Check your online Scorecard for available Health Actions, which may include:

Log into Rally and take the Health Survey = 10%

Complete a biometric screening, this may be done at an onsite event with CBIZ or your doctor’s office Biometric Screening = 10%

Complete a preventive exam Preventive Annual Exam = 25%

Meeting biometric target values (LDL cholesterol, blood sugar, blood pressure, body mass index) LDL Screening = 5% Achieve BMI target = 20% Blood Sugar = 10% Target Blood Pressure = 10%

Tobacco or nicotine free = 10%

These are just some of the ways you can earn percentages
