S Recommend approval of 2018-2019 Child Study Team and student matters as per Mr. Michael
Scapellato:

1. Recommend approval for student #3508909716 to attend Burlington County Special
Services School District at $224.58 per diem (transportation not included).

1. Recommend approval for an independent central auditory processing evaluation
for student #6099328814, to be completed by REM Audiology. Per district policy
2468, the maximum allowable expense to the district for this independent evaluation
will not exceed $500.00. The total cost for the evaluation is $595.00. As such, the
parent will be responsible to pay the remaining $95.00 for this independent central
auditory processing evaluation in the form of a money order, made payable to the
City of Burlington Board of Education. The City of Burlington Board of Education
will then remit payment to REM Audiology after this independent evaluation is
completed.
2. Recommend approval of the 2019-2020 fee schedule for the Educational EXHIBIT PR1
Services Unit.
3. Recommend approval for 10 hours of bedside instruction for Student #6137960356,
provided by the Bridgeton Board of Education from January 7, 2019 through
January 11, 2019, not to exceed $300.00.
4. Recommend approval for bedside instruction for student #5378976201, provided by
Brookfield Schools from January 22, 2019 through March 15, 2019, not to exceed
$2,808.00.
