Educating clients and caregivers at varying degrees of education and experience in community settings on disease process, medication management, fall prevention, and utilization of community resources to provide client assistance. Timely completion of documentation using comprehensive Cerner Roadnotes application utilized for managing Physician orders, vital signs, client assessments, and OASIS forms. Genesis Health Care – October 2012- September 2014. Staff RN/Charge Nurse – Responsible for total care of patients in Sub Acute, Long Term, and Alzheimer/Dementia units. Duties included admission assessment and paperwork, discharges, administration of medications via PO, IM, SQ, IV, PR, PEG, and TD. PICC line care and maintenance, G-tube and PEG tube care. Straight catheterization and Foley insertion and care. Colostomy, Ileostomy, Nephrostomy, and Urostomy care. Trach care and suctioning. Wound care, including wound vacs. Medication reconciliation. Complete medical assessments. Coordination of care with staff through complete and clear shift reports. Collaboration with MDs, RNs and other medical specialists in the treatment and execution of care for patients. Caregiver for special needs child – 2000 – July 2015. Provided assistance with ADLs and IADLs, coordination of care with specialists, home physical and speech therapy support, and medication administration. Learned the true meaning of advocacy. Developed expertise in coordinating, managing, and follow-up with multiple health care professionals and other professional and social organizations.
