NEW and IMPROVED Leave Programs – effective January 1, 2014With our core value of Putting People First, CHG is committed to understanding how life events can impact our people. Our goal is to offer leave programs that provide solid financial support to all of our employees during these events. Based on feedback from our annual Employee Opinion Survey and a focus group, we are excited to introduce a new Cigna short-term disability benefit and CHG paid leave program.

Cigna Short-term Disability Benefit — All EmployeesThis Cigna-administered plan provides supplemental coverage in the event of a disabling illness or accident and can also be used to cover part of your maternity leave.

What you should know about the new short-term disability benefits:

• Now paid 100 percent by CHG

• Provided to all employees beginning the first day of employment

• All current employees and new hires will be signed up automatically

• No preexisting conditions will apply to people who are employed as of January 1, 2014

• Provides 70 percent of employee’s income (regular pay, including commissions but not annual bonuses) up to a $2,500 weekly maximum

• After taking five days of either paid time off (PTO) or unpaid leave, employee’s short-term disability benefits will begin

• Provides coverage for approved disability up to 12 weeks*, after which long-term disability is available for approved leave

• Covers multiple events within the same year

• Short-term disability will be paid weekly with taxes and benefits deductions applied

*The length of time you will receive short-term disability pay is based upon your condition.

CHG Paid Leave Program — First Year of EmploymentDuring your first year of employment, you are not protected by FMLA, which means that while you are out of the office for either maternity leave or other conditions, such as surgery, your job is not protected through employment law. Also during this time period, you are not eligible for the CHG paid leave program. However, you are eligible to receive short-term disability providing that you are not pregnant and have not received a diagnosis or received care for an illness during the previous three months prior to your hire date. (Important: This does not apply to employees who are employed prior to January 1, 2014.)

• Maternity and Medical Leave After taking five days of either PTO or unpaid leave, an employee may be eligible to receive a total of six weeks approved short-term disability pay for maternity or medical leave.

The following example is for both maternity and medical leave within the first year of employment based on a leave date of January 2 due to an illness or a normal delivery.*

CHG Paid Leave Program — More Than One Year of EmploymentIf you have worked for CHG for more than one year, you are eligible to receive paid time off for the following life events:

• Maternity Leave After taking five days of either PTO or unpaid leave, employees are eligible to receive short-term disability pay. Once short-term disability pay is exhausted (typically five weeks for a normal delivery), CHG will provide three additional weeks of paid maternity leave at 100 percent of regular pay, including commissions but not annual bonuses. Additional PTO or unpaid leave can be taken to cover up to 12 weeks of approved leave under the Family and Medical Leave Act (FMLA).

The following example shows a total of 12 weeks of FMLA-approved maternity leave, eight of which are paid by short-term disability and the paid leave program, and is based on a normal delivery* date of January 2.

• Paternity Leave After taking five days of either PTO or unpaid leave, CHG will provide employees up to one week of paid leave for an approved paternity leave. PTO or unpaid leave can be taken to cover additional leave time.

• Foster Care Placement Leave After taking five days of either PTO or unpaid leave, CHG will provide employees up to one week of paid leave for an approved foster-care placement. PTO or unpaid leave can be taken to cover additional leave time.

• Adoption Leave After taking five days of either PTO or unpaid leave, CHG will provide mothers up to five weeks of paid leave for an approved adoption leave. PTO or unpaid leave can be taken to cover additional leave time.

• Medical Leave After taking five days of either PTO or unpaid leave, employees with more than one year of employment are eligible for up to a total of 12 weeks* of approved short-term disability pay for a medical leave. After the 12 weeks of short-term disability pay is exhausted, employees will be eligible to apply for long-term disability coverage.

*The length of time you will receive short-term disability pay is based upon your condition.

The following example shows medical leave for an employee with more than one year employment based on a January 2 leave date.

Grandfathered Extended Illness Bank (EIB) Transitional PeriodThere is a small group of employees whose Extended Illness Bank (EIB) would provide a better benefit for a leave than the new short-term disability and paid leave program. Therefore, CHG has decided to grandfather all EIB balances as of December 31, 2013 for a three-year transitional period. Benefits specialist Melissa Jones will be available to help employees decide whether EIB or short-term disability will provide the most comprehensive benefit while on leave.

• EIB will no longer accrue effective January 1, 2014, but available balances will show on employee paychecks.

• During the three-year transitional period, after taking five days of either PTO or unpaid leave, EIB will be available for:

– Care of a family member – FMLA-approved leaves – Intermittent FMLA leaves until EIB is exhausted

• On December 31, 2016, any unused EIB will no longer be available.
