Must accomplish a Memorandum ofUnderstanding from the College Secretary

Must be on On-the-Job Training for aminimum of 180 hours

Must be 100% in attendance duringpreliminary, final and emergency defense of

the topic to earn a grade of S.

Topics to be considered during the defenseshould be in line with the courses already

REQUIREMENTS DURING THEPRACTICUM PERIOD 

Journal of daily activities involved in thepracticum. Each daily activities noted in

the journal should be countersigned byyour direct supervisor.

REQUIREMENTS DURING THEPRACTICUM PERIOD 

Documentation of all possible dailyactivities that can be observed using

digital photographs. Actual date takenshould be included in the photographs.

REQUIREMENTS AFTER THEPRACTICUM PERIOD 

Accomplishment of the “Evaluation of onthe Job Trainee” Form by the direct

supervisor. This form will be sealed in anenvelope and should be countersignedby the evaluator.

REQUIREMENTS AFTER THEPRACTICUM PERIOD 

Certificate of Completion of the requirednumber of hours of field practice to be

included in the Appendices of the finalmanuscript.

Attend the final topic defense scheduled

by the department.

Grading

A grade of S for every 2.0 or 3.0 unitsregistered (except last unit/s) if the

minimum requirements for that semesterwere accomplished.

A grade of U if minimum requirements

were not met for no valid reason.

“What happens when I get agrade of U?” 

Required to retake the units where agrade of U was given.

“What if I have already registered the

6.0 units but still unable to finish themanuscript?

Register 1.0 unit per term until able tosubmit the approved final manuscript but

only up to a maximum of 3 terms forundergraduate thesis (total of 9.0 units)

If still not able to submit the finalmanuscript at the end of the time limit, agrade of U will be given and should re-enroll all the 6.0 units.
