GLOSSARY OF TERMS

32

Copayment (copay): The amount you pay per visit to the doctor or a health care facility. This is generally a flat dollar amount that is paid at the time of service. The amount can vary based on your plan coverage terms and the type of covered health service you receive.

Deductible Amounts: This is the amount of covered expenses that you must pay before the plan pays any benefit. Once you meet this threshold, the plan will begin to pay benefits for covered expenses that you incur; this applies to both individual and family plans.

Coinsurance: A percentage of the cost of covered medical services you must pay after you have met any plan deductible. Coinsurance is a percentage of the cost of the service post Cigna discount (if you stay in-network). It does not include charges for services not covered by your plan.

In-network: Health care professionals or facilities that have signed contracts with Cigna and may therefore offer discounts for health services when you receive care and directly bill Cigna.

Out-of-network: Health care professionals or facilities that do not offer discount arrangements for services with Cigna and may require that you pay for services at the point of care. You may visit any health care facility you choose by choosing a doctor who does not participate in the Cigna Network may lead to higher out-of-pocket cost.

FREQUENTLY ASKED QUESTIONS

34

MBA Program – FAQMBA Program – FAQ

35

Q: Do I need a Cigna ID card?A: Yes. Your Cigna ID card is recognized by most providers around the world. By using your Cigna ID card, we can directly reimburse the doctor or hospital where you received care. When you receive your permanent ID card, pleaseverify your information is correct and call Cigna immediately if a change is required. Present your ID card whenever you receive services from a provider.

Q: Is my Cigna ID card a credit or payment guarantee card?A: No. Your Cigna ID card is purely a means of identifying you. It has no payment capabilities. You should contact us for payment guarantees or questions.

Q: Do I need to select a primary care physician (PCP)?A: You are not required to select a PCP. However, it is recommended that you establish a relationship with a personal doctor, such as a family practitioner or an internist, in advance of requiring care. A personal doctor will care for you and your covered family members, including routine physical exams, sick visits and follow-up care. They can also provideinformation and guidance when selecting specialists. They will become a valuable resource and can be a personal health coach for you and your covered family members.

Q: How can I locate a doctor and how do I search for providers in Cigna’s global direct settlement network?A: With a network of more than one million providers worldwide, it’s easy to locate a doctor or hospital. To locate an international provider, go to CignaEnvoy.com and click on the “Find health care” tab or call us using the number on your ID card for assistance. You do not need to submit a claim if you visit a healthcare professional that has a direct billing arrangement or has obtained a Guarantee of Payment from Cigna. In these scenarios, the provider will file the claim directly with Cigna – reducing the need for you to submit any paperwork or pay in full upfront.

MBA Program – FAQ

36

MBA Program – FAQQ: What if my doctor is not in Cigna’s international network?A: You can see any licensed doctor in your assigned country. Cigna will be able to support you with our Guarantee of Payment process and reach out to your doctor directly to initiate the payment. If the provider is unwilling to directly settle the claim with Cigna, you will need to submit a claim for reimbursement. You have the ability to submit claims directly through our customer website www.CignaEnvoy.com, via our Mobile App or by fax, mail/courier or email.

Q: How do I request a guarantee of payment from Cigna? A: Ask your doctor/hospital to call Cigna and Cigna will work with your doctor/hospital to directly settle the claim. The Phone number can be found on the back of your ID card. Emergency GOPs can be completed in 1-2 hours and non-urgent GOPs are turned around in roughly 48 hours. If a doctor/hospital requests a GOP, the confirmation will be faxed to them. If a member requested a GOP, Cigna can send the GOP to the hospital and member.

Q: How do I get my prescriptions filled while I am away?A: If you receive a prescription from a local doctor while working outside of your home country, you can have it filled locally. If you have any questions, please contact Cigna using the number on your ID card. Our customer service team will help you identify available options. Please be aware medications can only be filled locally in the country where the prescription is written. For example, if you have a medication prescribed by a doctor in China, it cannot be filledin the United States (U.S.). Likewise, a prescription written in the U.S. cannot be filled in a pharmacy outside of the U.S.

Q: What if I have a medical emergency?A: Should something serious happen, visit the nearest hospital and contact the global service center immediately. The professionals at our global service center will help you get the emergency assistance you need. From ground transportation and translators to finding a specialist and facilities, we’re here to help.

37

MBA Program – FAQQ: What types of reimbursement options are available if I pay out of pocket? A: You can receive a variety of reimbursements in over 135 different currencies. Depending on the currency requested, Cigna can reimburse via check, ePayment Plus®, direct deposit, and EFT. Wire transfer is another option, but is suggested only for large claims since there are lifting fees typically associated with the transfer, which would become your responsibility to pay.

Q: If I had to pay out of pocket, what is the quickest and easiest way to be reimbursed? A: Cigna suggests submitted any claim that has been paid out of pocket via the website and/or mobile application. By submitted electronically via mobile app, you do NOT need to fill out a claim form and once you load your preferred reimbursement method and banking details online, they will be saved. You may also check the status of your claims online and track all submissions.

Q: What is an Explanation of Benefits (EOB) and how can I check on my claim status?A: Your EOB is a summary of how your claims were processed and what you may owe, not a bill. Your provider or the facility may bill you directly for the remainder of what you owe. To view your claims status, follow these steps:

1. Log in to CignaEnvoy.com.2. Select “Check my claims.”3. Select “View all claims.”4. Under Explanation of Benefits, select “View” to see the specific EOB you are looking for.

Q: Are there any pre-existing condition limitations or waiting periods with the new Cigna program?A: No. You may access healthcare starting January 1st 2019 under the new program with no limitations on your pre-existing health conditions nor do you have to satisfy any waiting periods.
