4. Accredited Case Manager (ACM)

The examination is available to registered nurses and social workers. However, there are specific eligibility requirements necessary to take the examination. These requirements include a blend of both experience and professional practice:

A Registered Nurse applicant must possess a valid and current nursing license. RN applicants must provide a nursing license number, state and expiration date.

Social Worker applicants must have a Bachelors or Masters degree from an accredited school of social work OR a valid social work license.

Social Worker applicants must provide the degree, name of school and year of completion OR a current social work license number, state and expiration date.

All applicants must have at least two (2) years of full time experience in Hospital/Health System Case Management or 4,160 work hours in Hospital/Health System Case Management.

If you hold both a RN and SW license, you must indicate which exam you wish to take and provide the applicable eligibility documentation.

9. Certified Case Manager (CCM)

To be eligible for certification, you must hold a license or certification which meets two criteria:

1st At a minimum, the license or certification must be based on an educational requirement of a post-secondary degree program in a field that promotes the physical, psychosocial, or vocational well-being of the persons being served. A post-secondary degree is defined as any nursing school, college or university diploma obtained after graduating from high school (nursing diploma or associate’s, bachelor’s, master’s or doctorate degree).

2nd The license or certification must grant you the ability to legally and independently practice WITHOUT THE SUPERVISION OF ANOTHER LICENSED PROFESSIONAL.

To be eligible for certification, you must qualify under one of these categories:

Category 1- 12 months of acceptable full-time case management employment experience supervised by a Certified Case Manager (CCM®). Supervision is defined as the systematic and periodic evaluation of the quality of the delivery of the applicant’s case management services.

OR Category 2- 24 months of acceptable full-time case management employment experience. (Supervision by a CCM® is not required under this category).

Category 3- 12 months of acceptable full-time case management employment experience as a supervisor of individuals who provide DIRECT case management services.

by CCMC in determining your eligibility for certification. However, internship, preceptor-ship, practicum, and volunteer activities are NOT acceptable employment experience. All part-time employment experience will be pro-rated based on a 37 hour full-time work week.

IS MY EMPLOYMENT EXPERIENCE ACCEPTABLE?

For your employment experience to be accepted, it must meet the following six conditions:

1st You must be licensed or certified to practice independently during your work experience in order for it to be accepted. Any employment experience you had while not licensed or certified, is not acceptable and will not count.

2nd Your employment experience must be fully satisfied before the application deadline (see section 2).

3rd Your employment experience must be focused primarily on case management practice and occupy at least 50% of the time.

4th In your employment, you must perform all 8 Essential Activities with direct client contact.

5th The work you do, must be performed within at least five of the six core components of case management.

6th Within each of the five of the six core components, you select:

1. You must perform all 8 Essential Activities. 2. You must provide services across a continuum of care, beyond a single episode of care, that addresses the ongoing needs of the individual being served. 3. You must be responsible for interacting with other relevant parties within the client’s healthcare system. 4. You must be primarily responsible for dealing with the client’s broad spectrum of needs.

10.

Chronic Care Professional(CCP)

Recommended for non-clinical team members such as community health educators, program leaders or consultants who support chronic care improvement.

ANY Professional working in health care or a health care related field is eligible for CCP certification.

Nurses and advanced practice nurses, case managers, physicians, pharmacists, dieticians, rehabilitation therapists, social workers, psychologists, respiratory therapists, exercise physiologists, among others, have all completed CCP certification.

CCP certification is also often recommended for non-clinical team members such as community health educators, program leaders, or consultants who also support health and chronic care improvement. In rural settings and developing countries, CCP certification can prepare informal community

Certification in Critical Care Nursing (CCRNS)

Current unencumbered licensure as an RN or APRN in the U.S. is required. Practice as a registered nurse is required for 1,750 hours in direct bedside

care of acutely and/or critically ill patients during the 2-year period prior to application, with 875 of those hours accrued in the most recent year preceding application.

The CDMSC will accept only full-time paid employment (or its equivalent in part-time work) as a valid fulfillment of the employment criteria. All part-time employment will be calculated on a pro-rated basis of 37 hours per week (full-time employment as defined by the Commission). Internships and volunteer activities are NOT acceptable as employment experience.

In order for your employment experience at any position (past or current) to qualify as “acceptable” for certification purposes, at least 60% of your time must be/must have been spent in at least two of the three job-function areas below, performing disability management services to individuals with disabilities receiving benefits from a disability compensation system. Your supervisor, employer, or purchaser of services will be asked to attest to this on the Employment Verification Forms.
