Notice to Vendors
The Choctaw County School System seeks a program evaluator to evaluate the Choctaw School Climate
Transformation Project. The evaluator shall provide the following services:
1. process evaluation to determine the fidelity between the program in theory and the program in
practice,
2. advocate for a utilization-based approach to evaluation with built-in feedback as a conduit to
translate evaluation results into program improvement,
3. provide an evaluation plan and consultations, and
4. provide timely evaluation reports

proposals shall be submitted in accordance with the solicitation document available from the
previous cited sources. All proposals shall be completed in accordance with the accompanying

1.0 Introduction
This RFP is issued by the Choctaw County School System. The system seeks a highly qualified
individual, group, or company to contract as an independent external evaluator for the CSCTP.
The Choctaw County School System is responsible for negotiating contracts for professional
educational services in accordance with applicable state and federal laws and regulations. The
selected individual, group, or company may have no vested interest in CSCTP. Family members
of school system employees and/or program partners are excluded from participation in this
RFP.
1.1 Primary Project Goals and Objectives
The Choctaw County School System’s vision is to equip and inspire all learners to reach their
highest potential. This includes providing positive learning environments. The project seeks to
reduce problematic behaviors, including alcohol and drug use, improve school climate, positive
relationships, and mental wellness within all schools in the district.
2.0 Scope of Work
The evaluator will provide both process evaluation to determine fidelity between the program
in theory and the program in action and outcome evaluation to assess the program’s progress
towards expected outcomes. The evaluator will advocate for a utilization-based approach to
evaluation with built-in feedback as a conduit to translate evaluation results into program
improvement. This individual or team of individuals must ensure capacity to provide timely
evaluation and offer readily available assistance or consultation when required. The evaluator
must also provide timely reports and summaries of program progress to stakeholders.
2.1 Required Skills and Experience
Bachelor of Arts or higher in Statistics or Education, Psychology, Counseling, or Applied
Behavior Analysis with course work in research methods, evaluation and /statistics or,
Masters or PhD student working on Program Evaluation certification,
Proven experience with program evaluation,
Operational knowledge of Positive Behavior Interventions and Supports, and
Perform evaluation in an ethical manner with integrity.

2.2 Responsibilities
Become and remain aware of the social, political, and educational context of the
community as identified in the grant,
Provide support in gathering quantitative and qualitative data on students in the
program,
Analyze all gathered data and provide recommendations for continuous program
improvement,
Conduct on-site monitoring,
Collect data in a way that causes little to no program interruptions,
Work with district technology department to collect and handle data in a confidential
manner – adhere to district guidelines and Family and Educational Rights and Privacy
Act,
Solicit feedback from project director, project manager, principals, and PBIS team to
better understand data by determining contextual issues that might impact data or
reporting,
Prepare a local evaluation plan on grant specific goals, as well as the Federal and State
guidelines,
Describe and document procedures, data sources, and rationales in detail to ensure
reliable and valid conclusions and/or recommendations without bias,
Submit reports in a timely manner,
Correct report errors in a timely manner,
Provide “next steps” (action plan suggestions) with each conclusion,
Analyze quantitative and qualitative data on students in the general population
(comparison group data) from student data systems, and surveys/focus groups to
identify specific areas for program improvement,
Design the evaluation to provide change-over-time data to assess program effectiveness
that will guide continued program improvement,
Assist project manager with Institutional Review Board (IRB) rules and regulations as
well as the rules and regulations of the local school system, if necessary,

Develop survey questions that are useful but also sensitive to culture, socio-economic
status, and family situations,
Make executive summaries of reports available to stakeholders,
Communicate executive summaries of reports to the local Board of Education, and
Provide assistance in developing assessment tools, as needed

3.0 Overview of Procurement Process
3.1 Authority
This procurement is being conducted as a competitive sealed proposals procurement in
accordance with the procedures set forth in this RFP.
One of the objectives of this RFP is to make proposal preparation easy and efficient, while
giving vendors ample opportunity to highlight their proposal. The evaluation process must also
be manageable and effective. When a vendor submits a proposal, it shall be considered a
complete plan for accomplishing the tasks described in this RFP and any supplemental tasks the
vendor has identified as necessary to successfully implement its proposed plan. Vendors, by
submitting proposals under this RFP, must agree to the selection and procurement procedures
specified herein.
3.2 Contract Period
The Choctaw County School System will negotiate the contract in accordance with applicable
state and federal laws and regulations. The contract period shall commence upon execution of
the contract by Choctaw County’s Superintendent of Education, and shall end on September 30,
2021.
The contract may be renewed annually three additional school years, 2021-2022, 2022-2023,
and 2023-2024 depending upon grant renewal and vendor service. Likewise, the contract may
be cancelled within thirty days (30) by either party given written notice and after thirty (30)
days for just cause by either party given written notification.
3.3 Special Conditions
The Choctaw County School System may choose to include special conditions in the contract.
Nothing in the special conditions of the contract shall supersede the general conditions, but
shall serve to supplement the general conditions, except where a conflict exists between the
general conditions and special conditions, in which case the special conditions shall apply.

3.5 Written Questions and Responses
Vendors shall carefully review this solicitation for defects and questionable or objectionable
matter. If a prospective vendor wants a formal answer to questions concerning this RFP, the
prospective vendor shall submit such questions in writing to the procurement officer specified
in “Issuing Office and Contact Person”, provided that questions are received no later than the
time and date specified in “Significant Dates.”
Such documents shall contain pertinent information to identify the prospective vendor, its
telephone number, e-mail address, the RFP, and be addressed to the procurement officer. The
Choctaw County School System will not respond to verbal or informal questions.
To facilitate a meaningful response, written questions shall reference the page, paragraph, and
line or sentence of the RFP to which the question relates.
Official responses will be e-mailed to all prospective vendors who submitted a timely
registration form. The Choctaw County School System is not responsible for delays or non-
receipt of such responses or any communications by prospective vendors. Response(s) to the

vendor written question(s) shall be compiled, shall omit reference to the source(s) of the
question(s), shall be issued as an addendum to the RFP, and shall become a part of the RFP.
If a vendor submits a question after the scheduled date, the Choctaw County School System
may answer the question but does not guarantee that the answer will be provided prior to the
proposal due date. Question and answer sets may be provided to all vendors. The Choctaw
County School System may publish the questions as they are submitted including any
background information provided with the question; however, the Choctaw County School
System, at its sole discretion, may paraphrase the question and background content for clarity.
3.6 Submission of Proposals
Each vendor shall submit only one (1) proposal. Alternate proposals will not be accepted.
Each vendor shall submit two (2) copies and one single-sided unbound original of the vendor’s proposal (for a total of three [3]).
Proposals shall be received and time-stamped by the Issuing Office no later than the time and date specified in “Significant Dates.” Any proposal received after that time and date will be rejected. The original proposal shall be single sided, unbound, and clearly marked, “Original.” Each copy shall be marked, “Copy ____ of _____.” Copies may be bound and double-sided.
FAXED OR E-MAILED PROPOSALS SHALL NOT BE CONSIDERED FOR AWARD.
Proposals shall be mailed or hand delivered to the Issuing Office (See “Issuing Office and Contact Person” for address information).
The outside cover of the package containing the proposal shall include the name and address of the vendor, and be marked:

3.7 Proposal Opening
Proposals shall not be opened publicly, but shall be opened in the presences of two or more
school system leadership team officials.

3.8 Costs for Proposal Preparation
All costs incurred by the vendor in preparing and submitting a proposal and conducting Due
Diligence and Contract Discussions shall be at the vendor’s sole expense and are the vendor’s
sole responsibility. This includes the cost of any visits to Choctaw County School System
locations by the vendor, but does not include any costs incurred by the Choctaw County School
System or its representatives for vendor demonstrations or site visits, if any.
3.9 Disqualification of Proposals
The Choctaw County School System reserves the right to consider as acceptable only those
proposals submitted in compliance with all the requirements set forth in this RFP and which
demonstrate an understanding of the issues involved and the scope of work. Any proposal
offering any other set of terms and conditions, or any terms or conditions contradictory to
those included in this RFP, shall be disqualified.
A vendor shall be disqualified, and the vendor’s proposal shall be rejected for any one or more
of the following non-exclusive reasons as solely determined by the Choctaw County School
System:
1. Proposal is received after the established proposal submission deadline.
2. Proposal is not properly completed or signed by an authorized individual.
3. More than one proposal is received from an individual, firm, corporation or joint
venture under the same or different names.
4. Proposal is submitted on a form other than that furnished by the Choctaw
County School System, or an exact copy thereof.
5. Failure to possess proper licenses, facilities, equipment or sufficient experience
to provide the proposed solution.
6. Proposal shows any noncompliance with applicable laws, alterations of form,
conditional proposals, incomplete proposals, un-initialed erasures, other defects,
or prices which are obviously unbalanced.
7. Failure to submit written answers to any questions from the Choctaw County
School System.
8. If there is evidence to Choctaw County School System sole satisfaction that
collusion exists among vendors, any or all proposals may be rejected and the
participants in such collusion shall be barred from future proposal submissions.

3.10 RFP Amendments
The Choctaw County School System reserves the right to amend this RFP at any time prior to
the closing date for proposals. Any amendment will be issued in writing, and will be distributed
to vendors.
3.11 Cancellation of RFP; Rejection of Proposals
This RFP may be canceled and any or all proposals may be rejected in whole or in part, when it
is determined to be in the best interest of the Choctaw County School System.
3.12 Selection and Execution of Contract
Award, if any, shall be made to the responsible vendor whose proposal is the highest scoring
and most responsive to the RFP. A minimum score of eighty (80) points will be necessary to be
considered for an award. Failure to achieve the minimum score may result in a finding that the
proposal is non-responsive. If an award is made, the successful vendor will be required to enter
into a formal written contract with the Choctaw County School System. This RFP and the
successful proposal will be incorporated in the resulting contract by reference; to the extent
that they conflict, the terms of the RFP shall govern, unless otherwise agreed upon by the
Choctaw County School System. Upon the discretion of the Choctaw County School System a
single award or multiple awards may be issued.
3.12 Representations of the Provider
The vendor is advised that if awarded a contract under this RFP, vendor shall make the
representations contained in this paragraph, and the Choctaw County School System shall rely
upon such representations as a material inducement to entering into this contract:
Compliance with Laws. As of the date of the proposal, the vendor complies with all
federal, state, and county laws, ordinances, codes, rules, and regulations, as the same
may be amended from time to time, that in any way affect the vendor’s performance.
Licensing and Accreditation. As of the date of the proposal, the vendor holds all licenses
and accreditations required under applicable federal, state, and county laws,
ordinances, codes, rules, and regulations to provide the required services.

4.0 Proposal Instructions
4.1 Proposal Preparation
One of the objectives of this RFP is to make proposal preparation easy and efficient, giving
vendors ample opportunity to highlight their proposals. The evaluation process must also be
manageable and effective. When a vendor submits a proposal, it shall be considered a complete
plan for accomplishing the tasks described in this RFP and any supplemental tasks the vendor
has identified as necessary to successfully meet the obligations outlined in this RFP.
The proposal shall describe in detail the vendor's ability and availability of services to meet the
primary project goal of this RFP as stated in “Primary Project Goals and Objectives.” Proposals
shall be prepared in a straightforward and concise manner, in a format that is reasonably
consistent and appropriate to the purpose of this RFP. Emphasis should be on completeness
and clarity of content. If any additional information is required by the Choctaw County School
System regarding any aspect of a vendor’s proposal, such shall be provided within two (2)
business days of the Choctaw County School System’s request.
This section prescribes the standard format for a proposal submitted in response to this RFP.
The standard format will make it easier for the Choctaw County School System to review,
compare, and evaluate proposals, and determine minimum requirements. The format is not an
attempt to limit the content of a proposal in any way. The vendor may include any additional
data or information that is deemed pertinent to this RFP. However, as noted above, a proposal
should be prepared simply and economically, providing a straight forward and concise
delineation of the vendor's ability and proposed solution to satisfy the requirements of the RFP.
The contents of any proposal shall not be disclosed during the review, evaluation, or discussion
process. Once the notice of the award is posted, all proposals (both successful and
unsuccessful) become available for public inspection. Those parts of the proposal that a vendor
desires to remain confidential after the notice of award is posted must be identified and
marked in accordance with “Confidential Information” of the RFP.
Submission of a proposal shall constitute an incontrovertible representation by the vendor of
understanding, acceptance, and compliance with every requirement of this RFP, and that the
RFP documents are sufficient in scope and detail to indicate and convey reasonable
understanding of all terms and conditions of performance of the work.
Before submitting a proposal, each vendor shall:
Examine the solicitation documents thoroughly. (Solicitation documents include this
RFP, any attachments, and any other relevant documentation.)

Become familiar with state, local and federal laws, ordinances, rules, and regulations
that may in any manner affect cost, progress, or performance of the work specified
herein.
The proposal shall:
1. Include a transmittal letter to confirm that the vendor shall comply with the
requirements, provisions, terms, and conditions specified in this RFP.
2. Be organized into sections following the outline below using all titles, subtitles, and
numbering, with tabs separating each section described below. Each section must be
addressed individually and pages must be numbered sequentially for the entire proposal
(not by section).
3. Include the entity’s General Education Provisions Act (GEPA) statement.
4.2 Proposal Transmittal Letter
A proposal transmittal letter must be:
Signed by individual(s) authorized to legally bind the vendor,
Dated, and
Affixed with the corporate seal, if any
If the vendor is a corporation, evidence in the form of a certified copy of a corporate resolution
or certified copy of articles of incorporation or bylaws, or applicable sections thereof, shall be
submitted showing the individual's authority to bind the corporation. If the vendor is a
partnership or joint venture, each member of the partnership or joint venture must sign the
proposal letter, or evidence, in the form of a partnership agreement or joint venture
agreement, or applicable sections thereof, must be submitted showing that the individuals
signing the proposal transmittal letter have the authority to bind the partnership or the joint
venture. If the vendor is a limited liability company (LLC) or other type of entity, evidence in
the form of a certified copy of the articles of organization, operating agreement, other official
document, or applicable sections thereof, shall be submitted showing that the individual(s)
signing the proposal transmittal letter have the authority to bind the partnership or the joint
venture. The fully executed proposal transmittal letter must be submitted along with the
vendor proposal. (Note: Companies with no previous Alabama experience shall not be
penalized.)
The proposal letter must be on the vendor's official business letterhead and shall contain the
following:

1. Contact Person - The name of a duly authorized person that the Choctaw County School
System is to contact regarding vendor’s proposal, and that person’s address,
telephone/fax numbers, and email address.
2. Terms and Conditions - A statement that the vendor understands and shall comply with
all terms and conditions of the RFP. If a vendor does not plan to comply with one or
more of the terms or conditions of the RFP, this must be stated and all exceptions listed
and fully described.
3. Assumptions or Constraints - A statement on whether the proposal contains any
assumptions or constraints and identifying and describing each such assumption and
constraint. If neither assumptions nor constraints are included in the proposal, a
statement to that effect must be made.
4. Deviations - If the proposal deviates from the specifications or requirements of the RFP,
a statement must be included identifying and describing each such deviation. If no
deviations are included in vendor’s proposal, a statement to that effect must be made.
5. Subcontracting - A statement that the products and services of the proposed solution
shall be provided solely by the vendor or whether a subcontractor(s) shall assist. The
vendor’s use of subcontractor(s) requires the prior written approval of the Choctaw
County School System.
6. Table of Contents – The table of contents shall clearly identify the material by section
and by page number.
4.3 Vendor Qualifications
Staffing Requirements
A successful vendor must demonstrate they have adequate personnel to provide the required
service. The vendor shall review and identify areas of staff expertise that align with the
identified areas of service. Expertise shall include but not be limited to:
1. EXPERIENCE AND PROFESSIONAL QUALIFICATIONS
Key staff possess the required certification/licensure,
Key staff possess specialized expertise relative to the services being sought,
Key staff possess experience on similar or related projects,
Key staff is well versed in the federal requirements governing privacy,

2. CAPACITY TO ACCOMPLISH THE WORK IN THE REQUIRED TIME
Applicant has available staff resources in relation to project requirements,
Applicant’s resources are available for the project requirements, and
Applicant has in-house resources and can provide direct on-site services as
appropriate.
4.4 Description of Program Evaluation Service
Interested vendors are required to submit a proposal detailing the program evaluation service
they offer and their skills and experience for fulfilling the responsibilities stated in the
Introduction. A responsive proposal must outline service package cost describing what is
included in the amount. Provide details of the complete cost of deliverable services, including
air and ground transportation, per diem, material, supplies, and any other costs that will be
incurred for such services.
The proposal shall follow the proposed outline below:
I. Proposal Transmittal Letter (Cover Sheet)
II. Proposal Narrative
A. Qualifications of Vendor/Staff: Identify and provide documentation of
vendor/staff expertise
B. Evidence of Required Knowledge and Skill: Describe previous program
evaluative service and how they assisted program improvement
C. Outline Areas of Service: Identify and describe all areas of service to be
provided and cost
III. General Education Provisions Act (GEPA). Submit a copy of the individual or
company’s GEPA Statement.
3.5 Certification
By submitting a proposal, the vendor certifies that:
The prices and cost data were arrived at independently, without consultation,
communication, or agreement with any other vendor, competitor, or Choctaw County
School System employee.

Unless otherwise required by law, the prices and cost data that were submitted have
not been knowingly disclosed by the vendor, directly or indirectly, to any other vendor
or competitor prior to the award of the contract.
No attempt was made or will be made by each vendor to induce any other person or
firm to submit or not to submit a price for the purpose of restricting competition.
3.6 Confidential Information
The vendor’s proposals and all information, data, and other material provided by the vendor to
the State shall be subject to the Information Act. The vendor shall designate in writing to the
Procurement Specialist those portions of its proposal that contain trade secrets or other
proprietary data that are to remain confidential. The vendor shall state in a written
communication to the Procurement Specialist, the reasons for designating the material as
confidential. The vendor shall submit the material designated as confidential in such manner
that the material is clearly marked “confidential” and readily separable from the rest of
vendor’s proposal in order to facilitate inspection of the non-confidential portion of the
proposal.
Vendors should note that they cannot designate their prices (anything in the cost proposal
form) as confidential. Following award of the contract, all respondents proposed prices will be
available for public disclosure upon request.
5.0 Proposal Evaluation
5.1 Introduction
The Choctaw County School System will conduct a comprehensive, fair, and impartial
evaluation of the proposals it receives in response to this RFP. Key to the completeness and the
impartiality of that process is a Review Team that will assign points to each proposal in
accordance with the evaluation criteria described in this section. Price is not the sole factor in
determining award of the contract.
Evaluation Organization
All proposals will be reviewed by the Choctaw County School System. It is anticipated that for
this RFP, the Review Team will be designated from the Choctaw County School System.

Evaluation Criteria
Evaluation criteria and the associated points are specified herein. The award will be made to
the responsive and responsible vendors whose proposal is determined to be the most
advantageous to the Choctaw County School System based on the evaluation criteria listed in
this section.
Review Criteria
The evaluation of proposals received in response to the RFP will be conducted
comprehensively, fairly and impartially. Structural, quantitative scoring techniques will be
utilized to maximize the objectivity of the evaluation.
Evaluation Process
A Review Team selected by the head of the purchasing agency or procurement officer shall
review and evaluate proposals.
