Date of loss (DOL) How the accident happened including any injuries or damage Type of OPW equipment involved Contact information of claimant Ask questions while accident is fresh Take photos, if possible Do not make any statements about fault or make any

commitments to anyone You are only there to collect information

Ask the owner to remove the equipment from service and preserve properly Do not send an email – Call the Risk Manager immediately at 513-382-9835 Send your report/notes via US mail No opinions please Anything written down may end up in court!
