Assistance. *Services chosen must be cost effective & within the county’s allocation of Department funding. *Individual age 3 or over who require an ICF/MR level of care. *Determined to have MR as defined in current OMR literature.Services available: environmental accessibility adaptations, habilitation services (residential, day, prevocational, supported employment, homemaker/chore services), adaptive equipment, respite care, specialized therapy, transportation, visiting nurse.

Person/Family Directed Support Waiver (P/FDSW)
